In this book we reviewed insulin secretion in both healthy individuals and in patients with type 2 diabetes. Because of the risk associated with progression from insulin resistance to diabetes and cardiovascular complications increases along a continuum, we included several chapters on the damage of endothelial cells in type 2 diabetes and genetic influences on endothelial cell dysfunction. Cardiovascular complications occur at a much lower glucose levels, thus a review on the oral glucose tolerance test compared to other methods was included. The medical conditions associated with type 2 diabetes such as pancreatic cancer, sarcopenia and sleep disordered breathing with diabetes were also discussed. The book concludes with several chapters on the treatments for this disease offering us hope in prevention and successful alleviation of the co-morbidities associated with obesity and type 2 diabetes.

Le tabagisme chez les patients diabétiques entraîne une augmentation du risque cardio-vasculaire et du risque de développer des complications. L'étude pose la problématique du vécu des patients diabétiques de type 2 concernant le diabète et son association au tabac. Méthode : La méthode employée était une méthode qualitative avec la réalisation d'entretiens individuels semi-structurés. Les patients recrutés étaient diabétiques de type 2 et fumeurs, anciens fumeurs ou n'ayant jamais fumé. Résultats : Les patients évoquaient surtout les changements à faire en termes d'alimentation et d'activité physique mais avaient des difficultés à les mettre en pratique. La découverte de la maladie était vécue de façon brutale et différemment selon les patients. La minimisation du diabète par les patients traités par antidiabétiques oraux était omniprésente. Les patients ne comprenaient pas toujours leur maladie car ils étaient asymptomatiques. Les connaissances de l'association diabète et tabac étaient très vagues. Les patients ne voulaient pas connaitre les risques qu'ils encouraient. Ils ne trouvaient pas l'association inquiétante et la cigarette était vécue comme un défouloir. Ils retrouvaient en la cigarette le plaisir « perdu » avec le régime diabétique. Conclusion : Les patients diabétiques de type 2 ont encore des interrogations concernant leur maladie. Les intervenants médicaux et paramédicaux doivent intensifier leur éducation. Les risques de l'association diabète et tabac ne sont pas connus des patients. Ils voient en l'arrêt du tabac une restriction supplémentaire. Le défi est de trouver des motivations qui inciteraient ces patients à cesser de fumer.
