Book excerpt: The therapeutic use of glucocorticoids is associated with weight gain and may pl ay an important role in the initiation of obesity, an effect which is undesirabl e and even detrimental. Studies have shown that glucocorticoids increase the dep osition of glycogen in the liver, but it is not known whether this is caused by a decrease in breakdown and /or an increase in synthesis. The present experiment was designed to study the effect of a high pro tein-high fat diet versus a high protein-low fat diet on in vivo postprandial he patic glycogen synthesis of rats taking glucocorticoids. Thirty nine six-week ol d male Sprague-Dawley rats were randomly divided into five groups: C, control; H P-LF, high protein-low fat; HP-HF+G, high protein high fat with prednisolone; HP -HF, high protein-high fat; HP-HF+G, high protein-high fat prednisolone. Pellets containing 0.25mg of prednisolone each (60-day release) were implanted subcutan eously. The rats were then maintained on their respective diets for 6 weeks, and then fasted overnight. On the day of the experiment, each rat was given 1.25g o f its respective diet by gavage, immediately injected i.p. with 4mCi of H2O, sac rificed one hour later. Blood, liver and fat were taken for analysis. Results showed the plasma glucose concentrations of rats maintained on HP-LF diets were higher than those maintained on HP-HF diets and this was not a ffected by the presence of glucocorticoids. Glycogen content (mg/g liver) was si milar among all groups. Hepatic glycogen synthesis was higher in rats maintained on HP-LF diets compared with those on HP-HF diets and was not substantially aff ected by the presence of glucocorticoids. The percentage of glycogen synthesis v ia pyruvate (indirect pathway) was also higher in rats maintained on the low fat diets compared with those on high fat diets; glucocorticoids did not seem to pl ay a role in this case either. Under high protein conditions, hepatic glycogen synthesis as well as i ts synthesis via pyruvate was increased in the LF groups, and this was not alter ed by prednisolone. Therefore, dietary manipulation seems to be the main determi nant of glycogen metabolism, an effect which does not seem to be overridden by t he presence of glucocorticoids.

Book excerpt: Food intake is supposed to be controlled, to a certain extent, by metabolic factors including glycogen metabolism in the liver. Glutamine and dihydroxyacetone are substances known to increase hepatic glycogenesis. In obese individuals, macronutrient composition of the diet has little effect on the magnitude of weight loss over the short term unless nutrient composition affects caloric intake. Recent weight loss studies have shown the effectiveness of high protein diets in the obese but the complete physiological mechanism behind the success of these diets is still somewhat vague. The present study was designed to investigate the acute effect of different high protein diets, with or without glutamine and dihydroxyacetone, on hepatic glycogen synthesis and food intake of normal Sprague-Dawley rats. Thirty-two male Sprague-Dawley rats were randomly divided into five groups. Groups were divided according to diet (C: control, HPLF: high protein low fat, HPLF+GDHA: high protein low fat supplemented with glutamine and dihydroxyacetone, HPHF: high protein high fat, HPHF+GDHA: high protein high fat supplemented with glutamine and dihydroxyacetone). Rats were ad lib fed the appropriate diet for 3 days. On the last day, they were fasted overnight, tube fed the following morning with 1.25g their respective diets, injected i.p. with 3mCi of 3H20 and sacrificed one hour later. Plasma and tissue samples were collected. Results showed a significant decrease in food intake and weight gain in HPHF+GDHA group. Postprandial plasma insulin level was also the lowest in the HPHF+GDHA group. Postprandial hepatic glycogen synthesis was higher in GDHA supplemented diets when compared to the unsupplemented high protein diets. In conclusion, high protein, high fat diet supplemented with glutamine and dihydroxyacetone resulted in a decrease in weight gain and food intake possible due to diarrhea. This was associated with an increase in postprandial glycogen synthesis.
