Book excerpt: Aim: The purpose of this study was to compare treatment outcomes and stability of growing and non-growing patients treated with either functional appliances or surgical maxillary impaction with mandibular advancement for the correction of their Class II skeletal malocclusion characterized by mandibular retrusion and increased vertical dimension. Materials/Methods: The early-treatment or functional group (FG) consisted of 17 patients (10 females, 7 males), with a mean age of 9 years 5 months who received Bionator with posterior bite block and high pull headgear. The surgical group consisted of 15 patients (9 female, 6 male), mean age 23 years 6 months. These patients were treated with Lefort I osteotomy for maxillary impaction combined with bilateral sagittal split ramus osteotomies with rigid fixation for mandibular advancement. The experimental groups were compared to a control group consisting of 17 non treated patients from the Bolton and Michigan growth studies. All 17 patients (CGI) were matched with the functional group, while 15 of the original 17 (CG2) were matched, when they achieved full skeletal maturation, to the surgical group. Lateral cephalograms taken for the early-treatment group at T1 (initial records), T2 (completion of functional appliance treatment), and T3 (completion of comprehensive treatment) were utilized and compared with radiographs taken at T1 (initial records), T2 (immediate post-surgery and T3 (long term post-surgery) for the surgical patients. A null hypothesis of no difference in treatment outcomes between the functional and surgical groups was proposed. Book excerpt: Cephalometric head films from a sample of 35 patients who had undergone a LeFort I maxillary osteotomy combined with a simultaneous mandibular advancement were evaluated to determine the postsurgical stability of the skeletal segments. Twenty-six of the patients had rigid fixation to stabilize the segments and nine patients had skeletal wire fixation. Twenty-seven cephalometric parameters were examined before surgery, immediately after surgery, and after a follow-up period averaging fifteen months. The results showed the maxilla to be stable for both types of fixation but the mandible was significantly more stable in the rigid fixation sample, especially in terms of the ability of rigid fixation to maintain rotational control between the proximal and distal segments. Measurements of cephalometric points representing resting muscle lengths indicated a propensity for the stretched muscles to relapse back to within 5 percent of their original lengths.
