Book excerpt: Over the past two decades Vietnam has made enormous progress towards achieving universal coverage (UC) for its population. Significant challenges remain, however, in terms of improving equity with continuing low rates of enrollment. Ensuring financial protection also remains an elusive goal. The Master Plan for Universal Coverage approved in 2012 by the Prime Minister directly addresses both these deficiencies in coverage. The objective of this report is to assess the implementation of Vietnam SHI and provide options for moving towards UC. This is a joint assessment with development partners, World Health Organization, United Nations Children's Fund (Unicef) and Rockefeller Foundation. Expanding breadth of coverage, particularly for those hard to reach groups such as the near-poor and informal sector would require substantially increasing general revenue subsidies and fully subsidizing the premiums for the near-poor. High enrollment rates would, however, have little impact on financial protection and equity if OOP costs remain high. Achieving UC will require sustained efforts to improve efficiency in the system, and gain better value for money from available budgetary resources; without these efforts, any further progress towards UC would be financially unsustainable. There is considerable scope for improving efficiency in Vietnam. Fragmentation in the pooling of funds gives rise to unnecessary costs. Inefficiencies in resource allocation and purchasing arrangements include: (i) an overly generous benefits package; (ii) provider payment mechanisms and the mix of incentives facing providers which result in an oversupply of services; (iii) high prices, overconsumption and inappropriate use of pharmaceuticals; and (iv) the structure and incentives embedded within the delivery system. The organization, management and governance of SHI are fragmented and often dysfunctional. The present institutional setting for SHI needs to be assessed and changed.

Increasing temperatures, sea-level rises, changes in precipitation patterns and extreme events are expected to increase a range of health risks, from the direct effects of heat-waves, floods and storms, to more suitable conditions for the transmission of important infectious diseases, to impacts on the natural systems and socioeconomic sectors that ultimately underpin human health. Much of the potential health impact of climate change can, however, be avoided through a combination of strengthening key health system functions and improved management of the risks presented by a changing climate. The critical first step in this process is to carry out a vulnerability and adaptation assessment. This allows countries to assess which populations are most vulnerable to different kinds of health effects, to identify weaknesses in the systems that should protect them, and to specify interventions to respond. Assessments can also improve evidence and understanding of the linkages between climate and health within the assessment area, serve as a baseline analysis against which changes in disease risk and protective measures can be monitored, provide the opportunity for building capacity, and strengthen the case for investment in health protection. WHO has responded to this global demand by building on past guidance and technical tools to outline a flexible process for vulnerability and adaptation assessment. In 2009, the WHO Regional Office for the Americas and WHO prepared draft guidance for this process, which was pilot-tested in studies across all WHO Regions. In July 2010, representatives of ministries of health from 15 countries came together in Costa Rica with WHO and subject area experts to share their experiences and provide feedback on how to improve the guidance for the conduct of vulnerability assessments. This document is the result of this process. It is intended not as a final, definitive guide but as an important part of an evolving set of resources that will support effective and evidence-based action to protect health from climate change.

Book excerpt: In the last three decades, many developing and middle-income nations' health care systems have been financed via out-of-pocket payments by individuals. User fees charges, however, may not be the best approach or the most equitable approach to finance and/or reform health services in developing nations. This study investigates the status of Vietnam's current health system as a result of implementing user fees policies. A recent mandate by the government to increase the universal cover to 100% attempts to tackle inadequate insurance cover, one of the four major factors contributing to the high and increasing probability of destitution for Vietnamese patients (the other three being: non-residency, long stay in a hospital, and a high cost of treatment). Empirical results, however, suggest that this may be catastrophic for low-income earners: if insurance cover reimbursement decreases below 50% of actual health expenditures, the probability of Vietnamese falling into destitution will rise further. Our findings provide policy implications and directions to improve Vietnam's health care system, in particular by ensuring the utilization of health services and financial protection for the people.

