Book excerpt: Forty years after Kramer's (1974) seminal work, Reality Shock, new graduate nurses continue to have difficulty transitioning to the registered nurse (RN) role. The purpose of this phenomenological study explores the lived experience of new graduate nurses who completed a Veterans Affairs externship program in their senior year of nursing coursework and its perceived impact on their successful transition into the practice role 3-24 months following graduation. Benner's (1984) Novice-to-Expert Model, Karmer's (1974) Reality Shock Theory, and Selder's (1989) Life Transition Theory support a framework for this study. Twelve telephone interviews were conducted using van Manen's (1990) method for researching the lived experience. One overarching theme "feeling confident" and three main categories, "transitioning to the RN role," "making decisions," and "interacting with professionals," were identified from the data. Externship programs assist the student to comfortably and smoothly transition as a new graduate nurse by offering additional clinical and practice experiences. Given the complexity of today's health care environment, there is a growing need to better prepare the graduate nurse for their transition into nursing practice. Externship programs can serve this purpose.

Book excerpt: The transition from student nurse to professional nurse is often difficult for the new graduate especially if transitioning into critical or progressive care areas of practice. It is imperative for new graduates to receive a clinical orientation that meets their educational and transitional needs. The orientation process should provide them with a strong foundation in critical or progressive care practices. Educators are challenged to provide educational opportunities while accommodating diverse learning needs and continuing to maintain competency that ensures the highest quality of patient care and improved patient care outcomes. New graduate nurses hired into the critical or progressive care areas were asked to participate in a study evaluating the effect of blended orientation approaches on their level of self-confidence and satisfaction with current orientation methods. Their level of self-confidence and satisfaction were evaluated and measured using a pretest and posttest questionnaire. New graduates entering critical or progressive care areas of practice were evaluated following nursing school (prior to hospital orientation) and following a two-month hospital orientation model. The results did not reveal any significant difference between pre and post orientation. The review of literature suggests that the use of a more blended learning approach to teaching and learning can be beneficial. Blended learning can include online learning, case studies, human simulation, clinical time with a preceptor, service-based classroom lectures, quizzes, games, videos, mock events, review of pertinent evidence-based articles, and demonstration with return demonstration (p. 10), and is a growing area in education requiring more evaluation and research.

Book excerpt: The Future of Nursing explores how nurses' roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America's increasingly complex health system. At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: Based on research studies and documented professional experiences there appears to be an issue with high new graduate nurse (NGRN) turnover rates within the nursing profession. This poses a problem within the profession as health care organizations prepare for the anticipated nursing shortage projected by the Bureau of Labor Statistics. High turnover rates have been linked to poor transition into practice. Inadequate transition can cause nursing errors that jeopardize the quality and safety of patient care in addition to causing health care organizations to lose money. This indicates the need for effective transition into practice programs as NGRNs join the workforce. One solution is to implement nurse externships. Externships have been common practice for many professions as a successful way to prepare students for the roles and responsibilities required by a given profession. Implementing externships within the nursing profession would be an effective means in preparing nursing students for the profession and ultimately lower NGRN turnover rates (Redding and Flatley, 2003). Currently, it is common practice for health care organizations to adopt new graduate programs (NGP) to help NGRNs transition into practice. While these programs have been successful in lowering NGRN turnover rates to some degree, they are costly to organizations and the issue still remains, justifying the need to explore other options. The author of this paper explores the concept of nurse externships as a proposal to produce better prepared NGRNs and reduce NGRN turnover rates; identifying an appropriate plan for implementation, evaluation, and dissemination to ensure timely and appropriate integration into practice while utilizing current research to support key claims.

Book excerpt: " Despite the evidence supporting the benefits of Nurse Residency Programs, anecdotal evidence suggested that the curriculum does not provide a focus on the lack of confidence new graduate nurses have transitioning into practice, nor does it provide resources to enhance confidence development. This evidence-based change project included new graduate nurses who were enrolled in a transition to practice, Nurse Residency Program (NRP) cohort. The participants engaged in a quality improvement project that provided confidence building exercises and resources to increase inter-professional confidence as an addendum to their current NRP curriculum. A confidence building curriculum addendum project was implemented during months five and six of their twelve month program. The data to measure the outcomes of this evidence-based change project was collected using the Casey-Fink Graduate Nurse Experience Survey as a pretest/posttest. New graduate nurse residency participants (N=10) showed a limited overall improvement in their confidence/comfort level post intervention. One area of noted increase was having less difficulty organizing patient care needs (mean increase 24.14%). The survey results suggest numerous possibilities for improvement of new graduate nurses transitioning into practice-such as confidence building initiatives, to include seasoned nursing staff education on the importance of their role in the onboarding of new graduate nurses. Lack of confidence has been described as a source of uncertainty during the role transition from student nurse to practicing nurse. Cognitive apprenticeship (CA) is an instructional strategy that can facilitate ease of transition for newly graduated nurses (NGNs) during the clinical or preceptorship portion of hospital orientation. The purpose of this mixed methods pretest-posttest design was to explore if there would be a difference in confidence and ease of transition to practice, after completing a cognitive apprenticeship based hospital orientation. Newly graduated, newly hired registered nurses as well as experienced nurse preceptors were solicited for participation in this study. The Casey-Fink Graduate Nurse Experience Survey was administered as both the pretest and posttest. Qualitative data were also collected from focus group interviews of both newly graduated nurses and preceptors. The findings indicated that there was a statistically significant difference in confidence after a CA-based orientation. Furthermore, the findings revealed the importance of preceptor training for effective preceptorships. This study adds to the literature regarding transition to practice strategies as well as pedagogical strategies for preceptorships.

Feelings associated with student nurse transition can be eased by a variety of interventions. One important intervention is the maximization of the final practicum experience. The final practicum experience often provides student nurses vital time in the clinical setting, where they can solidify their knowledge and practice their nursing skills. To measure the effectiveness of the final practicum the Preparedness for Graduate Nursing Practice Questionnaire was used to determine levels of self-reported preparedness using a senior group of associate degree nursing students. The results confirm the importance of the final practicum experience properly preparing student nurses for their transition into professional practice. Study results confirm the importance of evaluating the effectiveness of the final practicum and tailoring it to meet the needs of student nurses to maximize potential benefits.
