If the patient's symptoms improved, but the lesions did not Show improvement, the case was not considered improved. Unimproved means that the patient's lesions or symptoms or both were not improved and that the lesions steadily progressed despite treatment. Some of the cases showed such marked improvement under treat ment at first that although they subsequently died it was deemed important to call attention to this class of cases by tabulating them under the heading of primarily improved, subsequently died. Our experience has led us to expect certain cases to do very well under treatment at first and raise great hopes of recovery in the patient and the therapeutist. Such cases have frequently been reported in the literature during this stage of improvement, and not uncommonly the patients have relapsed and died by the time the reports have been published. Some cases are improved but still unde'r treatment, so that their ultimate results are not known. These are classified in the column improved, still under treat ment. The human body obtains its energy and nutrition from three types of foodstuffs: proteins, fats, and carbohydrates. For gen eral purposes of work, warmth, and nutrition all three groups serve well; the muscular tissues are, however, more specific in their requirements. The working cells of the muscles do not utilize each type of foodstuff alike; they find the sugars the most useful for their energy production. It is possible, but not very probable, that they can use other materials for the same purpose, but this in too slight an extent to make the substitution of any practical importance, and we may, therefore, for the present leave this possibility out of our calculations. Book excerpt: Excerpt from American Journal of the Medical Sciences, 1912, Vol. 144 Such a thing as natural constipation, in my Opinion, does not exist. Some reason must he looked for as a cause for the deficient evacuation. In childhood such reason is found occasionally in some anatomical malformation, but this is seldom. The common cause is neglect of the habit of evacuation, or, even more commonly, a wrong diet. When I looked over these boys I found absolutely no anatomical deformity, but on questioning the mother as to their habits and diet, I found that the custom of having stool at a special time of day had never been insisted upon, and the mother told me that she was very careful as to their diet and had always been. I was away on my vacation when consulted as to these boys, so had no means of making careful record of the physical findings, weight, urine, etc. Suffice it to say that the only abnormality found was a condition of inanition in both children. When the mother told me how she was feeding them, into the details of which it is unnecessary to enter, I was not surprised at their physical state. They were being fed on foods which had almost no residue, and they were not receiving enough even of such food. To the joy of both boys 1 increased their diet considerably, giving both foods which had more food value, and what is just as important, a sufficient amount of the fodder vegetables to produce enough bulk to the residue to stimulate peristalsis. I also pre scribed the amount oi water to be taken daily and the times at which it should be taken. I instructed the mother to have the boys go to stool at the same time each day and try to have a passage. If this did not come without great straining, to give a small enema of soaped water to start it. I also ordered that all laxative medicines be stopped. After the third day the enema was not needed. It is over a year since this advice was given, and the boys have neither of them required a laxative Since. I have several such: cases on record, so I feel sure that we can put down as one of the chief etiological factors in constipation, especially in children, a lack of sufficient bulk to the residue of the food, and, as another, neglect of the habit of evacuation. The former can readily be supplied by the use of a sufficient amount of the fodder vegetables, and the latter by discipline. Book excerpt: Excerpt from The American Journal of the Medical Sciences, 1919, Vol. 158 Depending upon the locality, the etiological factors producing pneumonia may be expected to show variation. From experience in army base hospitals, two facts of importance have been rather universally recognized. The first fact refers to the general preva lence of the streptococcus (normally one of the hemolytic varieties) in pneumonia following measles. The second fact refers to the prevalence of the streptococcus, likewise one of the hemolytic varieties, as an organism associated in many localities with the pneumococcus in the production of epidemic pneumonia not pre ceded by measles or influenza, but as a distinct type in which the pneumococcus has appeared to play but a minor role. In certain localities, during the recent epidemic of influenzal pneumonia, the pneumococcus as the prevalent type of infection both in the acute disease and in the subsequent empyemas. In other localities the pneumococcus and streptococcus have been associated, while in others streptococcus pneumonias have been more frequently encountered than had been heretofore generally recognized. In the series of empyema here reported only those are included which required treatment by aspiration or operation. There were available for this report 35 empyema patients who recovered by repeated aspirations alone, while came to Operation. Those patients were not included who died within a few days of admission with an established diagnosis of empyema or those who died during the course of aspirations designed for their relief, but who, because of activity of the pneumonic process, acute bilateral empyema, purulent pericarditis, meningitis or other serious complications, were not considered reasonable operative risks. Book excerpt: Excerpt from The American Journal of the Medical Sciences, 1885, Vol. 90 In the preparation of this paper it has been my intention to present a full report of a case of retention cyst of the pancreas, which has recently come under my observation, and, at the same time, summarize, in a com pact form, the clinical history of similar cases which will serve as a basis for some general remarks on the surgical treatment of cysts of the pan creas. Case I. Cyst of Pancreas; Laparotomy; Recovery - vo12, set. 19, laborer, German, was admitted to Milwaukee Hospital Nov. 28, 1884. He is small for his age and not robust, but he claims that, with one excep tion, he has never been sick, and that no hereditary tendency to disease exists in his family. Five weeks ago, while enjoying perfect health, he was thrown from a wagon, striking the. Ground on thelett side of the abdo men, a heavy keg falling upon his back, and increasing the force of the fall. The pain felt immediately after the accident was confined to his back, at a point where he was struck by the keg, but it was not sufficient in intensity to prevent him from following his occupation as a mason's apprentice. In a few days, however, diarrhoea set in, which persisted for two weeks, greatly reducing his strength and weight. If he had any fever during this time it was not. Sufficiently severe to attract his attention. His appetite was not impaired, and, though he vomited occasionally, neither vomiting nor the diarrhoea seemed to be aggravated by the time of eating or the kind or variety of food. After two weeks he noticed a tumor in the left hypochondriac region, which was round, smooth, and painless. The tumor increased rapidly in size, and soon gave rise to a sensation of fulness in the stomach, and later on, to regurgitation and vomiting soon after meals. Appetite slightly impaired. At this time the patient was treated for a short time by my friend, Dr. F. H. Day, of nvauwatosa, Wis., who resorted to symptomatic treatment, and observing no improve ment, referred him to me for diagnosis, and, in case it should be deemed advisable, for surgical treatment.
