What happened? In The Fault in Our SARS, evolutionary epidemiologist Rob Wallace catalogs the Biden administration's failures in controlling the outbreak. He also shows that, beyond matters of specific political persona or party, it was a decades-long structural decline associated with putting profits ahead of people that gutted U.S. public health. COVID-19 isn’t just an American tragedy. Each in its own way, countries around the world following the "profit-first" model failed their people. Book excerpt: Immunization against disease is among the most successful global health efforts of the modern era, and substantial gains in vaccination coverage rates have been achieved worldwide. However, that progress has stagnated in recent years, leaving an estimated 20 million children worldwide either undervaccinated or completely unvaccinated. The determinants of vaccination uptake are complex, mutable, and context specific. A primary driver is vaccine hesitancy - defined as a "delay in acceptance or refusal of vaccines despite availability of vaccination services". The majority of vaccine-hesitant people fall somewhere on a spectrum from vaccine acceptance to vaccine denial. Vaccine uptake is also hampered by socioeconomic or structural barriers to access. On August 17-20, 2020, the Forum on Microbial Threats at the National Academies of Sciences, Engineering, and Medicine held a 4-day virtual workshop titled The Critical Public Health Value of Vaccines: Tackling Issues of Access and Hesitancy. The workshop focused on two main areas (vaccine access and vaccine confidence) and gave particular consideration to health systems, research opportunities, communication strategies, and policies that could be considered to address access, perception, attitudes, and behaviors toward vaccination. This publication summarizes the presentations and discussion of the workshop.

Book excerpt: Self-care interventions are among the most promising and exciting new approaches to improve health and well-being, both from a health systems perspective and for people who use these interventions. The World Health Organization (WHO) uses the following working definition of self-care: Self-care is the ability of individuals, families and communities to promote health, prevent disease, maintain health, and cope with illness and disability with or without the support of a health worker. The scope of self-care as described in this definition includes health promotion; disease prevention and control; self-medication; providing care to dependent persons; seeking hospital/specialist/primary care if necessary; and rehabilitation, including palliative care. It includes a range of self-care modes and approaches. While this is a broad definition that includes many activities, it is important for health policy to recognize the importance of self-care, especially where it intersects with health systems and health professionals. Worldwide, an estimated shortage of 18 million health workers is anticipated by 2030, a record 130 million people are currently in need of humanitarian assistance, and disease outbreaks are a constant global threat. At least 400 million people worldwide lack access to the most essential health services, and every year 100 million people are plunged into poverty because they have to pay for health care out of their own pockets. There is an urgent need to find innovative strategies that go beyond the conventional health sector response. While "self-care" is not a new term or concept, self-care interventions have the potential to increase choice, when they are accessible and affordable, and they can also provide more opportunities for individuals to make informed decisions regarding their health and health care. In humanitarian settings, for example, due to lack of or limited health infrastructure and medical services in the crisis-affected areas, self-care could play an important role to improve health-related outcomes. Self-care also builds upon existing movements, such as task sharing, which are powerful strategies to support health systems.

In this context, the authors, members of the Faculty of Societal Safety Science, Kansai University, conducted many discussions and concluded that the major strategy is consistent independently of the type and magnitude of disaster or accident, being also the principle of the foundation of our faculty. The topics treated in this book are rather widely distributed but are well organized sequentially to provide a clear understanding of the principles of societal safety. In the first part the fundamental concepts of safety are discussed. The second part deals with risks in the societal and natural environment. Then follows, in the third part, a description of the quantitative estimation of risk and its assessment and management. The fourth part is devoted to disaster prevention, mitigation, and recovery systems. The final, fifth part presents a future perspective of societal safety science. Thorough reading of this introductory volume of societal safety science provides a clear image of the issues. This is largely because the Japanese have suffered often from natural disasters and not only have gained much valuable information about disasters but also have accumulated a store of experience. We are still in the process of reconstruction from the Great East Japan earthquake and the Fukushima nuclear power plant accident. This book is especially valuable therefore in studying the safety and security of people and their societies.
