Melioidosis is one of the most neglected tropical diseases (NTDs), so much so that it is not even included in the WHO list of NTDs. Yet modeling suggests that it kills more people worldwide every year than diseases that are much better known, such as leptospirosis and dengue. The reasons for this under-recognition are numerous, including the fact that it mainly affects the disadvantaged rural poor in areas that are poorly supplied with the diagnostic capability to make the diagnosis. In 22 separate articles, expert authors from around the world have summarized what is known about the burden of the disease in humans and animals and the presence of the causative bacterium in the environment in their countries or regions. They have also identified the main obstacles and challenges to establishing the true burden, and to ensure that patients receive accurate diagnosis and optimal care for this all too frequently fatal disease. Rather than focusing on the theoretical risk of the use of Burkholderia pseudomallei as a biological weapon, this book highlights its importance as a clear and present danger to global public health.

Book excerpt: Although the wide influence of Soren Kirkegaard's writings upon modern Christian thought is now generally recognized, the insights of the Danish knight of faith have often been misread and misinterpreted by those who seek to canonize their own theologies by appealing to Kierkegaard. The fact that modern Christian thought owes much to Kierkegaard does not mean that modern Christian thought speaks fo Kierkegaard at all points. In this short study, Carnell has allowed Kierkegaard to speak for himself, thus providing a valuable glimpse into the thought of the great Danish philosopher-theologian. Restricting the number of critical comments (including a short closing chapter entitled Yes and No), Carnell has written a short survey of Kierkegaard's thought with generous documentation from his writings. The two major theses which the author discusses are existential living and Truth is subjectivity. He elucidates the latter thesis by explaining the four affirmative elements in Kierkegaard's idea of subjective truth: faith, suffering, hope, and love. Carnell concludes by stating that because Kierkegaard strove to enrich the body of Christ by developing a fresh interpretation of spiritual truth, his books should be read, and read again. This volume will do more than encourage readers to do that; it will help them when they do.

Book excerpt: This dissertation explores the changing media environment through the study of the media ecology of two rural midwestern communities using qualitative methods. Building from Friedland's (2001) community communication model, this study analyzes how media structure place-based communities and what is lost as media decline. The contrasting case studies-one community rich in local media, the other facing a thin media environment-illuminate an important function of local media: they organize information about a community to bring that community into view, which results in community members identifying as part of that community. This process of synthesis and binding cannot be replaced by regional or social media, which emerge as dominant media forms when local media decline. Rather, when media lose their structuring role in place-based community, the burden shifts to individuals to navigate and make sense of a complex informational environment with consequences for place-based democratic community.

Its central defining feature is the prominence it accords to genes in the explanation of evolutionary dynamics. Since the advent of the 21st century, however, the Modern Synthesis has been subject to repeated and sustained challenges. These are largely empirically driven. In the last two decades, evolutionary biology has witnessed unprecedented growth in the understanding of those processes that underwrite the development of organisms and the inheritance of characters. The empirical advances usher in challenges to the conceptual foundations of evolutionary theory. The extent to which the new biology challenges the Modern Synthesis has been the subject of lively debate. Many current commentators charge that the new biology of the 21st century calls for a revision, extension, or wholesale rejection of the Modern Synthesis Theory of evolution. Defenders of the Modern Synthesis maintain that the theory can accommodate the exciting new advances in biology. The original essays collected in this volume survey the various challenges to the Modern Synthesis arising from the new biology of the 21st century. The authors are evolutionary biologists, philosophers of science, and historians of biology from Europe and North America. Each of the essays discusses a particular challenge to the Modern Synthesis treatment of inheritance, development, or adaptation. Taken together, the essays cover a spectrum of views, from those that contend that the Modern Synthesis can rise to the challenges of the new biology, with little or no revision required, to those that call for the abandonment of the Modern Synthesis. Book excerpt: Widely considered the preeminent Chinese woman poet, Li Qingzhao (1084-1150s) occupies a crucial place in China’s literary and cultural history. She stands out as the great exception to the rule that the first-rank poets in premodern China were male. But at what price to our understanding of her as a writer does this distinction come? The Burden of Female Talent challenges conventional modes of thinking about Li Qingzhao as a devoted but often lonely wife and, later, a forlorn widow. By examining manipulations of her image by the critical tradition in later imperial times and into the twentieth century, Ronald C. Egan brings to light the ways in which critics sought to accommodate her to cultural norms, molding her “talent” to make it compatible with ideals of womanly conduct and identity. Contested images of Li, including a heated controversy concerning her remarriage and its implications for her “devotion” to her first husband, reveal the difficulty literary culture has had in coping with this woman of extraordinary conduct and ability. The study ends with a reappraisal of Li’s poetry, freed from the autobiographical and reductive readings that were traditionally imposed on it and which remain standard even today.

To support decision-makers in shaping context-specific diet and nutrition policies, this review provides a summary of the NCD burden and activities in the Region; outlines the NCD-related health benefits of the MD and ND; describes interventions and policies in 15 countries; reviews four identified studies into the effectiveness of MD and ND policies on NCD outcomes; and discusses policy implications and options. In the context of MD and ND interventions for NCDs, there remains a Region-wide need to increase translation of evidence into action, monitor and evaluate the impact of existing policies on NCD outcomes and share activities through public platforms to support information sharing.

This book, opening with these words, presents an entirely new theory of the process of design. In the first part of the book, Christopher Alexander discusses the process by which a form is adapted to the context of human needs and demands that has called it into being. He shows that such an adaptive process will be successful only if it proceeds piecemeal instead of all at once. It is for this reason that forms from traditional un-self-conscious cultures, molded not by designers but by the slow pattern of changes within tradition, are so beautifully organized and adapted. When the designer, in our own self-conscious culture, is called on to create a form that is adapted to its context he is unsuccessful, because the preconceived categories out of which he builds his picture of the problem do not correspond to the inherent components of the problem, and therefore lead only to the arbitrariness, willfulness, and lack of understanding which plague the design of modern buildings and modern cities. In the second part, Mr. Alexander presents a method by which the designer may bring his full creative imagination into play, and yet avoid the traps of irrelevant preconception. He shows that, whenever a problem is stated, it is possible to ignore existing concepts and to create new concepts, out of the structure of the problem itself, which do correspond correctly to what he calls the subsystems of the adaptive process. By treating each of these subsystems as a separate subproblem, the designer can translate the new concepts into form. The form, because of the process, will be well-adapted to its context, non-arbitrary, and correct. The mathematics underlying this method, based mainly on set theory, is fully developed in a long appendix. Another appendix demonstrates the application of the method to the design of an Indian village.
