Ces enfants ont un risque élevé de développer un syndrome d'apnées obstructives du sommeil (SAOS), parfois associé à une hypoventilation alvéolaire importante. L'objectif principal de cette étude est de comparer les échanges gazeux nocturnes (Pression transcutanée en dioxyde de carbone ou PtcCO2 et Saturation pulsée en oxygène ou SpO2) d'enfants atteints de Trisomie 21 à ceux d'un groupe témoin. Patients et méthodes : Etude rétrospective monocentrique de type cas témoins portant sur les polysomnographies (PSG) d'enfants atteints ou non de Trisomie 21. Les dossiers cliniques des patients ont également été étudiés afin de rechercher des signes cliniques de SAOS et des comorbidités associées à la Trisomie 21. Les cas et les témoins étaient appariés sur 2 paramètres : l'index d'apnées hypopnées (IAH) et l'âge. Résultats : 28 enfants atteints de Trisomie 21 ont été inclus dans cette étude, puis appariés à 28 témoins ayant bénéficié d'une PSG pour un SAOS clinique. La PtcCO2 moyenne au sommeil était significativement plus élevée dans le groupe Trisomie 21 comparé au groupe témoin (44.7 mmHg vs 42.4 mmHg, p = 0.0089), ainsi que le pourcentage de temps de sommeil passé avec une PtcCO2 supérieure à 50 mmHg (9.1 vs 1.4, p = 0.011). La SpO2 moyenne au sommeil était significativement diminuée par rapport au groupe témoin (95.7% vs 96.3%, p = 0.048). Discussion : Cette étude est la 1ère étude comparant les échanges gazeux d'enfants atteints de Trisomie 21 et ceux d'enfants non syndromiques, présentant ou non un SAOS à la PSG. Elle montre notamment que les enfants atteints de Trisomie 21 ont une PtcCO2 plus élevée, indépendamment du degré de sévérité d'un éventuel SAOS (mesuré par l'IAH). Les 2 hypothèses que nous formulons ici pour expliquer ces résultats sont la présence d'une hypotonie des muscles respiratoires et/ou l'existence d'une dysfonction du système nerveux autonome et donc de la commande ventilatoire. Perspectives : Une étude prospective serait intéressante afin de confirmer ces résultats. Elle permettrait également de rechercher chez les enfants atteints de Trisomie 21 es signes d'hypotonie et de dysautonomie ; et un éventuel retentissement neuropsychologique d'une hypoventilation alvéolaire.
