Book excerpt: New graduate registered nurse residency programs are being adopted as best practice by many organizations. The purpose of this thesis was to evaluate the effectiveness of two different types of programs implemented at an organization to see which produced the largest amount of retention. One group was hired directly into a nursing unit or area where orientation began. Another group was hired into a practice area and rotated through three different areas prior to being matched to a unit or area and then completing orientation. Dr. Patricia Benner's Novice to Expert Framework was used to guide this study because successful residency programs should expound onto the new graduate registered nurses educational base and should offer a variety of experiences to assist with moving towards the stage of expert nurse. Retention data was collected retrospectively and included 12 months of retention data for the two cohort groups being compared. Comparison of this data showed a 4.5% increase in retention from one to another. Further comparison of this data showed that the difference in length of service at 12 months between these two cohort groups was not statistically significant (p=.285). Further comparisons of independent hospitals and practice areas also showed no statistically significant differences in overall length of service at 12 months.

Book excerpt: The Future of Nursing explores how nurses' roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America's increasingly complex health system. At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: Nursing residency programs are a tool used by hospitals to attract and retain new nursing employees. They were developed to ease the transition of a graduate nurse into the roles expected of a registered nurse in an acute care setting. In 2000, a hospital system in Washington state implemented a residency program for eligible new employees hired into the medical, surgical, and orthopedic units of two hospitals owned and perated by the system. This project evaluates the impact of this residency program in attracting and retaining new nursing employees. The study also explores the value of a registered nurse in the acute care setting and the effectiveness of preceptors in the orientation process. Methods used in this study include Likert Scale surveys that focus on the orientee's perception of preceptror support throughout the orientation process. Data supplied from the Human Resource Department was used in examining the effects of the program on retention rates. The results of the project suggest that residency programs may attract new nurses to health care facilities and bolster their self-confidence in the patient-care setting.

Book excerpt: New graduate nurses encounter "transition shock" when they enter the workplace, especially in acute care settings such as hospitals. New graduate nurses have a difficult time transitioning into the work environment because they are unprepared by their academic training for the realities they will face and they often don't have the proper support in place to guide them through the transition. This leads to a high attrition rate of up to 30% within the first 18 months of employment which costs hospitals and health care facilities. High nurse turnover of new graduate nurses who aren't prepared for the complexities of clinical practice affect the quality of care provided. Patient outcomes are negatively affected because it often leaves hospitals dealing with a shortage of experienced nurses, which they compensate for by having high patient to nurse ratios. New graduate nurses must develop the necessary critical thinking skills to handle higher acuity patients and difficult situations, develop confidence in their clinical skills, and adapt to peer relationships. One solution to the "transition shock" that new graduate nurses face is the implementation of nurse residency programs. Nurse residency programs are shown to help new graduates have a better transition into the workplace and increase nurse retention. This paper discusses the problem of new graduate nurse turnover, the benefits of nurse residency programs, and the numerous research studies which have been done showing the success of nurse residency programs on new nurse graduate retention and job satisfaction. It also discusses how to implement a nurse residency program and monitor and evaluate its success.

This MSN project introduces an enhancement to the current nurse residency program that includes clinical rotations in its educational foundation to solve job satisfaction issues among new graduate nurses. Evidence-based research indicates that clinical rotations benefit nurses early in their careers. These benefits include increased involvement in work, acquired additional skills, and enhanced job satisfaction in the workplace. This MSN project provides a clear path for implementing this unique type of residency program in nursing to be piloted at the University of Utah Hospital system in Salt Lake City. The new residency program format will be eight months and be implemented in a twenty-week timeline. The project implementation includes a project proposal, a PowerPoint presentation for staff and administration, and a lesson and assignment plan for educators in the program. Additionally, the project will provide pre- and post-surveys to test the understanding and success of project implementation and allow for future improvements and changes based on trends. The project also identifies ways to disseminate significant results and learned information to the university's stakeholders and other organizations nationwide. Providing clinical rotations to train and support new nurses can significantly impact the long-standing nurse retention crisis.

According to the Arizona Nurses Association, more than 50% of all new graduate nurses leave their first job and sometimes nursing altogether because of decreased orientation time and the shock that exists when they transition from student to professional nurse (AzNA, 2009). New graduate nurse turnover costs facilities $49,000 to $92,000 per nurse (Trepanier, Early, Ulrich, and Cherry, 2012). This impacts patient care as it interferes with staffing on units, continuity of care, and loss of staff productivity. The solution presented in the project is to institute nurse residency or internship programs in most facilities which will help to transition new nurses from student to competent and confident professional nurses who can provide safe, evidence-based care. This solution is based on results seen in extensive research from both quantitative and qualitative research studies. This solution will decrease the costs related to nursing turnover. This project details what needs to happen to implement a nurse residency program as well as how it will be evaluated and how the information will be disseminated to key stakeholders and other healthcare professionals.

