Book excerpt: This study was designed to assess the independent contributions of cardiorespiratory fitness (CRF) and adiposity to the metabolic health of obese individuals. It specifically focused on how body composition and CRF affect the relative contribution of glucose, fat and protein to energy production (EP) in resting state as well as during and after exercise. Eight obese fit (OF - 30±12 yrs., 178±6 cm, 105±8 kg) and eight lean fit (LF - 36±9 yrs., 181±5 cm, 82±5 kg) were screened for medical conditions and food/health supplement use and partook in two sessions - separated by 72 hr - that consisted of the following measurements: (1) a basal metabolic rate (BMR) in a fasting state for the determination of substrate oxidation (baseline) followed by an incremental running test for the determination of V̇O2max and of exercise intensity that elicits maximal fat oxidation (MFO); (2) a BMR, an iso-caloric (300 kcal expenditure) treadmill exercise at MFO, a post-exercise metabolic rate (PEMR) in fasting state (30 min after exercise), and two PEMR in the fed state at 90 and 150 min after exercise. The current epidemic of overweight and obesity in the United States affects the military services. The pool of available recruits is reduced because of failure to meet body composition standards for entry into the services and a high percentage of individuals exceeding military weight-for-height standards at the time of entry into the service leave the military before completing their term of enlistment. To aid in developing strategies for prevention and remediation of overweight in military personnel, the U.S. Army Medical Research and Materiel Command requested the Committee on Military Nutrition Research to review the scientific evidence for: factors that influence body weight, optimal components of a weight loss and weight maintenance program, and the role of gender, age, and ethnicity in weight management.

Scientists have established that caloric intake, macronutrient composition of the diet, basal and resting metabolic rate, thermic response to food, energy expenditure associated with movement and physical activity, and preferential storage of the surplus of calories as fat or lean tissues are critical determinants of energy balance and body weight. While much has been learned, the field is poised for major advances with the advent of a variety of imaging techniques, progress in quantitative and molecular genetics, use of transgenic rodent models and of breeding experiments with informative inbred strains, availability of stable isotopes for metabolic and behavioral studies, and a growing number of useful experimental animal and human models. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. Book excerpt: Energy balance can be maintained by adapting energy intake to changes in energy expenditure and vice versa, where short-term changes in energy expenditure are mainly caused by physical activity. Questions are whether physical activity is affected by over and under-eating, is intake affected by an increase or a decrease in physical activity, and does overweight affect physical activity? Presented evidence is largely based on studies where physical activity is quantified with doubly labeled water. Overeating does not affect physical activity while under-eating decreases habitual or voluntary physical activity. Thus, it is easier to gain weight than to lose weight. An exercise induced increase in energy requirement is compensated by intake while a change to a more sedentary routine does not induce an equivalent reduction of intake and generally results in weight gain. Overweight and obese subjects have similar activity energy expenditures than lean people despite they move less. There are two options to reverse the general population trend for an increasing body weight, reducing intake or increasing physical activity. Based on the results presented, eating less is most effective for preventing weight gain, despite a potential negative effect on physical activity when reaching a negative energy balance.​