This book also examines barriers that have hampered the successful design and implementation of health care PPPs in Vietnam. These barriers may be broadly categorized as barriers in the PPP policy and regulatory framework, in the public sector, in the private sector, and in the financial sector. Book excerpt: Over the course of several weeks in June and July of 2014, Rubicon Strategy Group conducted close to forty interviews with country managers of pharmaceutical and device companies, director level administrators in public and private hospitals, successful pharmacy and medical device shop owners, and entrepreneurs at the forefront of the cutting edge in the Vietnam healthcare market. In addition the team went out and conducted a validated survey questionnaire of consumers to judge their preference for particular products, and their preference for healthcare access options in the public and the private sector. The goal of this paper revealed itself in the course of compiling the data: to bring out insights from the front lines of the sales channels and the business models that make up the pharmaceutical and medical device markets in Vietnam. It is the hope of the authors that the information presented in this way can help inform sales strategies and the development of value add services for companies involved in the marketing and/or distribution of drug and medical device products in Vietnam. On the eve of a pair of large negotiations, - a free trade agreement negotiation between the EU and Vietnam, set to be finalized in October 2014, and the negotiation of the Trans Pacific Asean partnership – it was found that much of the conversation with healthcare system company representatives and sales channel participants turned to the issue of the public tender system as well as some of the hardships brought on by Vietnam’s ongoing healthcare market reform. Consequently, one section of this report is geared towards exploring how the policy and regulatory level challenges of the current tender process. However, in constructing this section it became clear that the value of the research conducted during this study is not simply in explaining the tender process as it is supposed to function at the policy level and the attendant issues that stem from that design, but also highlighting how the tender process impacts the decision making of pharmaceutical and medical device executives in-country, in real time, as well as how it impacts the various operators across sales channels. In other words, a core value of the study is necessarily attendant to its exploration of the strategies currently being employed by executives active in Vietnam’s healthcare market. In trying to present a picture of the ground-level impacts of policies and regulatory structures impacting Vietnam’s healthcare space, it is of course important to present a clear outline of the issues that the research revealed. At the same time, it became apparent to fully communicate the ground-level happenings as they related to the tender process, it was also helpful to present a series of case studies that would help add color and nuance to the issues clearly presented.

This world included not only psychiatrists and their patients, but also prosecutors and parents, neighbors and spirit mediums, as well as the police and local press. How each group interacted with the mentally ill, with each other, and sometimes in opposition to each other, helped decide the fate of those both in and outside the colonial asylum.

Book excerpt: When I embarked on this project back on one cold January day of 2015 (this was one of the coldest winters I had ever witnessed with its relentless cold snaps and winter storms that froze my ideas at times), I thought I would only receive a lukewarm response from the Vietnamese-American community because I did not know the great majority of these practitioners-many of them being from the 2.0 generation. They did not know me and I did not know them either. It was basically a "cold sale" process. And I wondered how to approach these physicians, how to entice them to write essays about their training, research, and practice and to open the door to their hearts and souls. Doctors are a strange group of people. They ask a lot of questions from their patients but one never knows what goes inside their minds. I was also leery about the time factor. Since their time is limited and therefore, very valuable, they might not want to spend time writing an essay for a stranger. Then, there is a big difference between writing a scientific paper or a textbook and trying to explain your feelings in an essay. It is then not surprising that a few who had written papers and medical textbooks in English before, categorically refused to write an essay for me. In their case, English is not the main reason. The great majority, especially the first-generation physicians, simply do not feel comfortable enough to write in English, although they have lived in the U.S. for quite a long time. In the end, they wrote more than half of the book you are holding in your fingers. Although the reactions of some physicians are predictable, the response I got from the majority of them remains a mystery to me. Some volunteered on the spot to work with me while others did not even return my email. I am thankful to the essayists who volunteered their time and effort to write the essays and to open their hearts and thoughts to the Vietnamese community in particular and to the wide audience in general. Along the way, I have made new acquaintances and known more people than I had ever expected. I did not know that there are that many successful compatriots working out there in the U.S. as well as in Canada. I am privileged to have known them. In these essays, the contributors have narrated their journeys in the U.S. health care system during the last four decades. As each journey is personal-many have even indicated it in the title of their essays-each one is different from the other. The resulting complex tapestry of journeys provides us with an insight into the ways they have responded to and dealt with their personal, social, and professional problems in a time of upheaval. They have opened their hearts and minds to the public and offered a glimpse of their often tumultuous lives from their war-torn country to the peaceful U.S. While the first-generation physicians tend to detail their harrowing escapes from communist Vietnam, their retraining, and practice in the US, the second-generation physicians attempt to define their identity and their place in the health care system of their adopted country, the U.S. The essays not only represent the "voices" of a new enterprising health care community, but also a historical perspective from an emerging minority community in the U.S.
