Book excerpt: Although there was consensus that the system should continue to be needs based but needed reform, the concept of path dependency explains why reform remains stalled. It was found that the ideas of the policy elites are "stuck" in what has been created. The ideas and interests supported the existing institutional framework that privileges the public funding of medically necessary hospital services and physician services. Policy decisions that have been made at an earlier time have limited the choices and influenced the ideas of the policy elites on what should be funded under Medicare. The sustainability of the Canadian model of health care known as Medicare is the subject of intense public debate and has focused on updating the determination of what services should be publicly funded. Provincial/territorial health insurance plans must fully cover all medically necessary hospital and/or physician services. Even though there have been numerous reports over the last few decades identifying similar themes, reform has been stalled. The objective of this thesis was to assist in the policy process by ascertaining the views of policy elites from key stakeholder groups across Canada on what should be "in" and "out" of the publicly funded system. Using a non-experimental, cross-sectional design, a self-administered questionnaire (available in both French and English) was distributed between January and April, 2002, to policy elites involved in health policy discussions, either as health providers (doctors, nurses, pharmacists, home care providers, hospital/health authorities) or as business. The results are based on 2,523 responses. The ability of government to "buy real change" will largely depend upon its ability to mediate the scope of conflict within the existing institutional framework. Even incremental changes could prove contentious, as evidenced by the views on funding insured hospital services regardless of the site of delivery. A shift to a more holistic approach that would include an expanded funding role for government could be even more contentious, as the path of the medical model is so solidly established in governmental regulations and funding frameworks. Successful reform will need to address, rather than assume, a broader view of health.

Book excerpt: In light of its recognition of the OMA as the exclusive bargaining agent for physicians, the Ontario Ministry of Health and Long-Term Care (MOHLTC) has historically consulted and negotiated with the OMA regarding the "tariffs" or fees to be paid to physicians for the provision of publicly funded services. [...] Prior to the election of the Ontario Progressive Conservative Party in 1995, the relationship between the OMA and the MOHLTC was negotiated through "Framework Agreements".7 Two framework agreements had been successfully negotiated, the first covering the period of 1991-1994, and the second covering the period of 1994-1997. [...] Page 7 of 25 The government membership of the PSC is selected from the Provider Services Branch of the MOHLTC.28 Under the agreements a professional facilitator, chosen by the parties, facilitates the PSC.29 Each party funds its own members, and the MOHLTC funds the administration costs of the Committee and the cost of the facilitator. [...] Under the 2000 Agreement, the agenda is determined by the facilitator in consultation with the co-chairpersons appointed respectively by the MOHLTC and the OMA. Book excerpt: In 1996, the Institute of Medicine (IOM) released its report Telemedicine: A Guide to Assessing Telecommunications for Health Care. In that report, the IOM Committee on Evaluating Clinical Applications of Telemedicine found telemedicine is similar in most respects to other technologies for which better evidence of effectiveness is also being demanded. Telemedicine, however, has some special characteristics-shared with information technologies generally-that warrant particular notice from evaluators and decision makers. Since that time, attention to telehealth has continued to grow in both the public and private sectors. Peer-reviewed journals and professional societies are devoted to telehealth, the federal government provides grant funding to promote the use of telehealth, and the private technology industry continues to develop new applications for telehealth. However, barriers remain to the use of telehealth modalities, including issues related to reimbursement, licensure, workforce, and costs. Also, some areas of telehealth have developed a stronger evidence base than others. The Health Resources and Service Administration (HRSA) sponsored the IOM in holding a workshop in Washington, DC, on August 8-9 2012, to examine how the use of telehealth technology can fit into the U.S. health care system. HRSA asked the IOM to focus on the potential for telehealth to serve geographically isolated individuals and extend the reach of scarce resources while also emphasizing the quality and value in the delivery of health care services. This workshop summary discusses the evolution of telehealth since 1996, including the increasing role of the private sector, policies that have promoted or delayed the use of telehealth, and consumer acceptance of telehealth. The Role of Telehealth in an Evolving Health Care Environment: Workshop Summary discusses the current evidence base for telehealth, including available data and gaps in data; discuss how technological developments, including mobile telehealth, electronic intensive care units, remote monitoring, social networking, and wearable devices, in conjunction with the push for electronic health records, is changing the delivery of health care in rural and urban environments. This report also summarizes actions that the U.S. Department of Health and Human Services (HHS) can undertake to further the use of telehealth to improve health care outcomes while controlling costs in the current health care environment.
