Smadja's concept, a true clinical and conceptual invention in the field of psychopathology, has evolved from work originally carried out by Pierre Marty, Michel de M'Uzan, Christian David and Michel Fain, pioneers of the psychoanalytic approach to subjects with somatic illnesses. These authors, internationally known as the "psychosomaticists of the Ecole de Paris", have contributed extensively to our understanding of the relations between the life of the psyche and the working of somatic disease. Using his own clinical experiences as a psychoanalyst and psychosomaticist, the author significantly extends the study of "operational states" and their metapsychological implications.

The author surveys the approaches to the subject from the observational to the experimental level. He presents the findings of a study carried out, at a community health screening survey, to test the association between physical and psychiatric disorder. The methodological difficulties of this kind of study, such as sampling the general population and reliable measurement of clinical states, are described. The results of the study, which confirm the two types of morbidity have a positive and significant association, are discussed in terms of the ecology of disease and psychosomatic medicine. It is suggested that the findings have implications for the delivery of health care and preventive medicine.

The outline of the present volume was conceived nearly three years ago. In 1987, a conference on this theme was held in Maastricht, the Netherlands, on the occasion of the founding of the European Society for Philosophy of Medicine and Health Care (ESPMH). Most of the chapters of this book are derived from papers presented at that meeting, and for the purpose of editing the book Stuart Spicker, Ph. D. , joined two founding members of ESPMH, Henk ten Have and Gerrit Kimsma. The three of them successfully brought together a number of interesting contribu tions to the theme, and ESPMH is grateful and proud to have initiated the production of this volume. The Society intends that annual meetings be held in different European countries on a rotating basis and to publish volumes related to these meetings whenever feasible. In 1988, the second conference was held in Aarhus, Denmark on "Values in Medical Decision Making and Resource Allocation in Health Care". In 1989, a meeting was held in Czestochowa, Poland, on "European Traditions in Philosophy of Medicine. From Brentano to Bieganski". Inspired by the work of the French phenomenologist Maurice Merleau-Ponty, a phenomenological theory of psychosomatics is worked out as an alternative to traditional, biomedical thinking. The patient who presents somatic symptoms with no clearly discernible lesion or dysfunction presents a problem to the traditional health care system. These symptoms are medically unexplainable, constituting an anomaly for the materialistic understanding of ill health that underlies the practice of modern medicine. The traditional biomedical model is not appropriate for understanding a number of health issues that we call “psychosomatic” and for this reason, biomedical theory and practice must be complemented by another theoretical understanding in order to adequately grasp the psychosomatic problematic. This book establishes a complementary understanding of psychosomatic ill health in terms of a non-reductionistic model allowing for the (psychosomatic) expression of the lived body. A thorough presentation of the work Merleau-Ponty is followed by the author’s application of his thinking to the phenomenon of psychosomatic pathology.
