When, some years ago, I determined to make a special study of Childrens' Garments, I found, on entering this branch of the business, an utter absence of all means for the production of children's garments by system and proportions of age and size. Beyond a few good patterns held by a limited number and guarded with extreme jealousy, no definite knowledge of the Juvenile Clothing business seemed to exist. Whatever good patterns there were in the country were originated in the most of cases experimentally; and by a constant system of revision, based on a close observation of the requirements of living subjects, were toned down to correct proportions and fit. The amount of trial involved in the production of good patterns on such a basis, made them of great value, and almost impossible to obtain. As to any system by which to reproduce them and the proportions upon which they were based, it was unknown, or if known, was a Dead Secret. I determined to attain a thorough and exhaustive knowledge of the law of growth and the proportions of the human body during all the years of juvenile growth, and of the harmony of the growing form of any size. I turned to nature, and dealt only with the living subject. After the labor of years in measuring children of all ages and sizes, and compiling the averages into a system of common sense tables, I have the pleasure to know that my work is completed, and that it is my high privilege to be able to benefit others, be it ever so little, by supplying the lights that have guided me to success. Those of poverty, bad housing, bad feeding, and neglect, may be severally recognized, but the extent of their interrelations with actual respiratory and alimentary disease - those to which the greater part of the mortality is referred - can be traced only with difficulty. It is the object Of this work to aid ln the labour of cutting away the matrix and entangling fibres and to lay bare the hidden ramifications of at least one important causative agency - epidemic diarrhoea. This affection - if we accept provisionally the more novel and generally favoured conception. As to its nature - is revealed as something very like an ordinary infectious disease, and one which permeates all classes, while the excessive mortality it gathers round itself in urban centres must be regarded as something superadded, owing to the vicious circle it forms with those baneful conditions of slum life mentioned above. On the other hand, its peculiarly intimate association with the circum stances of domestic life, from the continual faecal pollution of the interior of the household by infants and others, tends to make it more so than other affections of the kind peculiarly a class disease, and an especial scourge of dirty neighbourhoods. Dirty towns may however be saved from excessive mortality by a high percentage of breast feeding. A notable point in the interesting comparison that can be drawn between diarrhoea and typhoid fever is that, in accordance with their peculiarly opposite age-incidence curves, the marked and habitual depositing of infectious excreta within the household in the former disease may make the question of water-closet versus conservancy pan a matter of far less importance than in the latter. Then came the discovery that lower altitudes would do if certain conditions were obtained; and so arose the great sanatorium movement. But slowly we found that patients could not spend their lives in sanatoriums; and one day on making my way up to one of them in England, I met on the way patient after patient, slouching along, bored to death with themselves and with each other; and even worse in morale than in body. Better discipline and better notions of thera peutics mended some of that; still I could not forget those listless saunterers, and it became evident to some of us, however unwillingly, that Hope was drooping again. The sanatorium was doing a great educative work no doubt; but at the end of its four or six months - what then? To send the patient away with recommendations about light jobs, and a regime, was almost a mockery or quite. What about the wage, and the family to be supported? The next lesson was brought home to me by a visit with other commissioners to certain cities, concerning some such problems. Before me now I see a gaunt hollow-eyed man, coughing, and leaning against the wall as he tried to talk to us, saying that his mates when he came out of the sanatorium - good fellows as they were - had bought him a milk that he might creep round, and earn a bit. The brave wife, shawl on head and mill apron on, had just come from the factory, and apologised for the dirty house - as well she might. The poor thing was working all day at the factory to keep the wolf from the door. All being dragged down together into the pit! What is the value of a good house, or a clean house, if no wages! What is there for the children? And what is to stop the infection! Who then would have the imagination, the initiative, the business capacity, to lift this burden, like lifting a world? Consequently in this text, details of embryology, histology, and physiology have been largely left to the textbooks dealing with those subjects, in order that gross human anatomy might be the more thoroughly explained. While all of "the recognized authorities" and the publications of the various anatomical associations have been freely consulted, these descriptions have been based entirely on the actual facts to be found in the human body and are not dominated by any one single authority. The many new discoveries and the changes in teaching methods that have been made in recent years, have made it necessary to make so many additions and changes in the text that the entire work had to be rewritten. Parts of the nomenclature of the German Anatomical Society are now being used so universally, that it became necessary to include it, in its entirety, in the Latin form. However, since comparative anatomy has been almost entirely ignored in the B. N. A., and as so many of its terms are not only inconsistent but also evidence personality rather than broad anatomical principles, it seems certain that it still requires much revision if it is to endure; therefore while the B. N. A. terms are given in the Latin form, only such of them as seem entitled to survival are given in the English form in preference to the older regular terms. It is confidently hoped that the student will find these descriptions not only accurate and up-to-date, but intelligible as well.