Book excerpt: "New graduate nurse residency programs' primary function is to provide new graduates with clinical experience and bridge the gap between student and competent registered nurse (Walsh, 2018). While the focus of these programs is on teaching new graduates, a gap in practice has been identified when it comes to the education of the nurses that are training these new graduates. The purpose of this program evaluation was to increase preceptor feelings of preparedness for the precepting role, as well as increase preceptor education attendance rates to at least 50% of eligible nurses. This was a program evaluation with a pre-/post-test design that evaluated the impact of preceptor education in 10 registered nurses. Participants submitted surveys to evaluate their feelings of support and preparedness for their educator roles before and after the educational presentation. There was a statistically significant increase in post-education scores (M=11.2, SD=2.15) when compared to pre-education scores (M=6.6, SD=2.8); t(9) = -5.81, p=0.00025597. Furthermore, 100% of respondents had a higher total score on the post-survey than the pre-survey with a mean difference of +4.6 points +/-2.5 (SD), indicating that they felt more supported and prepared to act as preceptors after the education. By offering a preceptor education curriculum that is easily accessible, organizations will see increased attendance rates and therefore higher feelings of support and preparedness from nurse preceptors. These preceptors will then be able to teach and empower new RNs on their journey to practice, offering a smoother and safer transition to professional independence, and decreasing burnout rates and turnover costs to the healthcare organization (Trepanier et al., 2012)"--Abstract.

This increase in insured Americans necessitates the increase of primary care providers. Nurse Practitioners (NPs) are the key players in alleviating the healthcare workforce shortage that our society is facing today. However, the increased demand for primary care providers creates a challenge for new graduate nurse practitioners entering the primary care setting. Increased patient demand limits the support given to new graduate nurse practitioners that can potentially affect the NP’s successful transition into practice and ultimately job satisfaction and retention rate in an organization. The purpose of this grant is to evaluate the relationship of a nurse practitioner residency program (NPRP) on job satisfaction and retention rate. This study will be conducted in thirty-five federally qualified health center (FQHC) that offers a NPRP. Participants for this study will include new graduate family NPs enrolled in the NPRP for year 2016-2017. At the conclusion of the NPRP, two survey instruments will be administered to NPs to determine their job satisfaction and intent to stay, as a predictor for retention. Data will be examined to analyze the assumption of the researcher that participating and completing a NPRP is positively related to the job satisfaction and the retention of new graduate family NPs. In effect, supporting the need for NPRP programs which will facilitate NP transition into practice and increase the number of NPs who will continue in primary care, alleviating the workforce shortage.

Research has shown the necessity and importance of the implementation of a nurse residency program, not only for new graduate nurses, but for the healthcare institution as well. A convenience sample of new graduate nurses participating in the Cohort Eight Nurse Residency Program were surveyed before the start and following a 6 month program. The data suggests that after participating in the 6 month long Nurse Residency Program, new graduate nurses were apt to be more knowledgeable on skill acquisition, more satisfied in UC San Diego Medical Center with fewer uncertainties in their professional practice, and their turnover intention had decreased from the presurvey results.

Book excerpt: Nurse Residency Programs (NRPs) were introduced to address the gap that exists between entry-level nurses' skill proficiency, confidence, critical thinking ability, comfort, and to improve the graduate nurses' ability to offer safe and competent care. However, there is only a small amount of literature to support that NPRs help entry-level nurses in developing confidence and competence. This Direct Practice Improvement Project was developed to assess the impact of NRPs on the confidence and competency of entry-level nurses. Benner's novice to expert theory and Duchscher's organizational theory of learning and experiential learning, served as the theoretical framework for this project. The quantitative project used a pretest/posttest design to determine whether or not the participation in an NRP was directly related to improved confidence in practice and improved competence, as measured by self-reported medication errors. The project was based on a case study of the Vizient/ American Association of Colleges of Nursing (AACN). Nurse Residency Program. It employed the convenience sampling method, and all the 26 nurse residents in the program were selected to participate in the project (N=26). The Wilcoxon Signed-Rank Test demonstrated a significant increase in the number of new nurse graduates with improved confidence, after the participation in the residency program (p= 0.05). The project was inconclusive concerning improvement in competence, as measured by medication errors, due to the non-existence of errors at the point of entry. Practical implications for practice include the incorporation of nurse residency program into the undergraduate curricula, and the adoption programs that are at least three months long.
