Available in PDF, EPUB and Kindle. Book excerpt: A dental prophylaxis paste containing 2.0% stannous fluoride in water-free solution was tested for its ability to reduce enamel solubility. The paste was applied with a rubber cup and also by a 2-minute topical application of a slurry made of equal proportions of the paste and water. Evaluation continued over an aging period of 16 weeks. The prophylaxis paste, applied full strength in a rubber cup, reduced enamel solubility in the range of 55% to 63%. Age did not significantly affect the protective performance of the paste. When the paste was applied as a topical application, its protection ranged from 58% to 72%. A significant increase in effectiveness was associated with aging. It is suggested that chemical stability, a factor related to the preservation of the stannous ion, is induced in this paste by placing the stannous fluoride in solution in glycerin and scrupulously avoiding the incorporation of water at any stage of compounding. Water is added to the pellet to form a paste immediately prior to use. The advantages of the pellet are: (1) indefinite stability of the therapeutic agent; (2) assurance of effective yet nontoxic amount of fluoride administered to the patient; (3) increased ease of storage, dispensation, and mixing of paste; and (4) reduction of waste. With the use of the oral prophylaxis pellet, patients can be assured of receiving an effective oral prophylaxis with a minimum of adverse reactions. (Author).

Book excerpt: The in vitro effectiveness of an aqueous stannous fluoride (approximately 20 percent) prophylaxis paste was determined by measuring the amount of phosphorous removed from enamel surfaces both before and after treatment with the paste. Treatment reduced enamel solubility by over 90 percent in all experimental groups, and there was no loss of effectiveness when the paste was aged over an 8-week period. The substitution of liquid petrolatum, or silicone and petrolatum in combination, for water, did not increase effectiveness.

The degree of hydration of the treatment was shown to affect the ability of the paste to impart a reduction of solubility to enamel. Application of the past without the addition of water was ineffective in reducing enamel solubility. Use of the paste as a 50% slurry with mixed saliva resulted in a reduction in enamel solubility that approached that achieved by the use of a freshly prepared stannous fluoride prophylactic paste. Book excerpt: A stock solution of stannous fluoride was aged over a period of 8 weeks and tested at intervals as a base solution for the preparation of 10% stannous fluoride topical solution and, when added to silex, as a component of a prophylaxis paste. The compatibility of this solution was tested with four pumice and with silex in fully prepared prophylaxis pastes. The pumice contained approximately 7.0% stannous fluoride; the silex paste, about 3.7%. Performance of the test preparations was based upon the amounts of phosphorous withdrawn from enamel surfaces by 0.025 M lactic acid both before and after the test treatment. Protection afforded by the 10% stannous fluoride topical application ranged from 68.3 to 78.8%. Silex prophylaxis paste prepared from aging stock solution provided protection of from 68.2 to 79.1%. The flour pumice paste, aged as mixed, was 71.6 to 82.8% effective, while a comparable silex paste reduced enamel solubility by 67.9 to 76.9%. There was no apparent deleterious effect of age on any of the preparations tested. (Author).

Performance was measured by the amount of phosphorus removed from enamel before and after fluoride treatment. Phosphorus was determined automatically on the AutoAnalyzer. SnF2 solutions of 5.0, 10.0, and 20.0 percent concentration were significantly more effective than 0.4, 0.7, and 1.0 percent solutions. Aqueous preparations of stannous fluoride maintained their protective potency throughout the 12-week experimental period.
