Book The Surgical Treatment of Ulcer of the Stomach Classic Reprint

Download or read book The Surgical Treatment of Ulcer of the Stomach Classic Reprint written by C. Mansell Moullin and published by Forgotten Books. This book was released on 2017-12-19 with total page 62 pages. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The Surgical Treatment of Ulcer of the Stomach The reason of this discrepancy is simply that the condition of the mucous membrane of the stomach during life, especially when it is inflamed, is absolutely different from what it is after death. During life, and especially while digestion is going on, the mucous mem brane is exceedingly well supplied with blood. It is soft, bright red, thick, and full of fluid. After death it is thin, bloodless, flaccid, and in colour a pale slaty gray - it hardly looks like the same structure, the difference is so great; and in cases of acute ulceration, owing to the inflammatory changes in the surrounding tissues, the difference is greater still. Immediately around the ulcer the mucous membrane is swollen, con gested and raised above the level of the rest. Its colour is bright red, though the interior of the stomach may be paler than natural from loss of blood. Here and there I have seen the surface dotted with little blood-stained points from vessels that have given way. The sides of the ulcer are thickened and oedematous, and so soft and swollen that if the ulcerated surface is small, and does not extend deeper than the mucous membrane, they meet together over it and convert it into a narrow fissure. After death, when the blood pressure has fallen, especially in cases in which the vessels have been emptied by previous haemorrhage, all this thickening disappears, and then the ulcer assumes the crater-like form which is usually described; but I have never seen this during life. Book Diseases of the Stomach and Their Surgical Treatment Classic Reprint

Download or read book Diseases of the Stomach and Their Surgical Treatment Classic Reprint written by Arthur William Mayo Robson and published by Forgotten Books. This book was released on 2017-11-10 with total page 324 pages. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Diseases of the Stomach and Their Surgical Treatment The present work took its origin in the Hunterian Lectures delivered by one of us at the Royal College of Surgeons of England in 1900. The attention called to the subject by those lectures resulted in a large increase in our experience of the various gastric conditions amenable to surgical treat ment. To deal fully with subjects lightly touched upon in the lectures, to expand, in fact, those lectures into a volume, was a work which the lecturer unaided would have been hard pressed to accomplish in the space of time within which it was felt desirable to publish the volume. A collaboration see'med therefore desirable, and as we had been associated in hospital work more or less closely since 1887, when our relative positions were those of Honorary Officer and House Surgeon, the present collaboration seemed both natural and appropriate. For every statement made in the book we jointly hold ourselves responsible. The whole work has undergone a careful revision by both Of us, and much of it has been frequently discussed while in preparation and after comple tion. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The Surgery of the Stomach: A Handbook of Diagnosis and Treatment IN the following pages I have attempted to give a practical account of The Diagnosis and Treatment of those Affections of the Stomach which are amenable to Direct Surgical Inter ference. 1 The significance of the information to be gained from test-meals is considered in some detail, because I believe that such investigations are of great value in the diagnosis of gastric disease. It appears to me strange, that whereas at the present time, bacteriological and pathological examinations are used so extensively in the diagnosis of disease, chemical methods are so rarely made use of in practice, and dealt with so briefly in text-books. The stomach-tube should be part of the armamentarium of every practitioner. It is not essential that he should perform all the quantitative analyses to which I refer, but I venture to think that he would be repaid amply, if he carried out the ordinary qualitative tests, and the estimation of the total acidity of the gastric contents. The practitioner, be he consultant or otherwise, who, without the aid which such investigations afford, continues to treat a patient suffer ing from persistent indigestion, or gastric pain, is acting unfairly to his patient and unjustly to himself. Book excerpt: Excerpt from Section on Surgery: General and Abdominal Whether the case be a resection for cancer or a gastro-enterostomy for ulcer, the reserves of the starved patient must be built up by the introduction of water, sodium bicarbonate and glucose and especially by an adequate amount of sleep and rest; and acid formation must be diminished by the elimination, as far as possible, of worry, fear, anxiety, exertion, loss of Sleep, trauma and anesthesia. In the case of ulcer, the fate of the ulcer-bearing area is the chief consideration, for the evidence on many Sides, especially that presented by the Mayo Clinic, shows the tendency to cancer growth in ulcers of the stomach, though happily not in duodenal ulcers. Our lamented President, Dr. Rodman, made a logical departure in the complete excision of the ulcer-bearing area, but even this is not enough. At this point we must join hands with our medical confreres and for a period of many months - Six or more - the patient Should be kept under strict dietetic care, such, for example, as the Sippy regimen. If the patient is dis missed from the surgeon's care to the same habits of life, and the same dietetic carelessness that existed when the ulcer originated, and if in certain cases mouth infections and teeth defects are uncorrected, then there is bound to result a certain percentage of failures. It is just as illogical to exercise no care over a postoperative ulcer patient as it would be to dismiss a postoperative exophthalmic goiter patient without therapeutic instructions and prolonged oversight. Relevant to the geriatric age group, the volume covers epidemiology, physiology of aging, gastrointestinal physiology, pharmacology, radiology, pathology, motility disorders, luminal disorders, hepato-biliary disease, systemic manifestations, neoplastic disorders, gastrointestinal bleeding, cancer and medication related interactions and adverse events, all extremely common in older adults; these are often hard to evaluate and judge, especially considering the complex aging physiology. All have become important components of modern medicine. Special emphasis is be given to nutrition and related disorders. Capsule endoscopy and its utility in the geriatric population is also covered. Presented in simple, easy to read style, the volume includes numerous tables, figures and key points enabling ease of understanding. Chapters on imaging and pathology are profusely illustrated. Book The Great Ulcer War

Download or read book The Great Ulcer War written by William Stevenson Hughes and published by . This book was released on 2014-04-30 with total page 293 pages. Available in PDF, EPUB and Kindle. Book excerpt: In 1983, in Australia, a medical resident, Dr. Barry Marshall, and a hospital pathologist, Dr. Robin Warren, reported in two letters to The Lancet finding a bacterium associated with gastritis or inflammation of the stomach. The publication stimulated little reaction. However, a year later when they reported that the bacterium was also associated with ulcer disease and declared that bacteria caused ulcer disease, it had the effect of an assassination of an archduke. Most prominent clinical investigators in the United States and England argued that hyper secretion of acid was the cause of ulcer disease, and they collaborated with the pharmaceutical companies that made the new drugs that blocked acid secretion to attack the new bacterial theory. The Great Ulcer War tells how the war was fought, the weapons used, and the alliances made, and why the war in spite of overwhelming evidence in favor of the bacterial theory, lasted for ten years. The Great Ulcer War introduces a novel theory, the Pandora Hypothesis, to explain the length of the war. It proposes that the general medical establishment especially in the United States simply did not like the bacterial theories of major chronic diseases. These thought leaders-"the big guys"-facilitated and prolonged the opposition to the bacterial theory of ulcers largely by doing nothing to support the theory until the very end of the war. They were afraid that if a germ theory was accepted for ulcers, a Pandora's Box of germ theories developed within university departments of microbiology for other chronic diseases would be opened and released into the medical world. This revelation would diminish the reputation and profit of the medical establishment and the pharmaceutical industry by threatening their favored explanations of the causes of these diseases: genomic errors and dysfunctional biochemistry and physiology.
