Book excerpt: A principle problem of neuropsychology is to relate the neural structures damaged in traumatic brain injury with their functions in the cognitive tasks of daily life. This lecture reviews evidence that elementary operations of cognition as defined by cognitive studies are the level at which the brain localizes its computations. Orienting of visual attention is used as a model task. The component facilitations and inhibitions in visual orienting are related to neural systems through the study of focal neurological lesions. Visual orienting is a part of a more general selective attention system that also involves orienting to language. Our ability to be aware of and to act upon target events depends upon the connections of posterior orienting these pathways in studies of focal changes in cerebral blood flow during performance of language tasks. Although we do not have a general analysis of the mental operations performed by these anterior systems, there is some evidence relating the dorsolateral prefrontal and areas of the medial surface to aspects of focal selection. One way to study the generality of the attentional system developed in this lecture is to examine putative deficits of attention in disorders such as schizophrenia, depression and closed head injury where the organic basis for the deficit is largely unknown. Our preliminary studies of schizophrenia are used to support the utility of the joint functional and structural analysis proposed here.

That is, selective attention as a concept is typically reserved for the processes that allow for the prioritization of specific sensory input, while working memory entails more central structures for maintaining (and operating on) temporary mental representations. However, over the last decades various observations have been reported that question such sharp distinction. Most importantly, information stored in working memory has been shown to modulate selective attention processing – and vice versa. At the theoretical level, these observations are paralleled by an increasingly dominant focus on working memory as (involving) the attended part of long-term memory, with some positions considering that working memory is equivalent to selective attention turned to long-term memory representations – or internal selective attention. This questions the existence of working memory as a dedicated cognitive function and raises the need for integrative accounts of working memory and attention. The next step will be to explore the precise implications of attentional accounts of WM for the understanding of specific aspects and characteristics of WM, such as serial order processing, its modality-specificity, its capacity limitations, its relation with executive functions, as well as the nature of attentional mechanisms involved. This research topic in Frontiers in Human Neuroscience aims at bringing together the latest insights and findings about the interplay between working memory and selective attention.

Although your brain was receiving sensory input from this foot, you were not aware of your foot because you were reading and not attending to it. However, this discussion led you to move your attention to your left foot and to become aware of it. When I was a medical student, I saw a patient who was unaware of both the left side of his body and the left side of his environment. Unlike people in normal health, who when instructed can become aware of the left side of the body; this patient could not be made aware of his left arm or the left side of his environment. The patient's defect was so profound that despite being hungry he was unaware of food on the left side of his tray and did not recognize that his left arm belonged to him. This left-sided body and spatial unawareness could not be accounted for by a primary sensory defect. Although I knew that this man suffered from a large right-hemisphere stroke, I did not know the brain mechanisms that accounted for this profound example of unawareness. It was not until I had almost completed my neurology training in 1969 that I was able to return to this problem. At that time, most neuropsychological research was directed at understanding the language disorders associated with brain disease.

Book excerpt: The early years of modern experimental psychology were marked by a considerable amount of research on attention, and much work was carried out in the laboratories of Wundt, Titchener and Helmholtz. For various reasons, research on attention declined from 1920 until the 1950s. Under the early philosophy of behaviourism, attention became suspect as a ‘mentalistic’ concept. At the time of original publication in 1969, however, much work had been done to quantify and make objective research in this area. This was of increasing importance in a world dominated by communication networks, and ‘man-machine’ systems, in which the human element is the weakest link due to the limits on the rate at which man can handle information. Following the publication of Broadbent’s Perception and Communication in 1958, work on attention had begun to pour from an ever increasing number of laboratories. This book is dedicated to summarising what we knew, and attempts to survey the behavioural research in vision and hearing which throw light on how we share and direct attention, what are the limits of attention, to make some general methodological recommendations, to review current theories of the time, and to provide a guide to the relevant physiological work. As far as possible, work on memory has been omitted. A bibliography of the major work to the spring of 1969 is included.
