Societal and cultural obstacles to treatment are addressed including group stigmatisation, a lack of funding, and our current manual and group-based treatment protocols.

However, some researchers debate whether certain types of high-risk offenders, such as those with elevated levels of psychopathy, are able to form therapeutic alliances since characteristics of those with higher levels of psychopathic traits are not conducive to the formation of a therapeutic relationship. Furthermore, the literature has not addressed the possible impact that Factor 1 and Factor 2 scores within the construct of psychopathy may have on any alliance. The current study is a preliminary examination of the relationships between Factor 1 and Factor 2 scores and the Therapeutic Alliance among a sample of incarcerated sexual offenders. Total psychopathy scores and Factor 1 and Factor 2 scores, as measured by the Psychopathy Checklist-Revised, as well as client and therapist ratings on the Working Alliance Inventory (WAI) were statistically analyzed to explore possible impacts and correlations. Overall we found that there was a significant negative correlation between total psychopathy score and client ratings on the WAI. When this finding was explored further, results suggested that only the Bonds subscale of the WAI demonstrated this relationship. Results from the current study did not demonstrate any significant findings when exploring characteristics associated with Factor 1 and Factor 2 impacting the therapeutic alliance.

For fifty years, however, a small number of English prisons have promoted an alternative method of rehabilitation: the democratic therapeutic community (TC). These prisons offer long-term prisoners convicted of serious offences the opportunity to undertake group psychotherapy within an overtly supportive and esteem-enhancing living environment. Drawing upon original research conducted with ‘residents’ (prisoners) and staff at three TC prisons, Offender Rehabilitation and Therapeutic Communities provides a uniquely evocative and engaging portrayal of the TC regime. Individual chapters focus on residents’ adaptation to ‘the TC way’ of rehabilitation and imprisonment; the development of caring relationships between community members; residents’ contributions towards the safe and efficient running of their community; and the greater assimilation of sexual offenders within TCs for men, made possible in part by a lessening in ‘hypermasculinity’. By analyzing residents’ own accounts of ‘desistance in process’ in the TC, this book argues that TCs help offenders to change by enabling positive developments to their personal identity and self-narratives: to the ways in which they see themselves and their life.
