Book excerpt: Since the 1980s HIV/AIDS has occupied a singular position because of the rapidly emergent threat and devastation the disease has caused, particularly in sub-Saharan Africa. New infections continue to create a formidable challenge to households, communities, and health systems: last year alone, 2.7 million new infections occurred globally. Sub-Saharan Africa remains the epicenter of the suffering, with around two-thirds of infected individuals worldwide found there, and a disproportionate number of deaths and new infections. For years there have been widespread and concerted efforts to prevent the spread of HIV/AIDS, identify a cure, and understand and mitigate the deleterious social and economic ramifications of the disease. Despite these efforts, and some apparent successes, there is still a long way to go in terms of altering behaviors in order to realize the objective of dramatic reductions in the spread of HIV/AIDS in Africa. The authors in this volume examine the HIV/AIDS crisis in Africa, which persists despite major strides in averting deaths due to antiretroviral therapy. They tell an important story of the distinct nature of the disease and its socioeconomic implications.

Filling that gap, this book brings together recent evidence of AIDS impacts on rural households, livelihoods, and agricultural practice in sub-Saharan Africa. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. In the face of the impact of the epidemic as described in parts one and two of the series, the agricultural sector can play an important role in mitigating some of its effects. Strategies for agricultural intervention are of particular importance in sub-Saharan Africa, given the fact that most of the countries hardest hit by the epidemic are heavily reliant on agriculture. Different agricultural sector based mitigation strategies are discussed. It is further argued, however, that such interventions need to be complemented by interventions from the health and other sectors, in particular by treatment regimes including access to anti-retroviral drugs. The text is followed by an annotated bibliography.

Book excerpt: In all of the major challenges facing the world currently, whether it be climate change, terrorism and conflict, or urbanization and demographic change, no progress is possible without the alleviation of poverty. New approaches in development economics have in recent years started from the premise that we cannot successfully deal with poverty unless we also deal with vulnerability—but not only vulnerability to income poverty but also vulnerability to various others hazards—such as climate, conflict, macroeconomic shocks and natural disasters. This book provide insights into new approaches in conceptualising and measuring vulnerability. It includes chapters dealing with advanced issues such as the compilation of economic vulnerability indices (EVIs) on a macro-level, of conceptualizing and measuring local vulnerability across regions in a country, and of measuring the flip-side of vulnerability, namely resilience. The book also explores the sensitivities of the various measurements of vulnerability to vulnerability lines, poverty lines, and permanent income, with consideration to some of the most vulnerable groups in developing countries. Overall, the contributions in the book consolidate new approaches as far as the concept and measurement of vulnerability on different levels and outcomes are concerned, and note directions for future research. This book was published as a special issue of Oxford Development Studies.
