Book excerpt: Introduction : l'arthroplastie de hanche, qu'elle soit totale (THA) ou intermédiaire (HA), est le gold standard du traitement des fractures intracapsulaires déplacées du col fémoral. Parmi les complications, l'instabilité est un risque prépondérant. L'utilisation d'une cupule à double mobilité (DM) réduit le risque de luxation, et l'hémiarthroplastie est encore utilisée. En l'absence de recommandation concernant le choix de la technique, notre objectif était de comparer l'arthroplastie totale de hanche avec cupule à double mobilité avec la prothèse intermédiaire dans le cadre de la traumatologie. Matériel et méthodes : nous avons identifié rétrospectivement 112 patients dans le groupe HA et 153 patients dans le groupe PTH DM ayant bénéficié d'une arthroplastie totale de hanche DM ou HA entre 2010 et 2013, en urgence pour fracture cervicale du col fémoral dans notre institution. Le recul minimum était de de 5 ans. L'âge moyen était de 81 ans (range, 55 - 101 ans). Les patients pris en charge en chirurgie programmée, les fractures sur matériel, les fractures pathologiques du col fémoral, les fractures associées, et les chirurgies de reprise ont été exclus. Le critère principal était l'instabilité. Le suivi clinique était réalisé par le score de WOMAC et le risque de mortalité était déterminé avec ajustement aux facteurs de confusion. Résultats : le taux de luxation à 5 ans était de 1.96% dans le groupe THA double mobilité et de 6.25% dans le groupe HA (p=0.070). La survie à la révision pour instabilité (p=0.015), révision toutes causes (p=0.034), réopérations (p=0.029) ainsi que les complications (fractures, infection) étaient meilleures dans le groupe DM. La mortalité, après ajustement aux facteurs de confusion, était significativement plus importante dans le groupe HA : 39.5% dans le groupe THA DM vs 75% dans le groupe HA (p
