Book excerpt: When the learning style variables were entered into a regression model, the variance in clinical judgment was influenced by the way an individual reflects and transforms the experience. The study's findings did not indicate a significant relationship between learning flexibility and clinical judgment. Based on the findings, a prelicensure nursing student's learning style may influence clinical judgment within a human patient computer simulation environment. Further research is recommended to examine the relationship of clinical judgment and learning style from a developmental perspective throughout the nursing program curriculum and explore the role of learner flexibility in supporting varied instructional design approaches.

Book excerpt: New nurse graduates may be ill-prepared to care for multiple patients in the acute care setting due to limited clinical experience. Nurse educators must find ways of bridging the theory-to-practice gap. High-fidelity simulation is one mechanism for bridging this gap. This pilot study explored the effect of multi-patient simulation (MPS) on students' self-efficacy and perceived clinical judgment. This quasi-experimental study sought to explore the readiness of BSN students to care for multiple patients through the use of MPS and a self-efficacy survey measuring perceived level of confidence (LOC) in clinical care. Findings were inconclusive regarding whether MPS as a teaching method helped nursing students increase their perceived LOC and improve clinical judgment skills. Students agreed that the design should be part of nursing education and it identified gaps in their knowledge. This study raised an awareness of what students lack in terms of prioritization management when caring for multiple patients.

Book excerpt: Simulation has been identified as an effective approach for preparing nursing students to transition from theory to practice. It is widely accepted that high-fidelity patient simulation has a positive impact on student learning and decision-making abilities. Computer-based simulation may be an effective alternative to the high cost of high-fidelity patient simulators. The purpose of this quasi-experimental study was to examine if the use of an interactive computer simulation video had an impact on the clinical judgment of undergraduate nursing students. This study also examined whether there was a relationship between students' demographic and academic characteristics and their clinical judgment scores. A sample of 50 community college associate degree nursing students participated in this study. The students were placed into experimental (N=18) and control (N=32) groups. An interactive computer-simulation video was the intervention used for the experimental group. An independent samples t test was conducted to compare group findings. All participants completed a demographic/academic form including age, GPA, current working hours, and previous degrees to examine if a relationship existed between demographic and academic variables and clinical judgment scores as measured by the Lasater Clinical Judgment Rubric. The study found no significant differences in clinical judgment scores between students who used the interactive computer-simulation video and those who did not. The study also did not find any relationship between student demographic and academic variables and clinical judgment scores.

Book excerpt: Nurses are challenged to use critical thinking skills and make sound clinical judgment in achieving optimal patient outcomes and improving patient safety (Alfaro-LeFevre, 2017). Clinical judgment is the ability to interpret patient data, and develop appropriate and timely interventions (Bussard, 2018). The effect of simulation on the development of clinical judgment has been investigated extensively in traditional nursing students. The rapid development of second-degree nursing students to address the nursing shortage (Sedgwick, Kellet, & Kalischuck, 2014), requires further investigation of the effect of simulation on the development of clinical judgment in second-degree nursing students. Opportunities for simulation experiences promote the development of clinical judgment in second-degree nursing students (Mariani, Cantrell, Meakin, & Jenkinson, 2015). This study encompassed a regularly scheduled simulation experience for students enrolled in a gerontology course in a second-degree nursing program. The study used a cross sectional design and encompasses an educational preparation video and demographic survey prior to the simulation. The Lasater Clinical Judgment Rubric (Lasater, 2007) was used post debriefing and reflection to assess the effect of the simulation on the development of clinical judgment. Statistical analysis using SPSS software was used for data analysis, results, and understanding the implications for translating the evidence into practice.

