Book excerpt: Early Childhood Caries (ECC) is a serious disease that is about much more than cavities on baby teeth. In Canada, it is a growing public health problem with adverse long-term effects on children's physical, emotional and intellectual well-being. The failure to invest in preventive care has resulted in reactive, rather than proactive, measures against this disease. These measures are expensive and a needless drain on costs in the public health-care system. Children with severe ECC end up in hospital; in fact, in Canada, this disease is the most common reason children undergo day surgery. From 2010 to 2012, one in 100 children under age five required day surgery for ECC, with approximately 19,000 of these surgeries performed each year on children under age six. Canadian hospital costs for ECC day surgery in children aged one to five ranged from $1,271 to $1,963 per child, totalling $21.2 million between 2010 and 2012. Children from low-income families, along with aboriginal, immigrant and refugee children are disproportionately affected by dental disease, with between 50 per cent and 90 per cent of suffering from some form of ECC. This compares to an average of 57 per cent of children affected in the general population. A recent Alberta study indicates that when municipalities cease fluoridating their water supplies, children suffer increased levels of tooth decay. This has reignited the discussion around whether municipalities should add fluoride to the drinking water, or reinstate it in places where the water used to be fluoridated. While fluoridation can be an effective prevention strategy, this study also shows that fluoride alone is not enough. To reduce the costs and developmental consequences associated with severe ECC and improve well-being, oral health policies focused on disease prevention and health promotion are still necessary. This briefing paper provides background on the etiology, risk factors and prevalence of ECC in Canada to provide scope for the magnitude of this preventable disease in children. To address the avoidable socio-economic costs, three areas require policy development. First is the need for increased public education and access to ECC prevention services for at-risk populations. Parents need to know they should reduce their children's intake of sweet drinks, and avoid filling bottles with sugar water, juice or soft drinks, especially at night. They should also clean an infant's gums with a soft toothbrush or cloth and water starting at birth. When the baby's first tooth erupts, parents should commence daily brushing with toothpaste and book a first dental visit. Second is the need to empower health-care professionals to integrate ECC prevention in their early visits with parents of young children. Such visits are more common in family medicine, and these primary care providers can play a critical role in educating parents and promoting children's oral health. Curriculum and continuing education for these health professions should be enhanced to emphasize ECC's long-term health effects. Third, government should invest in preventive oral health services for children rather than relying on emergency dental care. Children should have access to early preventive dental services to instill in them habits for lifetime oral health. Provinces without universal public funding for children's preventive dental health should remove the access barriers that children without dental insurance face.

Book excerpt: Background: Early Childhood Caries (ECC) is one of the most common chronic illnesses among young children, affecting around 23% of children 2-5 years old, leading to pain, discomfort, and poor quality of life. It is a multi-factorial disease that develops through the combined effects of bacteria, tooth morphology, fermentable carbohydrates, time, and various social factors. Several studies have investigated the associations between dental caries and non-traditional factors acquired during the first years of life including; mode of delivery, breastfeeding, and Environmental Tobacco Smoke (ETS). However, the literature on these associations has been inconsistent. Objective: To investigate the unadjusted and adjusted associations between the presence of Early Childhood Caries (ECC) and Severe-ECC (S-ECC), and three non-traditional factors: breastfeeding, mode of delivery, and ETS, among children 1-5 years old. Methods: A cross-sectional design was employed, and a sample of 112 caregiver/ child dyads was recruited from the ongoing flow of patients at Temple University Maurice H. Kornberg School of Dentistry (TUKSoD). After consent, subjects completed a questionnaire and received a standard intra-oral examination and the American Academy of Pediatric Dentistry (AAPD) Caries-risk Assessment Tool (CAT) by a student doctor. The study was approved by the Temple University Institutional Review Board (Protocol # 23885). Chi-square tests, two-sample t-tests and bivariate logistic regressions were used to assess the unadjusted associations. Two multivariable logistic models were developed for ECC and S-ECC and included demographics, overall CRA, and the three non-traditional risk factors. Results: The prevalence of ECC and S-ECC were 61% and 30%, respectively. The following variables were significant in the unadjusted analysis for both ECC and S-ECC: child's age, maternal educational attainment, overall AAPD CAT classification, sugary snacks per day, presence of plaque on child's teeth and ETS. Exposure to ETS was associated with an increased adjusted odds ratio for ECC (aOR=5.39 [95% CI: 1.14-25.33], P=0.033), but not for S-ECC. Furthermore, C-section birth was associated with a decreased adjusted odds ratio for both ECC and S-ECC, respectively (ECC: aOR=0.132 [95% CI: 0.02-0.72], P=0.02; S-ECC: aOR=0.141 [95% CI: 0.026-0.748], P=0.021). With inclusion of the AAPD CAT, demographics, and the three non-traditional factors, the overall model accuracy at predicting ECC was 82.2%. Conclusions and Clinical Relevance: In this study of urban, predominantly African American, and low income children, ECC was found associated with two non-traditional factors, ETS and mode of delivery, suggesting that including them in CRA may improve prediction of future dental caries, and aid in the prevention and treatment of disease. Results from this study support the notion that ECC is a multi-factorial disease, and highlights the importance of adopting oral health education among caregivers.

