Book excerpt: As essential nutrients, sodium and potassium contribute to the fundamentals of physiology and pathology of human health and disease. In clinical settings, these are two important blood electrolytes, are frequently measured and influence care decisions. Yet, blood electrolyte concentrations are usually not influenced by dietary intake, as kidney and hormone systems carefully regulate blood values. Over the years, increasing evidence suggests that sodium and potassium intake patterns of children and adults influence long-term population health mostly through complex relationships among dietary intake, blood pressure and cardiovascular health. The public health importance of understanding these relationships, based upon the best available evidence and establishing recommendations to support the development of population clinical practice guidelines and medical care of patients is clear. This report reviews evidence on the relationship between sodium and potassium intakes and indicators of adequacy, toxicity, and chronic disease. It updates the Dietary Reference Intakes (DRIs) using an expanded DRI model that includes consideration of chronic disease endpoints, and outlines research gaps to address the uncertainties identified in the process of deriving the reference values and evaluating public health implications.

Intakes below the UL are unlikely to pose risks of adverse health effects in healthy people. This new framework encompasses both essential nutrients and other food components thought to pay a role in health, such as dietary fiber. It incorporates functional endpoints and examines the relationship between dose and response in determining adequacy and the hazards of excess intake for each nutrient.

In the Nei Ching, the oldest of the extant medical writings, the Yellow Emperor observed, "Hence, if too much salt is in the food, the pulse hardens, tears make their appearance, and the complexion changes". At about the same period in history, Job asked the question, "Can that which is unsavory be eaten without salt?" It is not apparent whether or not the Almighty provided a clear answer. The connection between dietary salt intake and hypertension was appreciated following the observations of AMBARD, BEAUJARD, VOLLHARD, ALLEN, and others. However, DAHL emphasized this relationship, as demonstrated by his epidemiological observations, his studies in human subjects, and his development of a genetically mediated form of salt-sensitive hypertension in rats. DAHL and his followers argued that hypertension was a disease of acculturation, or even of self-abuse. Undaunted by skeptics such as PICKERING, they suggested that if Western man would merely curtail his intake of the granular condiment, hypertension would not develop and blood pressure would not increase with age. Bucolic native societies were given as examples where such cardiovascular bliss was readily attained.

A short account of the history of the effort led to the Phosphate Workshops is appro priate and can be of interest to the reader. The idea for Phosphate Workshops was born in the early days of November, 1974. One of us (S. G. M. ) suggested the thought to a group of scientists gathered for a luncheon in one of the attrac tive small restaurants in Weisbaden, Germany. The purpose of the workshop was to bring together interested scientists to discuss the newer developments and the recent advances in the field of phosphate metabolism and the other related minerals. An Organizing Committee made of Shaul G. Massry (USA), Louis V. Avioli (USA), Philippe Bordier (France), Herbert Fleisch (Switzerland), and Eduardo Slatopolsky (USA) was formed. The First Workshop was held in Paris during June 5-6, 1975 and was hosted by Dr. Philippe Bordier. Its proceeding was already published. The Second Workshop took place in Heidelberg during June 28-30, 1976 and was hosted by Dr. Eberhard Ritz. Both of these workshops were extremely successful scientific endeavors, and the need for them was demonstrated by the great interest they generated among the scientific community. The Or ganizing Committee, therefore, decided to continue with the tradi tion to hold additional Workshops annually or every other year.

As a result, American blacks have a higher incidence of stroke (50%), heart disease (30%), and kidney disease (50%). Not only are blacks more likely to develop hypertension, but the disorder develops earlier, is often more severe, and is more likely to be fatal at an earlier age. While lack of early and aggressive treatment contributes to the problem, research has shown that physiological and environmental factors play an important role. Pathophysiology of Hypertension in Blacks examines much of the research that has been done to explain the pathogenesis of hypertension among black Americans. The book is divided into four sections. The first section considers genetic mechanisms of the disease. Increased sensitivity to salt, a common feature among both normotensive and hypertensive blacks, may have developed during the slave trade and slavery as a physiological adaptation to prevent death from excessive loss of salt and water; survival favored those most able to conserve salt, an ability which predisposes black Americans today to hypertension. During childhood, this enhanced salt-sensitivity may be complicated by insulin resistance and hyperinsulinemia. The second section examines the role of social, cultural, psychosocial, and socioeconomic factors in the pathogenesis of hypertension. The authors of these chapters present models and explanations that show how these factors may influence physiological variables. The third area of the book deals with the role of urbanization and salt (both in and out of Africa), the role of diet, the role of intracellular ion metabolism, and the increasing significance of renin. The last section of the book summarizes the evidence presented in earlier chapters, and also outlines therapeutic strategies that are effective in controlling blood pressure in hypertensive blacks. The book presents underlying physiological mechanisms which may become impaired and therefore sets the stage for the application of modern molecular biology to the pathophysiology of hypertension in blacks. This book is a volume in the Clinical Physiology Series of the American Physiological Society and is based on a symposium sponsored by the Society at the 1990 meeting of the Federation of American Societies for Experimental Biology. It will be valuable to both researchers and clinicians who study and treat hypertension in blacks.