Book excerpt: The research question of this study was: to what degree do nursing students perceive using the High Fidelity Simulation (HFS) learning environment to be helpful in their ability to achieve clinical competency. The seven research sub-questions explored the students' demographics as an influence on rating of "Reality" and "Helpfulness" and the correlation between the students' rating of reality in their HFS learning experience and their rating of "Helpfulness" of achieving clinical competencies as related to their ability to learn. The purpose of this study was to explore and describe the phenomena of student perceptions of learning in the simulation environment and the role of the level of "Reality". The significance of the study is the lived experience of the nursing student in the High Fidelity Simulation learning environment is better understood. The detailing of relationships between the study variables and the strength of those relationships may provide guidance for educators to direct their efforts more effectively in teaching and learning. This research used the research approach of a sequential mixed methods descriptive study: survey and focus groups. the data analysis reveals that for eight of the eleven items in Section II of the survey, which covered Role Expectations and Clinical Competencies, the students rated the simulation learning environment in the Helpful range, least Helpful; "Evidenced Based Practice", most helpful; "Teamwork and Collaboration". A second statistically significant correlation (r = .66) revealed the more real the student perceived the simulation environment they also rated more highly the "Helpfulness" of the environment in achieving clinical competencies. The positive correlation suggests that the more real the student perceives the simulation learning environment to be, the more helpful they found the environment in achieving clinical competencies. Another statistically significant finding (r = .62) : the more real the student perceived the simulation environment to be they also reported more strongly that the level of "Reality" had an impact on their ability to learn. Educators may use this new knowledge for making improvements to the learning environment in respect to why some competencies were perceived to be more challenging and others less so.

Book excerpt: Research literature provides evidence that new graduate nurses are often deficient in clinical judgment (CJ). One way to increase CJ is by using simulations. However, the literature is replete with descriptions of the high anxiety that simulation triggers. It is not currently known how anxiety in simulation affects clinical judgment for undergraduate nursing students. Therefore, the purpose of this study was to explore the effect of different types of anxiety on the clinical judgment of undergraduate nursing students in simulation. This research project used a one-group repeated measures quantitative design to answer the research questions using the conceptual framework of Tanner's (2006) model of clinical judgment. A convenience sample of 45 sophomore-level undergraduate nursing students participated in a study to explore how state and trait anxiety impacted their clinical judgment within an introductory simulation. The results indicated that anxiety did not have a significant impact on clinical judgment. When controlling for baseline state and trait anxiety, pre-simulation anxiety level did not significantly predict scores on the Lasater Clinical Judgment Rubric (LCJR) within the simulation. State anxiety did change significantly between the three time measurements, going up to significantly high levels at pre-simulation. These anxiety levels remained high at post-simulation. The findings imply a changed focus to reframe how anxiety is thought about and its effects. Some anxiety is good and facilitative, and therefore, faculty should not be so worried about reducing anxiety for all students. Rather, nursing educators should help students function despite anxiety, in order to prepare them for real world nursing practice.

Book excerpt: Nursing programs are increasingly facing issues with access to appropriate clinical sites, access to properly credentialed nursing faculty, and concerns regarding graduates readiness to practice safely. These issues have led to increased use of high fidelity simulation (HFS) experiences to replace traditional clinical rotations (Benner, Sutphen, Leonard & Day, 2010; Lasater, 2007a; Orsolini-Hain & Malone, 2007). Previous research has shown that traditional clinical rotations are instrumental in development of clinical reasoning skills and HFS experiences are valuable in developing technical skills. However, it does not provide insight into the impact of HFS experiences on development of clinical reasoning skills (Lapkin, Levett-Jones, Bellchambers & Fernandez, 2010). The purpose of this quasi-experimental pilot study was to determine the relationship between HFS clinical experiences and development of clinical reasoning skills in nursing students. Findings will influence the use of HFS to replace traditional clinical experiences and provide a foundation for future research on the efficacy of HFS in development of clinical reasoning skills. Analysis of data showed that there was no significant statistical difference in development of clinical reasoning skills between prelicensure nursing students attending traditional clinical experiences and those participating in simulated clinical experiences. This teaching methodology is a standard supplement to clinical experiences for nursing students to meet the requirements of clinical components within their degree program; however, the faculty of a small liberal arts-based Bachelor of Science in Nursing program was not using simulation to its fullest potential. Therefore, the purpose of this DNP project was to introduce the use of evidence-based practice simulation guidelines recommended by the National Council of State Boards of Nursing. The study sample of 16 included 10 full-time faculty, five part-time adjunct faculty, and one simulation lab coordinator. Implementing the educational training using the guidelines took place over 3 months in the spring of 2021. Participants completed the Faculty Attitudes and Adoption of Simulation assessment before and after implementing the education program. Additionally, participants completed an Educator’s Self-Efficacy questionnaire after the educational training was completed. The study revealed that formal educational training positively affects faculty attitudes and adoption of simulation. Thus, results of this project suggest that continuing education works and is necessary to develop new knowledge based on evidence-based practice.
