Book excerpt: Post-traumatic stress disorder (PTSD), generalized anxiety disorder (GAD), and depression are psychiatric disorders that interfere with daily-life activities. In Canada, the prevalence of PTSD is approximately 12%, 2.6% for GAD and 8% for depression. Patients with mental health disorders usually require pharmacological and/or psychological interventions such as cognitive-behavioral therapy, however approximately two-thirds of patients with major depressive disorder do not have adequate responses to conventional treatments. The purpose of this report is to review the clinical effectiveness of TMS for treating PTSD, GAD and depression, and to summarize the guidelines that are associated with the use of TMS for these conditions.

A potential alternative to pharmacological and psychological interventions are brain stimulation techniques such as transcranial magnetic stimulation (TMS). TMS is a non-invasive technique, whereby a small coil placed over a patient's scalp. The electric current circulating through the coil produces a magnetic field which can then pass through the scalp and bone and induce changes in nerve cell activity in the cortex. The purpose of this report is to review the clinical effectiveness of TMS for treating PTSD, GAD and depression, and to summarize the guidelines that are associated with the use of TMS for these conditions.

Book excerpt: The majority of literature on superficial brain stimulation for the treatment of psychiatric conditions is focused on transcranial magnetic stimulation (TMS) for major depressive disorder. Given its versatility and mode of action, TMS use has been now extended to other psychiatric disorders including anxiety disorders, bipolar disorder, psychotic disorders, and disorders of executive function. In this chapter we review the rationale and available evidence for the use of TMS as a treatment option in conditions other than major depression – post-traumatic stress disorder, obsessive–compulsive disorder, panic disorder, generalized anxiety disorder, attention-deficit/hyperactivity disorder, catatonia, schizophrenia, and bipolar disorder. Although the rationale for its use in the treatment of the above-mentioned conditions is strong, the available evidence is mixed and limited. At this juncture no definitive conclusions or recommendations can be drawn; however, given the existing positive signals and the significant limitations of the presented evidence, further research is warranted to assess the actual role of TMS in the treatment of psychiatric conditions other than unipolar depression.

Book excerpt: Post-traumatic stress disorder (PTSD), general anxiety disorder (GAD), and major depression disorder (MDD) are common debilitating disorders with a lifetime prevalence rate of approximately 12%, 2.6%, and 8% of Canadians, respectively. In addition to pharmacologic treatment, different treatment modalities such as psychotherapy, electroconvulsive therapy, transcranial magnetic stimulation, vagus nerve stimulation, deep brain stimulation, and neuro-linguistic programming (NLP), have been suggested for these disorders. NLP is a therapeutic technique used to detect and re-program unconscious patterns of thoughts and behavior in order to alter psychological responses. The creators of NLP have claimed that there is a connection between the neurological processes (neuro), language (linguistic) and behavioral patterns learned through experience (programming), and as such, it can be used in business, education, and medicine for conditions such as phobias, anxiety and depression. NLP is a multi-dimensional process usually comprised of steps such as building rapport between physician and patient, information gathering and desired health outcome definition, and use of techniques and tools such as sets of questions to facilitate a change in thinking and behavior of patients. Therapy sessions can take place weekly with the total number of sessions depending on individual needs. This Rapid Response report aims to review the clinical evidence of NLP for PTSD, GAD and depression. Evidence-based guidelines on the use of NLP for these disorders will also be examined.

Book excerpt: The Social Security Administration (SSA) administers two programs that provide disability benefits: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. SSDI provides disability benefits to people (under the full retirement age) who are no longer able to work because of a disabling medical condition. SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. Selected Health Conditions and Likelihood of Improvement with Treatment identifies and defines the professionally accepted, standard measurements of outcomes improvement for medical conditions. This report also identifies specific, long-lasting medical conditions for adults in the categories of mental health disorders, cancers, and musculoskeletal disorders. Specifically, these conditions are disabling for a length of time, but typically don't result in permanently disabling limitations; are responsive to treatment; and after a specific length of time of treatment, improve to the point at which the conditions are no longer disabling.
