Clinicians have to consider the best option about theeffectiveness of each procedure when choosing among them.Aim/HypothesisThis study aims to examine the outcome by analyzing examples of maxillary sinus lifting, bone graft, and simultaneous vertical ridgeaugmentation, through retrogressive studies.Materials and MethodsFrom 2005 to 2010, the patients who have severe alveolar bone loss due to long-term no treatment or progress periodontitis andreceived sinus lifting, bone graft, and simultaneous vertical ridge augmentations were selected. 15 patients who visited in SeoualNational Universitiy Bundang Hospital were analyzed according to clinical records and radiography (periapical view, panorama) andevaluated postoperative complications, success and survival rate of implants, complications of prosthesis, Implant stability quotient(ISQ), Vertical resorption of grafted bone in postoperative 1 year, 2 year, 3 year and final observation and marginal bone loss.ResultsThe average age of patients in total was 54.2 years. Among the 33 implants, 6 failed to survive, resulting in 81.8 % survival rate, and6 failed to success, resulting in 81.8% success rate. The ISQ of 1st surgery was 61.3 and 73.5 of the 2nd surgery or impression.Postoperative complications were characterized by 4 ecchymosis, 3 peri-implantitis, 2 sinusitis, 2 fracture of fixture, and 1 hematoma,1 bleeding, 1 exposure of titanium mesh, 1 numbness, 1 trismus, 1 loss of fixure. Prosthetic complications involved 2 screw loosening,1 fracture of abutment, 1 food packing. Complications were double counted on 1 implant. The resorption of grafted bone material was0.23mm after 1 year, 0.47mm after 2 years, 0.41mm after 3 years, and 0.37mm at final observation. The loss of marginal bone was0.11mm after 1 year and the loss of marginal bone at final observation was 0.16mm.Conclusions and Clinical ImplicationsWhen sinus lifting, bone graft, and vertical ridge augmentation was practiced simultaneously, postoperative complications areincreased and survival rates are low. For the long-term good prognosis, implant surgery was recommended that sufficient recoveryperiod to ensure good bone formation and the delayed implantation after bone graft are needed.

Book excerpt: Purpose : The Purpose of this study is to show the total survival rate of implants with maxillary sinus grafting and the effects that reach the survival rate by classifying types of graft materials, implant type, operation method, residual bone height and evaluate graft material resorption rate after sinus graftingMaterials and methods : 61 dental implants placed with sinus bone grafting in 24 patients at Wonkwang University Sanbon Dental Hospital were installed simultaneously or after regular healing.Various bone grafts (autograft, xenograft, allograft, alloplast) and fourth implant type(GSII, Xive, Implantium, Novel biocare) were used. All implants were investigated clinically and radiographically, being with average 20 months follow-up period after installation.Results : 3 fixtures were lost, resulting in 95.1% cumulative survival rate of 61 osseointegrated dental implant. Survival rate according to bone material type, Implant type, operation method, residual bone height, have no statistically significant differencies.The mean preoperative residual alveolar bone height was 4.75mm, average postoperative height of graft materials 10.8mm, vertical bone resorption rate was 10% after 2 years. Resorption rate according to operation method was 7%(simultaneous) and 5%(delayed) after 1 year.It can be suggested that maxillary sinus grafting may have predictable result with various bone graft materials and implant type, residual bone height, operation method.

Lateral window approach for sinus augmentation technique is a well-documented. Various biomaterials grafted sinus have been used such as xenografts, allografts, or composited graft. These biomaterial has demonstrated good outcomes, but most the grafts only with osteoconductive properties and had prolonged graft-healing time, therefore, the amount of new mineralized bone could be compromised. The recently reported that the sinus augmentation without bone graft might be considered effective impredictable bone formation. The advantages of one stage implant placement without bone grafts in sinus lift procedure are easily to identify the new bone formation around the implant, avoid bone grafts related infection complication, and reduce costs effect. The used L-PRF in sinus lift procedure may enhancing osteoprogenitor cells and modulate hard tissue transformation. The aim of this study was a case series of sinus augmented with xenograft, graftless, CGF + xenograft, or CGF alone ,to compare the sinus membrane elevation around implant volume stability and implant survival rate over 5-year follow up outcomes. Totally 10 patients with mean age 55.4. Fifteen sinuses lift with simultaneously 40 dental implants placed in augmented sinus, 20 implants placed in bone augmented sinus, other 20 implants in graftless sinus. Among the augmented sinuses 7 grafted and 8 without grafted bone materials, 3 sinuses used L-PRF without bone grafted in thickness Schneiderian membrane which potential for sinusitis complication. The lateral window shift to inner site as tenting to avoid membrane drop down. For grafted site, filled with Bio-Oss to supported membrane, the non-grafted site used collagen membrane and implant supported the membrane. The dental implants length insertion from 10 mm to 13 mm in mean residual bone height 4.3 mm. The lateral window cover with resorbable membrane with fixation pin and primary closure the wound. Submerged healed 6 months, then uncover the implants. All implant restorations via provisional restoration observation occlusion comfortable and stable then definitive prostheses delivery. The observation evaluations through CBCT measurement. All of implants firm primary stability were achieved, giving implants healed above 6 months. In grafting group the primitive bone significantly greater than graftless group (3.57 mm vs. 5.03 mm), and significantly bone gain when initial implant placement (11.28 mm vs.7.78 mm). But after 5 year bone remodeling, the augmented bone height no significantly difference. It indicate that grafting group and graftless group bone height gain the same, except the apex area, the grafting more larger than graftless group. Marked bone formation around implants observation with CBCT. One implant failed during provisional prosthesis stage, giving the survival rate of 97.5%. It is suggested that used non-grafted procedure in sinus augmentation may be an alternative treatment options.
