Book excerpt: The Government of the Islamic Republic of Iran has stated its commitment to improving the health and nutritional status of the population as articulated in the Interim Country Assistance Strategy (CAS), which lists as priorities addressing nutritional deficiencies, expanding coverage of basic health services, and improving efficiency of the health system and the quality of service at all levels. This health sector review is consistent with this in that it provides the platform and evidence base for such reform. It is also in-line with part three of the current Fourth Five-Year Plan, which includes health development, human security and social justice priority, the reduction of illness from malnutrition, increasing public health service coverage, and increasing access to quality health services while reducing the financial burden on the families. The objective of this study is to provide a comprehensive review and diagnostic of the performance of the health sector in the Islamic Republic of Iran. The government is quite advanced in the type of analysis it already undertakes on various aspects of the sector (e.g., burden of diseases, national health accounts, and utilization analysis). However, a consolidation of this information and analysis that encompasses several major elements of the health sector has not been done recently. The health sector review, through a synthesis of available data and other information on the health sector attempts to: (i) assess the strengths, challenges, and opportunities facing the current health system; (ii) offer analytic assessments of the health policies and plans; and (iii) provide a framework for developing strategic options as well as short- and medium-term recommendations and action plans to achieve the goals identified in the country's Fourth Five-Year Plan. The review is also intended to provide a platform for discussions on possible areas of collaboration between the Government of the Islamic Republic of Iran and the World Bank on the health sector.

Book excerpt: The 2011 Arab uprisings and their subsequent aftermath have thrown into question some of our long-held assumptions about the foundational aspects of the Arab state. While the regional and international consequences of the uprisings continue to unfold with great unpredictability, their ramifications for the internal lives of the states in which they unfolded are just as dramatic and consequential. States historically viewed as models of strength and stability have been shaken to their foundations. Borders thought impenetrable have collapsed; sovereignty and territoriality have been in flux. This book examines some of the central questions facing observers and scholars of the Middle East concerning the nature of power and politics before and after 2011 in the Arab world. The focus of the book revolves around the very nature of politics and the exercise of power in the Arab world, conceptions of the state, its functions and institutions, its sources of legitimacy, and basic notions underlying it such as sovereignty and nationalism. Inside the Arab State adopts a multi-disciplinary approach, examining a broad range of political, economic, and social variables. It begins with an examination of politics, and more specifically political institutions, in the Arab world from the 1950s on, tracing the travail of states, and the wounds they inflicted on society and on themselves along the way, until the eruption of the 2011 uprisings. The uprisings, the states' responses to them, and efforts by political leaders to carve out for themselves means of legitimacy are also discussed, as are the reasons for the emergence and rise of Daesh and the Islamic State. Power, I argue, and increasingly narrow conceptions of it in terms of submission and conformity, remains at the heart of Arab politics, popular protests and yearnings for change notwithstanding. Much has changed in the Arab world over the last several decades. But even more has stayed the same.

Book excerpt: The COVID-19 pandemic has caused unprecedented disruption worldwide highlighting once again the interdependency of health and socioeconomic development, and the global lack of health systems resilience. Two years into the pandemic, most countries report sustained disruptions across service delivery platforms and health areas with a profound impact on health outcomes. The impact of these disruptions is magnified within marginalized communities and in countries experiencing protracted conflict. There is an urgent need to focus on recovery through investment in the essential public health functions (EPHFs) and the foundations of health systems with a focus on primary health care, and whole-of-government and -society engagement. The aim of this Research Topic is to gather, transfer and promote operationalization of key experiences from COVID-19 to inform global and country level recovery that better promote health; guide policy direction towards building health systems resilience; and thereby ensure economic and social prosperity. Experience with COVID-19 has demonstrated that traditional approaches to health system strengthening have failed to achieve the complementary goals of Universal Health Coverage (UHC) and health security with the divide between the most vulnerable and well-off only widening. Much of what had been learned from previous experiences such as Ebola in West Africa has not been widely applied. This has left health and economic systems vulnerable to 21 st century public health challenges, ranging from conflict and natural disasters to aging demographics and rising rates of non-communicable and communicable diseases and antimicrobial resistance. These challenges require intentional focus and investment as well as whole-of-government and -society engagement with health to build health system resilience. Greater action is needed to prevent the devastating effects of war and conflict on the health of the most vulnerable. This Research Topic will convene the knowledge and practices of leaders in public health, health systems, and humanitarian and development sectors. This is to ensure lessons from COVID-19 inform the recovery agenda and promote sustainable health and socioeconomic recovery for all. Lest we forget and find ourselves again unprepared and vulnerable in the face of an even greater threat.

Book excerpt: Geographic information systems represent an exciting and rapidly expanding technology via which spatial data may be captured, stored, retrieved, displayed, manipulated and analysed. Applications of this technology include detailed inventories of land use parcels. Spatial patterns of disease, geodemographics, environmental management and macroscale inventories of global resources. The impetus for this book is the relative lack of research into the integration of spatial analysis and GIS, and the potential benefits in developing such an integration. From a GIS perspective, there is an increasing demand for systems that do something other than display and organize data. From a spatial analytical perspective, there are advantages to linking statistical methods and mathematical models to the database and display capabilities of a GIS. Although the GIS may not be absolutely necessary for spatial analysis, it can facilitate such an analysis and moreover provide insights that might otherwise have been missed. The contributions to the book tell us where we are and where we ought to be going. It suggests that the integration of spatial analysis and GIS will stimulate interest in quantitative spatial science, particularly exploratory and visual types of analysis and represents a unique statement of the state-of-the-art issues in integration and interface.
