Book excerpt: Contemporary laryngology had its beginnings near the close of the 19th century and is probably best exemplified in the work of Morel McKenzie and of Czermak. Subsequent to their pioneering efforts, another surge of interest could be said to have centered about the efforts of the Chevalier Jacksons in the 1920's. After those bold steps, and for almost 40 years, research in laryngology and interest in laryngology continued, but at considerably and increasingly less intense levels, certainly so far as the otolaryngologist population was concerned. In the 1940's Julius Lempert sparked a renaissance in otologic research, deVelopment, and surgery, and exciting new frontiers opened in otology. In our own time, otology remains a large basic and fundamental segment of the otolaryngologic purview, but the flood of new discoveries which followed Lempert, like those which had followed the Jacksons in the 1920's in laryngology, appears to have diminished. When the authors of this publication made acquaintance in the late 1960's, there were approximately 10 centers in the United States for laryngologic research which could be truly designated as voice research facilities. The senior author was at that time instrumental in formulating the major criteria for laboratories to be so designated. In the early 1980's interest in laryngology has revived. At a recent meeting it was possible to list over 40 such laboratories which now were known to the authors over a broad geographic sweep, covering the entire United States.

Book excerpt: Since this book was first published, four years ago, there has been a considerable upsurge of interest in the field of both normal and abnormal voice production. Tangible evidence of this lies in the publication of the Journal of Voice in the United States, and in the UK the formation of the British Voice Association. This organization has attracted an increasing membership from professionals involved in all aspects of voice care and use - actors and singers, laryngologists and speech therapists, teachers and phoneticians. The Association holds regular study days, holds an annual two-day symposium, and publishes a Newsletter which attracts entries from this broad spectrum of professionals. We have also seen an increase in the number of specialist voice clinics, and in the two final chapters in this book a contrast is presented between such a specialist setting and the more typical clinic that operates in the majority of general hospitals. This last chapter now contains a breakdown of voice referrals over an eight-year period, which must represent a unique published study in this country. There still remains, however, little research into the management of voice disorders. There is clearly a need for more efficacy studies into specific treatment methods, and the single case-study designs developed in the field of aphasia would seem to be appropriate here.

Phonosurgical treatment for paralytic dysphonia was first established in the late 1970s in the form of arytenoid adduction and medialization laryngoplasty. It made possible the improvement of patients’ post-op voices, but it was difficult to regain patients’ own pre-paralysis voices. It has recently been established that immediate reconstruction of the recurrent laryngeal nerve during tumor extirpation is effective in recovery of their original voices in patients with unilateral vocal fold paralysis (VFP). The activity of the thyroarytenoid muscle is needed to recover normal voices. Nerve–muscle pedicle (NMP) flap implantation with a refined technique to the thyroarytenoid muscle is a novel method for that purpose. NMP flap implantation combined with arytenoid adduction was applied by the author to patients suffering from dysphonia and most patients did recover their nearly normal voices after surgery. This book provides readers with (1) what the currently prevalent surgical procedures are, (2) unsatisfactory results of these conventional procedures, (3) results of immediate recurrent laryngeal nerve reconstruction during tumor extirpation, (4) the outcome of delayed reinnervation combined with arytenoid adduction in patients with VFP and further, (5) the scientific basis that explains the reasons why the author’s method is effective in the recovery of patients’ own pre-paralysis, normal voices.