Most oral diseases are preventable, but still common. Early Childhood Caries (ECC), the most common childhood disease, is linked to periodontal disease, poor academic performance, and increased likelihood of chronic disease in adulthood and poor quality of life for the children who are affected and their families. ECC disproportionately affects low-income and minority children. Head Start is a federal preschool program aimed to identify and provide support for low-income children and their families. Due to the federal mandates for Head Start grantees, children participating in these programs are in a position to receive increased access to dental care through oral screenings and referrals to dentists for oral health treatment. Studies have shown that Head Start enrollees are almost three times more likely to obtain a dental screening than those enrolled in other preschool centers. Even with these parameters in place, a great amount of tooth decay is still present in Head Start children. In a brief issued by the Children's Dental Health Project, "Keeping Health in Head Start: Lessons Learned from Dental Care", one of the key lessons was Medicaid dental benefits are important in granting access to dental care for Head Start children, however it is not enough. In order for Medicaid benefits have a major impact, dental professionals must accept the coverage, and the Head Start community must have knowledge of those that can and will provide services to their children. Purpose: The purpose of this study is to 1) examine the oral health status of low-income children in MS Head Start Programs and 2) evaluate linkage to care between the HSFs and the referral dentists, and 3) identify opportunities for Head Start children to find a dental home. This study uses descriptive and inferential statistical methods to test the objectives, hypothesis and specific aims outlined below: Methods: The study used the oral screening data and parental survey data from the Mississippi State Department of Health's Make a Child's Smile Program for the time period 2009-2014, Medicaid reimbursement data for children's dental claims, and licensure data from the Mississippi Board of Dental Examiners. Spatial attributes such as the location of the Head Start Facilities, service locations of referral dentists named in the MACS dataset, other non-referred dental service locations that have filed children's dental Medicaid claims, and calculated distances from Head Start facilities to the aforementioned dental service locations were compiled and analyzed. This information was used to examine the dental workforce available to the Head Start Facilities in the study sample. Both traditional statistical tests and spatial analysis were used to analyze data and create a visualization of the oral health landscape for low-income children in MS. Evidence indicates that chronic diseases, and adverse childhood experiences (ACEs), have destructive impacts on overall quality of life, health outcomes, and health care expenditures. The ACE investigation identified cumulative dose-response relationships between ACEs and development of diverse health and social consequences later in life. Research is scarce on investigating causal pathways to facilitate contextualized public health interventions addressing this seismic threat. Data was utilized from the 2011-2012 National Survey of Children's Health, inclusive of ages 1-17 for dentate status. The dependent variables identified untreated oral health care needs and preventive dental utilization. The key independent variable, ACEs, included exposure to parental death, parental divorce, parental incarceration, mental health illnesses, domestic violence, neighborhood violence, and racial discrimination. Exogenous variables included age, sex, race/ethnicity, number of children in household, socioeconomic status proxies, health insurance status, and presence of special health care needs. Path analysis, a special subcomponent of structural equation modeling was utilized to explore direct, indirect, and mediating causal pathways. The data, when adjusted for complex survey design, proportionately represents children in the United States. The results of the adjusted logistic regressions revealed ACEs demonstrating varying magnitudes of significance across diverse racial and ethnic profiles. Exposures to parental divorce and parental death particularly exhibited critical magnitudes of influence. Adjusted path analyses demonstrated alternative family structures contributing a mediating role between ACEs and oral health-related quality of life (OHRQoL). In keeping with the Pareto Principle, exposure to certain ACEs, namely parental divorce and parental death, potentially introduces more profound social and health-related consequences later in life. Moreover, invocation of Occam's razor, can assist in theorizing that exposure to alternative family structures (i.e. via parental divorce and/or parental death) initiate a concatenated deteriorating domino effect sequela of secondary ACEs. Therefore, contextualized interventions should prioritize psychotherapeutic child, marriage, and family counseling services improving the home, as an enabling environment, to potentially minimize detriments of ACEs on OHRQoL.
