Book excerpt: Arthrography of the hip has been practised for over 40 years, but its value as a useful method of investigation has not been generally accepted. Difficulties in technique and interpretation may explain some lack of appreciation. Arthrography of any joint should be practised regularly to facilitate technique and to gain the necessary experience of interpretation. The surgeon or radiologist interested in this technique should have access to a regular flow of patients. The arthrogram can produce valuable information regarding the pathology of the condition which may influence treatment or prove of interest in retrospective study. In congenital dislocation of the hip routine arthrography in the past led to greater understanding of intra-articular obstruction and to the appreciation of some complications of treatment in the early moments of life. Dr Grech has made a stimulating contribution in this monograph which describes technique in the greatest detail. He provides useful guidelines for interpretation and detailed case records which will attract clinical interest. All hip conditions where arthrography might be indicated are described. The author's work is addressed primarily to radiologists or surgeons who might wish to practise arthrography, but there is much in this monograph which will interest those who are already familiar with the technique.

Dr. Thijn has been interested in double contrast studies since he wrote his thesis on the double contrast examina tion of the colon. It would sound facetious to state that after he exhausted this field, he was in need of some other area where the same technique could be used. However, in the same exact and thorough way as in his colon studies, he has examined the knee joint. Considering that the knee is one of the most heavily taxed joints in man, with a multitude of afflictions, many of them closely connected with the age of the individual, radiological investiga tion has shown very few innovations over the decades. The true anteroposterior and lateral projections were ~ and still are ~ the mainstay of the investigation. Projections of the intercondylar fossa, and true patellar projections were used incidentally. Just prior to World War II the advent of arthrography as a double contrast investigation, as promoted by Oberholzer, was a real breakthrough.

Because it is so accessible drastic treatment may be undertaken prematurely and incorrectly. This does not threaten life but may cause permanent morbidity. Dr Stoker has set about examining this joint in depth as a radiologist. As a direct result the surgeon will be helped to make a correct diagnosis and avoid pitfalls, by a diagnostic procedure that is of little inconvenience to a patient. Arthrography is not new but a fresh appraisal is timely. This is not to suggest that there can be any slackening in clinical examination, or that other methods of examination do not have a place. But there are knee joint problems, particularly in teenage girls, in which a clinical diagnosis is very difficult, but must be made exactly. Arthrography must be accepted as a very useful method of examination of the knee joint and an essential one in certain circumstances.
