Leprosy is a multifaceted systemic disease with variable presentation and clinical picture. Its identification may therefore not be straightforward, especially outside endemic areas. During its chronic course, leprosy is characterized by acute phases during which there may be exacerbation of symptoms and rapid progression of damage. When leprosy affects the eyes, nerves, and kidneys, it can represent a true medical emergency. The aim of this book is to make the reader familiar with the characteristic signs of disease, including abnormalities of the skin, nerves, eyes, hands, feet, testes, and bone. Early identification of the disease is critical to prevent patient disability and establish appropriate therapy. Type 1 reaction is associated with cellular immunity and particularly with the reaction of T helper 1 (Th1) cells to mycobacterial antigens. This reaction involves exacerbation of old lesions leading to the erythematous appearance. Type 2 reaction or erythema nodosum leprosum (ENL) is associated with humoral immunity. It is characterized by systemic symptoms along with new erythematous subcutaneous nodules.

It wasn't until the twelfth century, however, that the dreaded disease entered the collective psyche of Western society, thanks to a frightening epidemic that ravaged Catholic Europe. The Church responded by constructing charitable institutions called leprosariums to treat the rapidly expanding number of victims. As important as these events were, Timothy Miller and John Nesbitt remind us that the history of leprosy in the West is incomplete without also considering the Byzantine Empire, which confronted leprosy and its effects well before the Latin West. In Walking Corpses, they offer the first account of medieval leprosy that integrates the history of East and West.In their informative and engaging account, Miller and Nesbitt challenge a number of misperceptions and myths about medieval attitudes toward leprosy (known today as Hansen’s disease). They argue that ethical writings from the Byzantine world and from Catholic Europe never branded leprosy as punishment for sin; rather, theologians and moralists saw the disease as a mark of God’s favor on those chosen for heaven. The stimulus to ban lepers from society and ultimately to persecute them came not from Christian influence but from Germanic customary law. Leprosariums were not prisons to punish lepers but were centers of care to offer them support; some even provided both male and female residents the opportunity to govern their own communities under a form of written constitution. Informed by recent bioarchaeological research that has vastly expanded knowledge of the disease and its treatment by medieval society, Walking Corpses also includes three key Greek texts regarding leprosy (one of which has never been translated into English before).

Rod Edmond explores, in particular, how these reactions were refashioned in the modern colonial period. Beginning as a medical history, the book broadens into an examination of how Britain and its colonies responded to the believed spread of leprosy. Across the empire this involved isolating victims of the disease in 'colonies', often on offshore islands. Discussion of the segregation of lepers is then extended to analogous examples of this practice, which, it is argued, has been an essential part of the repertoire of colonialism in the modern period. The book also examines literary representations of leprosy in Romantic, Victorian and twentieth-century writing, and concludes with a discussion of traveller-writers such as R. L. Stevenson and Graham Greene who described and fictionalised their experience of staying in a leper colony.

Extending his inquiry from the first century to late in the eighteenth century, Demaitre draws on translations of academic treatises and archival records to illuminate the professional standing, knowledge, and conduct of the practitioners who struggled to move popular perceptions of leprosy beyond loathing and pity. He finds that, while not immune to social and cultural perceptions of the leprous as degenerate, and while influenced by their own fears of contagion, premodern physicians moderated society's reactions to leprosy and were dedicated to the well-being of their patients.
