Considerable thought, energy and resources is given over to devising a strategic plan; and it is to the fine detail of the plan that attention turns when things go wrong. The plan, rather than the implementation comes in for scrutiny, because it is less problematic to analyse. Perhaps this is because, whereas the plan can be devised under pollution-free, almost laboratory conditions, the working out of the plan takes place in the real world infected with real world variables. But the whole point of a strategy is that it will be implemented and implemented successfully. Much has been written on change management and transformational leadership all very relevant to strategy implementation but there are comparatively few books which look at the implementation process as a whole. The Implementation Challenge aims to link the numerous concepts and approaches to the subject but recognizes the impact on implementation of the organization as it lives and breathes. From the work of leading thinkers in the field of strategic management, David Hussey has selected contributions specifically focusing on the theme of implementation. Book excerpt: In 1996, the Institute of Medicine (IOM) released its report Telemedicine: A Guide to Assessing Telecommunications for Health Care. In that report, the IOM Committee on Evaluating Clinical Applications of Telemedicine found telemedicine is similar in most respects to other technologies for which better evidence of effectiveness is also being demanded. Telemedicine, however, has some special characteristics-shared with information technologies generally-that warrant particular notice from evaluators and decision makers. Since that time, attention to telehealth has continued to grow in both the public and private sectors. Peer-reviewed journals and professional societies are devoted to telehealth, the federal government provides grant funding to promote the use of telehealth, and the private technology industry continues to develop new applications for telehealth. However, barriers remain to the use of telehealth modalities, including issues related to reimbursement, licensure, workforce, and costs. Also, some areas of telehealth have developed a stronger evidence base than others. The Health Resources and Service Administration (HRSA) sponsored the IOM in holding a workshop in Washington, DC, on August 8-9 2012, to examine how the use of telehealth technology can fit into the U.S. health care system. HRSA asked the IOM to focus on the potential for telehealth to serve geographically isolated individuals and extend the reach of scarce resources while also emphasizing the quality and value in the delivery of health care services. This workshop summary discusses the evolution of telehealth since 1996, including the increasing role of the private sector, policies that have promoted or delayed the use of telehealth, and consumer acceptance of telehealth. The Role of Telehealth in an Evolving Health Care Environment: Workshop Summary discusses the current evidence base for telehealth, including available data and gaps in data; discuss how technological developments, including mobile telehealth, electronic intensive care units, remote monitoring, social networking, and wearable devices, in conjunction with the push for electronic health records, is changing the delivery of health care in rural and urban environments. This report also summarizes actions that the U.S. Department of Health and Human Services (HHS) can undertake to further the use of telehealth to improve health care outcomes while controlling costs in the current health care environment.

This manuscript brings together existing research, along with policy analysis, in order to look at disaster recovery through the lens of justice. This includes understanding the mechanisms through which vulnerability is exacerbated, and the extent to which the regulations and agency cultures drive this outcome. While existing analyses have sought to understand the particular characteristics of both resilient and vulnerable communities, there have been few attempts to understand the systemic inequities and injustice that is built into United States disaster policies, programs, and legislation. This manuscript thus begins from the understanding that social and economic structures, including land use policies and historic practices such as redlining, have concentrated hazard risk into vulnerable zones whose inhabitants do not benefit from the very policies that create and increase their risk.

As a relatively new and, until recently, rather neglected field within the health sector, implementation research is something of an unknown quantity for many. There is therefore a need for greater clarity about what exactly implementation research is, and what it can offer. This Guide is designed to provide that clarity. Intended to support those conducting implementation research, those with responsibility for implementing programs, and those who have an interest in both, the Guide provides an introduction to basic implementation research concepts and language, briefly outlines what it involves, and describes the many opportunities that it presents. The main aim of the Guide is to boost implementation research capacity as well as demand for implementation research that is aligned with need, and that is of particular relevance to health systems in low- and middle-income countries (LMICs). Research on implementation requires the engagement of diverse stakeholders and multiple disciplines in order to address the complex implementation challenges they face. For this reason, the Guide is intended for a variety of actors who contribute to and/or are impacted by implementation research. This includes the decision-makers responsible for designing policies and managing programs whose decisions shape implementation and scale-up processes, as well as the practitioners and front-line workers who ultimately implement these decisions along with researchers from different disciplines who bring expertise in systematically collecting and analyzing information to inform implementation questions. The opening chapters (1-4) make the case for why implementation research is important to decision-making. They offer a workable definition of implementation research and illustrate the relevance of research to problems that are often considered to be simply administrative and provide examples of how such problems can be framed as implementation research questions. The early chapters also deal with the conduct of implementation research, emphasizing the importance of collaboration and discussing the role of implementers in the planning and designing of studies, the collection and analysis of data, as well as in the dissemination and use of results. The second half of the Guide (5-7) detail the various methods and study designs that can be used to carry out implementation research, and, using examples, illustrates the application of quantitative, qualitative, and mixed-method designs to answer complex questions related to implementation and scale-up. It offers guidance on conceptualizing an implementation research study from the identification of the problem, development of research questions, identification of implementation outcomes and variables, as well as the selection of the study design and methods while also addressing important questions of rigor.

In many algorithmic domains, worst-case bounds are too pessimistic and tractable probabilistic models too unrealistic to provide meaningful predictions of practical algorithmic performance. Experimental approaches can provide knowledge where purely analytical methods fail and can provide insights to motivate and guide deeper analytical results. The DIMACS Implementation Challenge was organized to encourage experimental work in the area of network flows and matchings. Participants at sites in the U.S., Europe, and Japan undertook projects between November 1990 and August 1991 to test and evaluate algorithms for these problems. The Challenge culminated in a three-day workshop, held in October 1991 at DIMACS. This volume contains the revised and refereed versions of twenty-two of the papers presented at the workshop, along with supplemental material about the Challenge and the Workshop.
