Book excerpt: Excerpt from The Ophthalmic Record, Vol. 8: October, 1899 I have the pleasure of bringing before the society this year an inter esting case of cyst of the vitreous. It is, as I take it, a simple cyst as we understand that term in surgery. A cyst may be defined as a cavity bounded by a distinct envelope composed of fibrous tissue lined with endothelium and called a cyst wall; or it may be covered by epithelium and certain secreting structures: a cyst may result from the increase of the secretion of an already formed space or cavity by extravasation into it, or the cavity may be a new formation. The patient, L B age 17, is the son of a practicing physician of New York City, and he came to me for' advice January 4, 1897, saying that his sight in the right eye had been defective for about a year; there was no pain or discomfort of any kind, but he thought he could not see as well as he had previously; there had been no change for the past year, and it had become no worse than when he first discovered that it was affected. The evening of the same dav he noticed that he could not see with his left eye, and that for several succeeding days the sight was growing worse. I want to say that I make exceptions occasionally - in cases of feeble old people, when the patient is very nervous, afraid or unruly. And I want to say further, that I do not treat these people slightly. Fash ions, or what one Oculist does, may not be the best for another; and no one ought to venture, except those with experience, and then always remember so precious a thing as vision is at stake. Excuse me if I seem presumptuous when I say, in substantia tion of my preferred operation and treatment, that I am not a beginner, having operated upon eight cataracts in one day, and forty-seven in one month. The half-metre test types are very useful at the distance of a half metre or less, but they are of very little use at one metre, and quite worthless at two, four or six metres. In skiascopy there are movements of light and shadow in the pupil that can be seen at one-half metre, which are not to be seen at one metre or any greater distance and in many eyes it is these movements, and these alone, that truly indicate the refraction of the eye.
