There is a dearth of analytical research on estimating barriers to trade in health services, particularly in the context of South Asia, and as such, this book assesses the potential benefits and economic costs of barriers to trade in health services in select South Asian economies. It also analyzes the impact of liberalization and regulatory reforms on economic welfare. It broadly addresses issues relating to trade in health services, the GATS (General Agreement on Trade in Services), such as: Why are the current levels of trade in health services low? How will the GATS legally affect a country’s health policy? What effect might liberalization have on national health systems? And what are the likely benefits of greater trade in health services? It also provides specific answers to the following questions: Does the substantial role of the government in health – as health service provider, financial supporter, regulator and promoter – have implications for the treatment of the sector under the GATS? What is the impact of liberalization of international trade in health services on the quality and availability of health services in developing SAARC countries? Given the importance of consumption abroad for trade in health services, and the gradual opening of health markets through Modes 1 and 3 (cross-border supply and commercial presence), how can problems associated with trade in these Modes be prevented? And are these problems sufficiently addressed by GATS disciplines? Answers to these questions will be of great use to researchers, policy makers as well as practitioners and NGOs of South Asia.

Book excerpt: This study was conducted in three South Asian countries viz: India, Bangladesh and Nepal. In order to gain insights into the trade in health services in India, the study examines the opinions of patients, hospitals owners and doctors involved in this service trade. A total of 190 foreign patients from SAARC countries that came to India to receive medical services, 20 hospitals across five metros, and 65 doctors active in offering professional services were surveyed using three structured questionnaires. Two case studies were carried out on two countries (Bangladesh and Nepal) to examine the demand aspects of health service trade. This study also aims to profile the health service sector of these two countries which might affect trade in health services. It focuses on the liberalization of health services and assesses the challenges and opportunities posed by the liberalization of trade on the provisions of health services and options for national policy makers. It further makes an effort to examine the barriers in health service trade in the two relatively small countries of South Asia. An effort is made to explore possible regional cooperation in health service trade among the above three countries of the SAARC region. The evidence is actually useful in order to understand commitments and challenges regarding trade in health services in the SAARC region.

Book excerpt: This study aims to assess barriers to service trade in the education, health, telecom, transport and travel and tourism sectors of selected South Asian economies such as India, Sri Lanka, Pakistan, Nepal and Bangladesh, including both trade and domestic restrictions. The analysis is focused on computation of aggregate and modal service trade restrictiveness indexes (STRIs) by sector, drawing on information gathered based on detailed questionnaires. The conclusions highlight that while significant regulatory reforms have taken place in certain (for example Telecom) service sectors over the last decade, a broad range of restrictions still remain. The most significant change in these service sectors has been the lifting or softening of the constraints imposed on foreign equity participation. Interestingly, the economies in terms of regulatory reforms are more open to non-South Asian economies and not so much open to each other. However, reforms have had varying degrees of impact on market structure depending on the country and the sector. Based on findings of the study it is possible to argue that industrialisation is not the only route to economic development. Overall, technological changes (with the help of splintering and disembodiment of services) have made many services tradable as happened in the case of manufactured goods. Therefore, adequate reforms may not only remove the barriers to services trade but also generate higher services trade in the region, through which the region may achieve higher income and employment.

Book excerpt: Southeast Asia is developing into an area of extensive growth and opportunity. The growing middle class population and wealth in the region is attracting increased foreign direct investment in the region. Growing in importance is the trade and investment in healthcare services within the region. The reduction in barriers to trade has seen an increase in the mobility of healthcare services. The internationalisation of foreign healthcare providers into Southeast Asia involves a change in the contextual environment and therefore the potential need to adapt or localise certain aspects of service. The purpose of this study is to understand the extent to which foreign healthcare firms adapt their services and business process to the Southeast Asian context. To answer this question a qualitative approach and multiple case study methodology was used. A total of nine cases of healthcare firms that had internationalised into or within Southeast Asia were characterised and explored. This research provides an empirical look at the firm level adaptation or standardisation decision. The outcomes of this research are particularly useful for healthcare firms looking to expand into or within Southeast Asia. The findings of this research show that the decision around whether to adapt and the extent of adaptation is not a decision purely made by the firm but is a result of the constraints and differences in the regulatory requirements. The main aspects of service that were found to be adapted to a high extent included the staff, recruitment and local networks. Whereas the quality and accreditation was found to be highly standardised across the foreign healthcare providers to indicate a level of service quality above that of local providers. Adaptation was found to also be driven by the motivation to develop cultural competence to deliver healthcare to one particular ethnic group, often done by firms looking develop their role in intra-regional medical tourism. The extent to adaptation is mostly driven not by the strategic decisions or choices by firms but rather the requirements and restrictions imparted by regulatory and socio-cultural institutions.

Book excerpt: South Asia region’s economies are beginning to recover, though unevenly: economic activity in industry and export sectors have recovered to pre-COVID levels but some labor-intensive services sectors and tourism have not. Inequality has worsened on many dimensions. The process of vaccinating South Asia’s population is underway, with India taking a leading role in production. The socioeconomic benefits of vaccinating most South Asians as soon as possible outweighs the cost by multiple times, and thus justifies having public sector financing. Cracks in the primary health care system became evident since the pandemic began, and the vaccine rollout is likely to have other additional challenges such as delays in production, bottlenecks in supply chain logistics and vaccine hesitancy from some groups (which could delay the process of herd immunity). There are also tradeoffs in the priorities that should be established in deciding who gets the vaccine first.
