It bridges innate and adapted immunity, participates not only in host defense but also in many essential physiological processes, old and new diseases and adverse conditions. Indeed, complement became a term that almost defies categorization. What was for a long time a subject for a limited number of specialists has now moved into the mainstream of experimental and clinical immunology. In 1973 I visited the Basel Institute of Immunology and met its director, the eminent scientist and Nobel laureate Nils Jerne. When I entered his office he greeted me with the following words: “Complement, does that really exist?” I was never certain whether he wanted only to tease me or whether he sincerely believed that the complement system was an unimportant biological curiosity, a misstep of evolution. But, of course, missteps do not survive the evolutionary process. Little did I foresee the dramatic developments of recent years when Hans J. Müller-Eberhard and I started to unravel the specifics of the action of the cobra venom factor on the complement system in 1968 and defined a new pathway to its activation. An elucidation of the role of the system in diseases and its control for therapeutic reasons is now getting closer to actual realization in the clinic although many problems, in particular those of highly specificinhibition free of side effects, have still to be resolved.

It is a highly complex system consisting of approximately 50 soluble and cell surface-bound proteins that interact to eliminate danger signals. These signals include factors such as invading microorganisms, necrotic cells, and immune complexes. Furthermore, complement can link innate and adaptive immune responses by regulating T cell and B cell responses. The complement system is tightly regulated to avoid uncontrolled activation. Dysregulation of the complement system has been linked to numerous diseases, both rare and common. Uncontrolled or exaggerated activation can lead to life-threatening conditions such as dysregulation of coagulation, fibrinolysis, systemic inflammation and shock, and possible failure of a variety of organs such as the eyes, kidneys, skin, brain, and vascular system. In recent years, the complement system has gained increased interest, especially as a promising candidate for therapeutic intervention. Eculizumab, the first approved complement inhibitor, is highly effective for treating atypical hemolytic uremic syndrome (aHUS) and paroxysmal nocturnal hemoglobinuria (PNH). Many more drug candidates are currently evaluated in ongoing clinical trials.

The reader will go to the book to discover a specific fact but be persuaded to read more and derive pleasure from the process. The authors' enthusiasm for the subject comes over strongly in the text, and this enthusiasm proves infectious. The risk of infection and inflammation is increased due to treatment-induced neutropenia, making them more dependent on their non-cellular immunity - the complement system, which consists of the classic, alternative and lectin route of complement activation. The dependence on innate host defense mechanisms might be more prominent in children because of the relative immaturity of the immune system. The identification of specific risk factors for infection could improve patient care. The focus of this study was therefore on the functionality of the complement system during oncological treatment in pediatric oncology patients. Using a longitudinal, prospective observation study we were able to show that pediatric oncology patients suffer from transient therapy-induced complement defects in one or more activation routes. Some these observed complement defects were associated to specific therapies or compounds, but the precise mechanisms have yet to be elucidated. We were able to show that Asparaginase is able to directly interact with the functionality of the lectin pathway of complement, independent of protein synthesis. The lectin pathway is one of the three activation pathways of the complement system and reduced activation of this route has been associated with an increased risk of infection in immunocompromised patients. MBL-deficiency is common and reconstitution with plasma-derived MBL has been used to restore the lectin pathway functionality, albeit with limited effect. Analysis of the characteristics of plasma-derived MBL following substitution showed the rapid inactivation of the product by the natural inhibitor, C1-inhibitor. These newly formed inactivated complexes are rapidly cleared from the bloodstream. Substitution using recombinant MBL could provide a more efficacious therapeutic intervention.
