Book excerpt: Excerpt from The New York Medical Journal, 1877, Vol. 26 Concerning the causation of the diathesis, and the develop ment of the cachexia, a discussion at this juncture is unneces sary, as my analysis will bear directly on these two points. Dr. David Prince, of Illinois, has recently contributed some Considerations in Relation to Diseases of the Joints, and he speaks of a condition which may be inherited and perma nent, acquired and permanent, or acquired and temporary. Now, to that condition, as embodied in the definitions I have already given, I shall for convenience' sake apply the name strumous, and by the terms of my theme it will be seen that I recognize some such factor in the production of a large proportion at least of diseases of the joints. The two great theories to-day are traumatism and non traumatism, and each has warm advocates. Some contend that every case of joint-disease is local, and is produced directly by a fall or other injury; while others contend that falls have very little to do in the causation of such - that the disease is constitutional, and is purely strumous. Some even go so far as to affirm that joint-diseases are always tubercular. Now, it seems to me that there is an immense amount of logomachy crowded into the literature on this subject. To say that a certain disease is traumatic, means to one mind that a fall or injury was the only cause, and to another mind it means that other and more important conditions were present, while the fall served simply to develop the disease in a system al ready predisposed. It cannot be employed in operations on the trunk, or on the face and neck, where surgeons would gladly, if possible, avail themselves of its advantages. These advantages, as claimed by Esmarch, are as follow First, the prevention of a great loss of blood during the larger Operations. It is assumed that this economy of the vital fluid will sometimes be the direct means of saving life that it will render more certain and speedy the healing process, and di minish the frequency of septicaemia and pyaemia. The other principal 'advantage of the method is the facility with which it enables the surgeon to accomplish deep and difficult dis sections, the tissues not being obscured by blood. It is said also to permit the easy extraction of foreign bodies, and the thorough examination of diseased bones and joints, with the view of deciding upon the extent of Operation necessary in any given case. I trust he will be saved from stone, although he did incur the danger of retention as I had foretold; for he can empty his bladder thoroughly now, and also wash it out with warm water at will, if necessary. But it is very clear to me, after observing his case through all these years, that if the patient had gone to work when he first applied to me for relief, and learned within the year to help himself with the catheter, he would have saved himself both time and torment, and that at this moment his bladder and kidneys would be in better con dition to serve him for the next ten years. At six months. After eight months the number suddenly dropped because most infants at this age had already received solids when brought to the physician and only those cases are here recorded in which the infants were ordered to receive solids as a therapeutic procedure for the first time when they came under my care. It must be borne in mind that none of these infants, except those suffering from severe diarrhea, received these prepared solids exclusively, i. E., milk in some form, as noted in the table, either human or properly adapted cow's milk or both, was simultaneously fed. It is demonstrable, however, that the addition of these substances to the diet in early infancy is not only harmless but decidedly advantageous and that less dependence need be placed upon milk as an exclusive article of sustenance during this period of existence. He learns, as a matter of course, the distinguishing features of each bone, its form, pro cesses, and relations to neighboring parts.
