Actuellement, les connaissances, le plateau technique et les différentes méthodes d'anesthésie offrent au praticien un large choix, lui permettant d'assurer, dans la plupart des cas, une prise en charge optimale. Cependant, des échecs existent. Le défi de l'anesthésie moderne est de savoir les analyser et les identifier afin de trouver une solution satisfaisante et non traumatisante pour le patient. Ces échecs peuvent être imputables à une erreur de geste technique du praticien, aux variations anatomiques ou encore à un état pathologique ou physiologique du patient. Dans certains cas, une médication utilisée à bon escient peut être indiquée. L'expression de la douleur et l'anxiété de certains patients face à une intervention dentaire sont aussi des facteurs à prendre en considération. Ils ont amené au développement dans notre spécialité de certaines techniques telles que le MEOPA, récemment arrivé dans les cabinets dentaires, ou d'autres issues de la « médecine parallèle » comme l'hypnose ou l'acupuncture.
