Recent, though limited, experience with the antitoxin treatment of tetanus proves that our hospital mortality from this disease will be very materially reduced in both races by this treatment. Having sufficiently discussed the general infections of special importance, we shall now briefly consider from the comparative point of view the various surgical diseases of the splachnic cavities, the abdomen, the thorax, and the cranium, which are of interest to the student of racial pathology. Under this title we at once recognize one of the most frequent and formidable surgical diseases of tropical and sub-tropical regions. A comparison of the statistics of our hospital with those of similar institutions in the Antilles and in Africa, where the influence of equatorial conditions upon racial pathology can be studied with greatest advantage, is of interest in showing that civilization and climate have diminished the universally admitted immunity which the negro enjoys from intestinal infections in his own habitat. Knowing, as we all do, that malaria and infective diarrhoea and dysentery are the most potent predisposing causes of suppurative hepatitis, it will be well to glance at the status of the negro race of the present generation with reference to their liability to these diseases. The negro once could boast of his unsusceptibility to malaria and live secure in regions fatal to the white man. This is still true in Africa; but in this country this exemption has been growing less and less complete, and to-day the colored mortality from miasmatic diseases is very much greater than it once was (Corson). Wiener skillfully integrates the history of medicine with social and intellectual history in this study of how race and sex complicated medical treatment in the antebellum South. Sex, Sickness, and Slavery argues that Southern physicians' scientific training and practice uniquely entitled them to formulate medical justification for the imbalanced racial hierarchies of the period. Challenged with both helping to preserve the slave system (by acknowledging and preserving clear distinctions of race and sex) and enhancing their own authority (with correct medical diagnoses and effective treatment), doctors sought to understand bodies that did not necessarily fit into neat dichotomies or agree with suggested treatments. Focusing on Southern states from Virginia to Alabama, Weiner examines medical and lay perspectives on the body through a range of sources, including medical journals, notes, diaries, daybooks, and letters. These personal and revealing sources show how physicians, medical students, and patients--both free whites and slaves--felt about vulnerability to disease and mental illnesses, how bodily differences between races and sexes were explained, and how emotions, common sense, working conditions, and climate were understood to have an effect on the body. Physicians' authority did not go uncontested, however. Weiner also describes the ways in which laypeople, both black and white, resisted medical authority, clearly refusing to cede explanatory power to doctors without measuring medical views against their own bodily experiences or personal beliefs. Expertly drawing the dynamic tensions during this period in which Southern culture and the demands of slavery often trumped science, Weiner explores how doctors struggled with contradictions as medicine became a key arena for debate over the meanings of male and female, sick and well, black and white, North and South.

Book excerpt: In 1748, as yellow fever raged in Charleston, South Carolina, doctor John Lining remarked, "There is something very singular in the constitution of the Negroes, which renders them not liable to this fever." Lining's comments presaged ideas about blackness that would endure in medical discourses and beyond. In this fascinating medical history, Rana A. Hogarth examines the creation and circulation of medical ideas about blackness in the Atlantic World during the late eighteenth and early nineteenth centuries. She shows how white physicians deployed blackness as a medically significant marker of difference and used medical knowledge to improve plantation labor efficiency, safeguard colonial and civic interests, and enhance control over black bodies during the era of slavery. Hogarth refigures Atlantic slave societies as medical frontiers of knowledge production on the topic of racial difference. Rather than looking to their counterparts in Europe who collected and dissected bodies to gain knowledge about race, white physicians in Atlantic slaveholding regions created and tested ideas about race based on the contexts in which they lived and practiced. What emerges in sharp relief is the ways in which blackness was reified in medical discourses and used to perpetuate notions of white supremacy.

Unequal Treatment offers recommendations for improvements in medical care financing, allocation of care, availability of language translation, community-based care, and other arenas. Reading deeply in the personal letters, daybooks, diaries, bedside notes, and published writings of doctors, Stowe illuminates an entire world of sickness and remedy, suffering and hope, and the deep ties between medicine and regional culture. In a distinct American region where climate, race and slavery, and assumptions about "southernness" profoundly shaped illness and healing in the lives of ordinary people, Stowe argues that southern doctors inhabited a world of skills, medicines, and ideas about sickness that allowed them to play moral, as well as practical, roles in their communities. Looking closely at medical education, bedside encounters, and medicine's larger social aims, he describes a "country orthodoxy" of local, social medical practice that highly valued the "art" of medicine. While not modern in the sense of laboratory science a century later, this country orthodoxy was in its own way modern, Stowe argues, providing a style of caregiving deeply rooted in individual experience, moral values, and a consciousness of place and time.
