Available in PDF, EPUB and Kindle. Book excerpt: This survey polled preferences for dental continuing education (C.E.) topics among non specialty-trained Army general dentists and their supervisors. Among general dentists, preferences for dental C.E. topics were found to vary across level of training, assignment location, experience, and presence of specialists in clinic of assignment. C.E. preferences of general dentists were found to be more narrowly clinically focused than those that their supervisors think they need. All groups ranked Management of Medical Emergencies as the most needed dental C.E. topic. The current inventory of Army dental C.E. courses is meeting the needs expressed by survey respondents with the exception of human relations courses. Topics such as Motivating Co-Workers, Management of Job Stress, and Working with Difficult People, which received high ratings in this survey, should be incorporated into the Army dental C.E. system. Book excerpt: The proportion of the normal duty day devoted to direct and indirect patient care, continuing education, other duties, and absences by Army dental officers who are mentors, residents, or non-teaching specialists has been determined in an eariler study. Not known was this same information about Army dental officers who are full time clinicians. A survey was made among Army dentists of all ranks at ten different Army installations to determine this information. One hundred dentists submitted four weeks of data each. They noted the hours during both the normal duty day and and outside normal duty hours devoted to the various duty categories mentioned above. Results showed that Army dentists whose duty is full time clinician spent between 69 and 78 percent of their average work week providing direct patient care. Indirect patient care accounted for between 6 and 10 percent of their time during normal duty hours, and continuing education pursuits accounted for less than 2 percent of the normal duty week. The results of this study also indicated that Army dentists spend about four hours per week outside normal duty hours on duties related to their work, and that two-thirds of this time is for continuing education. Book excerpt: Currently, 85 Army officers pursue graduate dental education (GDE) in 28 different training programs. Programs were surveyed to determine the direct and indirect costs to the Army for its sponsored GDE. Total training costs exceeded $48.40 million to complete one Army residency training cycle (from 1 to 6 years in duration) or about $576,000 per resident. When the dental procedures and services provided by the residents and mentors involved with GDE are considered as generated revenue, Army GDE operates at a net profit (revenues minus costs) of just under $865,000 or $10,300 per resident. If the net profit per resident is divided by the mean specialty trained career length of 17.91 years of service, the amortized net profit of training equates to + $587 per resident per retained year. The hypothesis that outsourcing GDE to civilian institutions was refuted by the data collected. The Army Dental Care System (ADCS) does have more dental specialists in its inventory then needed to service its current population which perpetuates a higher than average dental specialist to generalist ratio. Readiness requirements mandate some programs, e.g., Advanced Education in General Dentistry (AEGD) 1 and 2 year programs as well as oral and maxillofacial (OMFS) residencies, must be continued regardless of the cost. To remain viable the other product line GDE programs must share their best business practices; to think of GDE as a business and not a luxury easily affordable to the military. Failing to focus on the bottom line, to show a profit, may lead to their extinction.
