Download or read book Migraine Surgery written by Thomas Muehlberger and published by Springer. This book was released on 2018-06-26 with total page 283 pages. Available in PDF, EPUB and Kindle. Book excerpt: Migraine surgery is the only existing treatment option that can lead to a permanent symptomatic improvement. Surgical techniques and advances in the understanding of the underlying mechanisms are evolving rapidly. This book intends to familiarise surgeons with the concept of migraine headaches and explains the surgical deactivation of trigger sites, which consists of much more than the decompression of pericranial nerves. The traditional notion of the brain as the sole origin of migraine is supplanted by a concept where the alteration of afferent signals in conjunction with peripheral and central sensitization can trigger a migraine attack. Migraine surgery targets the crucial role of extracranial tissues in the genesis of migraine which is corroborated by substantial clinical, anatomical and pathophysiological evidence.

Book Minimally Invasive Surgical Treatment of Migraine

Download or read book Minimally Invasive Surgical Treatment of Migraine written by Francesco Simonacci and published by . This book was released on 2022 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Migraine headache (MH) is a very common disorder affecting 10,Äì12% of the world,Äôs adult population. The first line therapy for migraine is usually a combination of conservative treatments but some patients seem to be refractory. For this group of patients, the minimally invasive surgical treatment of migraine might offer a solution. Migraine is usually caused by extracranial sensitive nerve compression due vascular, fascial or muscular structures nearby. The aim of migraine surgery is to relieve such compression at specific trigger points located in the occipital, temporal and frontal regions. From June 2011 until July 2019, we performed MH decompression surgeries in over 269 patients with either frontal, occipital, or temporal migraine trigger sites. In the occipital and temporal areas, nerve decompression was achieved by occipital and superficial temporal artery ligation, respectively. In patients suffering from frontal headache we performed both endoscopic nerve decompression and transpalpebral decompression. Among patient suffering from occipital migraine, 95% of them showed improvement of their condition, with 86% reporting complete relief. As concern temporal migraine, positive outcome was achieved in 83% of the patients (50% complete elimination and 33% partial improvement). In patient suffering from frontal migraine, positive results were observed in 94% of the patients (32% complete elimination, 62% partial improvement). Migraine is a common and debilitating condition that can be treated successfully with minimally invasive surgical procedure especially for those patients non-responding to medical therapies.

Book excerpt: A chronic migraine is a condition which is both prevalent and disabling, and which has a great impact on society as a whole. The accurate diagnosis, workup, and treatment of a migraine requires a multidisciplinary approach, involving primary care physicians, neurologists, pain management specialists, mental health specialists, alternative practitioners, and surgeons. Across the spectrum of migraine patients, the most challenging are those deemed refractory to conventional therapy. These patients may be surgical candidates, and are best served in a multidisciplinary, patient-centred model of migraine care. Migraine Surgery should serve as a reference to any practitioner dealing with the migraine patient. The fundamentals of migraine diagnosis and workup are reviewed, in addition to conventional therapies. We review the medical optimisation of the migraine patient, and establish what it means to have an intractable migraine. We discuss the role of the allied health professions in the management of chronic migraines, with chapters on psychological considerations, complementary medical management, and exercise. Furthermore, the standard interventional treatment of migraines is presented, in addition to surgical therapies, with an emphasis on patient selection, surgical technique, and complication avoidance.

Book Surgical Management of Migraine Headaches

Download or read book Surgical Management of Migraine Headaches written by Michael Chung and published by . This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Although migraines are experienced by a significant portion of the population, current medical therapies often fail to completely alleviate the symptoms of many migraine sufferers, leading to significant residual disability. Within the last decade, migraine surgery has arisen as a viable option for patients who have exhausted all other medical treatments. Despite early resistance in the headache care community, it has become more accepted due to an increasing number of studies demonstrating efficacy and safety, and increasing evidence supporting the influence of peripheral nerves in the progression of migraine pathophysiology. Yet, it remains crucial to carefully select appropriate surgical candidates based on the assessment of various factors such as medication use, pain distribution, and any other medical conditions that may contribute to headache. It is equally important to ensure that the patient has a strong relationship with a neurologist for optimal medical management and postoperative medical support. After the appropriate trigger sites are identified, various techniques can be used to decompress the involved larger nerves, avulse the smaller nerves, and address any intranasal pathology that could be causing migraines.
