Book excerpt: Excerpt from The Sympathetic Nervous Sytem in Disease This book is based upon the Croonian Lectures delivered before the Royal College of Physicians of London in 1918. The kindly interest displayed in different quarters in those lectures emboldens me to hope that in this expanded and rearranged form they may prove of service. I have not attempted to give a detailed account of the anatomy of the autonomic or vegetative nervous system. Several excellent accounts are already in existence, but the mass of detail necessarily involved tends to give the general practitioner an exaggerated idea of its complexity. I have aimed rather to emphasize the main plan of the autonomic nervous system. This is simple, and its comprehension aids in the elucidation of many clinical problems. Nor have I attempted to describe in detail the reaction of the autonomic system to disease. Rather, I have chosen to illustrate its relations to a few large groups of disease - such as affections of the endocrine glands, of the digestive and circulatory systems, and glycosuria. The close interaction of the endocrine glands with the sympathetic nervous system on the one hand, and with the reproductive organs on the other, is a fact of paramount importance in the understanding of the emotional response of the sympathetic nervous system, and of the influence of emotion on structure. He has there pointed out the existence of a wide range of functional affections, simulations of the organic diseases of the system, which owed their origin altogether to what he called constitutional or nervous irritation, or what he would perhaps have called, had he written at a later period of his life, irritation of the brain or spinal cord. In fact, the experiments of the French physiologists were no sooner made known to the world, than Mr. Abernethy at once saw the important inferences they were certain to lead to in pathology, as appears from a note to the chapter on nervous and muscular disorders, in the last edition of his works. We will find in this work, which is not deficient in original observations, a concise but complete expose of important researches upon various questions in nervous pathology by L. Tuerck, Benedikt, Meynert, and other authors of the Vienna School. Among works of this character, which are, in my opinion, too little known among us, I shall restrict myself to calling attention to those which discuss the anatomical lesions of the nervous centres in hydrophobia, the lesions of the arteries in cere bral syphilis, and, finally, the observations which have established, according to Dr. Voigt, the relations of cutaneous anaesthesia and hyperaesthesia with the distribution of the nerves to the integument. The study of foreign works upon nervous diseases has led me to pay attention to certain studies in comparative pathology, which may be applied to the investigation of the changes which morbid types of this class experience, according to climate, nationality, race, etc., but without losing their autonomy. As a rule, we are only able to point out slight shades of difference in a study of this character. But the deviation may sometimes be characterized by more or less profound modifications, even in adjacent countries and under very similar parallels of latitude.
