At the request of the Social Security Administration, the National Academies of Sciences, Engineering, and Medicine's Board on Health Care Services organized a virtual workshop on July 21, 2020, titled The State of the Science of the Use of Biomarkers to Establish the Presence and Severity of Impairments. Workshop discussions focused on the current and potential uses for biomarkers; explored legal and ethical implications associated with biomarker use in clinical decision making; and considered the possible uses of nongenetic biomarkers as tools for the diagnosis or prognosis of the severity of specific physical and mental impairments. This publication summarizes the presentations and discussion of the workshop.

Interestingly, cognitive characteristics are different among depressed suicidal and depressed nonsuicidal subjects, and could be involved in the development of suicidal behavior. The characterization of the neurobiological basis of suicide is in delineating the risk factors associated with suicide. The Neurobiological Basis of Suicide focuses on how and why these neurobiological factors are crucial in the pathogenic mechanisms of suicidal behavior and how these findings can be transformed into potential therapeutic applications.

A host of ‘omics’ disciplines in addition to genomics, marked by multidimensional data and complex analyses, and enabled by bioinformatics, have pushed the trajectory of biomarker development even further. They have also made more tractable the complex mappings of genotypes to phenotypes – genome-to-phenome mapping – to which the concept of a biomarker is central. Genomic investigations of the brain are beginning to reveal spectacular associations between genes and neural systems. Neural and cognitive phenomics are considered a necessary complement to genomics of the brain. Other major omics developments such as connectomics, the comprehensive mapping of neurons and neural networks, are heralding brain maps of unprecedented detail. Such developments are defining a new era of brain science. And in this new research environment, neural systems and cognitive operations are pressed for new kinds of definitions – that facilitate brain-behavioral alignment in an omics operating environment. This volume explores the topic of markers framed around the constructs of cognitive and neural systems. ‘Neurophenotype’ is a term adopted to describe a neural or cognitive marker that can be scientifically described within an associative framework – and while the genome-to-phenome framework is the most recognized of these, epigenetics and non-gene-regulated neural dynamics also suggest other frameworks. In either case, the term neurophenotype defines operational constructs of brain-behavioral domains that serve the integration of these domains with neuroscientific and omics models of the brain. Book excerpt: "Diagnosis in psychiatry relies on the subjective assessment of symptoms, both reported by patients and observed by clinicians. This purely symptom-based classification has its limitations. Patients with the same diagnosis express different symptoms, and there is considerable overlap in symptoms across different mental disorders, leading to the possibility of over- or misdiagnosis. Moreover, these diagnoses tell us little about the individual patient, as treatment guidelines tend to follow a one-size-fits-all approach that may not be effective for every patient, resulting in delayed or ineffective treatment for some. To address these issues, there has been a shift towards more objective approaches to diagnosis and personalized treatment in psychiatry . An important means to this end is the search for objective biomarkers , which are measurable characteristics that indicate normal (neuro)biological processes or responses to therapeutic intervention. These biomarkers could help clinicians make more evidence-based decisions that are tailored to individual patients, potentially leading to more efficient and effective psychiatric care. The aim of this thesis was to investigate whether machine learning techniques could be used to find generalizable biomarkers for various psychiatric disorders. We used neuroimaging data to develop diagnostic biomarkers for OCD, (trans)diagnostic biomarkers for anxiety disorders in juveniles, and biomarkers predictive for ECT treatment outcome in patients with depression. This thesis showed that machine learning techniques can be applied to predict diagnosis and treatment outcome with variable success. Future studies should carefully consider sources of heterogeneity to facilitate the development of more accurate and clinically relevant biomarkers, ultimately improving psychiatric care."--
