Book excerpt: Excerpt from The British Journal of Dermatology, Vol. 21: January-December, 1909 Summary - The histology of the condition is characterised by a lifting up en masse of the horny layers of the epithelium from a rapid exudation of serum tending to collect into lakes on the surface, which exudate is Obviously the result of the striking (edema notice able in the prickle-cell layer, which in its turn is due to the vascular dilatation prevalent in the dermal and hypodermal vessels. These facts, together with the presence of cocci on the surface, probably due merely to accidental contamination, and the absence of any leucocytic infiltration, constitute the most striking histological features. If the exudation of polymorphonuclear leucocytes is considered to be an essential Sign of inflammation, then Ritter's disease is not a dermatitis. Beyond this, however, there are no facts to bring forward, and whilst the thrombosis hypothesis is a possibility, it is, as I am well aware, beset with many difficulties which must be met before it can be accepted. Book excerpt: Excerpt from The British Journal of Dermatology, Vol. 19: January-December, 1907 He cannot give a very reliable account of its onset, but he said that he believes it came on the front of the chest first, and shortly afterwards on the other places (forehead and upper extremity). He also thinks that the nodules came out all together, not in crops or successively, that the whole thing started almost at once, but gradually became more pronounced and prominent. There has been no increase, at any rate for the past year. His attention was not drawn to the eruption by any subjective symptoms; he had no discomfort, pain, or itching. The only subjective symptoms which he has had, and which appear to be trifling, are that on a cold day the pressure of his hat causes a little soreness, hardly amounting to pain, and that in the winter on a cold morning the cold water of his bath occasions some smarting pain in the skin of the front of the chest; sea bathing has the same effect. Pressure on the spots causes no discomfort. The eruption is present on the left side of the forehead, the front of the upper part of the thorax, the left shoulder, arm, and forearm, in the skin over the left scapula, and very slightly over the right shoulder and right scapula. Forehead. - The area involved is about 2 inches in breadth; it extends from the left eyebrow to the border of the hair. It is bounded on the right side by the central line of the forehead; the lesions do not reach all along quite to this central line, but none transgress it. The inner half of this area is the most prominent. It consists of pale-red nodules, mostly about 4 mm. in diameter, some coalescing with each other to form conglomerate nodules. The nodules are very firm to the touch; where they are massed the skin is much thickened. This tract narrows towards the upper part, and close to the hair it terminates in a single nodule, 4 mm. in diameter, which forms, as it were, the apex of this prominent tract. The outer half of the forehead area consists of scattered nodules, mostly of the colour of the skin; in places between these nodules the skin surface is uneven, as though growth was commencing beneath the surface. I propose, in the first place, to define exactly what I understand by the term "mole." Clinically, they are seen as circumscribed patches on the skin, which differ greatly in size, appear, as a rule, soon after birth, rarely at birth, grow larger during childhood, and generally remain stationary for the remainder of life. Book excerpt: Excerpt from The British Journal of Dermatology, Vol. 26 Case 1.- A man, aged 25 years, was shown by Mr. Shillitoe before the Derma tological Section of the lloyal Society of Medicine, June loth, 1011 (the coloured illustration is from this case), as a case of Granuloma inguinale. The patient consulted Mr. Shillitoe first on March 10th, 1911, and gave the following history: "Just before last Christmas he developed multiple sores around the corona (soft sores) and a bubo which was opened on January 10th, and which had not healed on his arrival in England. He left Singapore on February 17th, having already developed the three ulcers still present in his left groin. He was told he had syphilis, and advised to have salvarsan. During the voyage home he took 150 tablets of 8 minims each of iodine together with local applications of pure carbolic, silver nitrate, etc., without any marked effect on the ulcerations. He had been four years in the East and only really ill with malaria during the first year. Ten years ago he had albuminuria following typhoid fever. The urine had been periodically examined since, without any fresh appearance of albumen until early in February. "When Mr. Shillitoe saw him on March 16th the urine was acid, sp. gr. 1024, and a decided amount of albumen. He had three superficial ulcerations in the left groin and an unhealed bubo. The bases of the ulcerations, which were not punched out, were glazed, devoid of granulations, and discharged a thin sanious fluid; the edges were rounded and firm. Mr. Shillitoe did not consider them to be specific, nor could he find other evidence of syphilis., "March 23rd: Wassermann's reaction positive; no albumen, so he was put under mercury. "March 31st: The sores not healing, Mr. Shillitoe gave him in addition sanatogen internally and externally. "April 12th: The sores were gradually closing and granulations were appearing-, which bled readily. He has increased one pound in weight, now 10 st. 12 1b.; there was a small trace of albumen aud Herpes preputialis. "April 2lst and again on 24th: 0-59 grin, of salvarsan were given intravenously.
