Clients are experts in their own recovery from SUDs. Counselors should engage them in collaborative partnerships. Ambivalence about change is normal. Resistance to change is an expression of ambivalence about change, not a client trait or characteristic. Confrontational approaches increase client resistance and discord in the counseling relationship. Motivational approaches explore ambivalence in a nonjudgmental and compassionate way.

A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. Book excerpt: Given the high prevalence of obesity with low rates of physical activity among Appalachian adolescents, it is critical to understand the factors that are associated with promoting healthy lifestyle change. Behavioral researchers have been continuously examining factors that directly influence physical activity behavior among adolescents. However, the relationship between physical activity, BMI, and self-regulation skills is not fully understood among adolescents at high risk of obesity. The purpose of this study is to determine if self-regulation skills for exercise and exercise self-efficacy mediate the association between BMI percentile and moderate-to-vigorous physical activity (MVPA) at baseline among a sample of 9th and 10th-grade Appalachian adolescent boys and girls. This study was a secondary analysis that examined pre-existing baseline data from high school adolescents participating in a behavioral intervention from an NIH funded group-randomized controlled trial. Participants completed a self-reported questionnaire to measure how well they use self-regulation strategies to influence their physical activity. The self-regulation subscales measured include self-monitoring, goal setting, social support, self-reward, time management, and overcoming barriers. Free-living physical activity was measured using the wGT3X-BT Actigraph accelerometer over a 7-consecutive day period. BMI percentile was calculated using measures of height and weight and referenced by the CDC age-and-sex-specific growth chart. The bootstrap resampling method was used to examine mediating effects between the variables of interest. At baseline, the sample of 9th and 10th grade adolescents report low rates of use of self-regulation strategies (mean = 2.17 – 2.74). Among Appalachian adolescents, BMI percentile was not significant to daily MVPA at baseline. Additionally, none of the self-regulation skills or exercise self-efficacy mediated the relationship between BMI percentile and daily MVPA among 9th and 10th-grade Appalachian adolescents. In general, there was no mediation detected due to the nonsignificant relationship between BMI percentile and the self-regulation skills or exercise self-efficacy. The results of this study provide a descriptive analysis of self-regulatory skills and MVPA before participation in a G-RCT behavioral intervention. While no mediating effects were determined, more research is needed to examine other health markers of obesity that affect self-regulatory skills to further understand the impact on physical activity behavior.

Book excerpt: The transtheoretical model of Prochaska and DiClemente (1982) was developed to explain when and how individuals change their behavior, as well as which factors influence these changes. According to this model, individuals progress through the five stages of change (precontemplation, contemplation, preparation, action, and maintenance) during the process of changing their behavior. The validity of physical activity stages of change instruments has been supported by comparisons with measures of self-efficacy, perceived barriers and benefits, and self-reported participation in physical activity. Stages of change measures have not been validated with objective measures of physical activity. The goal of the current study was to simultaneously evaluate the validity of three commonly used physical activity stage instruments using both a self-report and an objective measure of physical activity. Measures of self-efficacy, barriers, and benefits were also examined. Seventy-two participants completed all measures. Participants wore a small electronic activity monitor (Tritrac accelerometer) during waking hours for one week and completed a semi-structured interview (PAR) to assess their participation in physical activity during the same period of time. The Tritrac and PAR provided scores reflecting a participant's daily minutes in moderate to vigorous physical activity. Consistent with past research, Self-Efficacy, Barriers, Benefits, PAR, and Tritrac data were examined in separate one-way ANOVAs for each stage instrument. Book excerpt: The purpose of this cross-sectional exploratory study was to examine the relationships among self-efficacy, stage of change, and exercise behaviour in a sample of younger (Grade 9) and older (Grade 12) adolescents. A secondary objective of this study was to apply the transtheoretical model of Stage of Change, as a measure of intention to change, in order to discover the applicability of the model to an adolescent cohort in relation to exercise behaviour. This five-stage model is a self-report measure of an individual's readiness to adopt a new behaviour (e.g., regular exercise). The transtheoretical model incorporates Bandura's self-efficacy factor, which is purported to be a predictive measure of exercise behaviour and a covariant of stage. Exercise behaviour was measured with the Physical Activity Scale, and the University of Rhode Island Change Assessment Scale (URleA) was used to measure the stage of change and self-efficacy variables. The results of this study indicated significant differences between younger and older adolescents, and between males and females in their exercise behaviour. No significant differences were found for grade and gender on stage of change as measured by either a single-item question or a continuous measure of stage. Although grade and gender subgroups were not significantly different in their self-efficacy, significant interaction was found in the grade*gender variable.

Book excerpt: BACKGROUND: Adolescence is a critical period for the study of physical activity as significant declines are often reported throughout the adolescent years. The reduction in physical activity is of great concern, due to its strong association with increasing levels of obesity among today's youth. Understanding the trajectory of change in physical activity and the influences of change has potential for informing future interventions and policy. PURPOSE: The purpose of this research was to examine trajectories of physical activity across adolescence and then compare correlates derived from Sallis' framework (demographics, self-efficacy, screen time and peer support). The correlates were examined as baseline predictors as well as potential dynamic factors, examining how change in influential factors may be simultaneously associated with changes in physical activity. METHODS: The data are derived from a school-based behavioral intervention trial involving an ethnically diverse sample of 1543 seventh graders from three urban schools in the Midwest. Latent trajectory models were used to model individual developmental change in physical activity, as well as dynamic correlates across adolescence. These models were then combined to compare the changes in correlates with the changes in physical activity. RESULTS: While the change in the group mean for physical activity was small, the latent trajectory analyses revealed significant individual change over time. Age, gender and race were each found to be associated with these trajectories. All hypothesized dynamic correlates (i.e., self-efficacy, screen time) were also found to change across time; however, only the trajectories of self-efficacy and peer support were simultaneously associated with changes in physical activity. CONCLUSIONS: The unique and dynamic changes in both physical activity and its correlates in adolescence underscore the importance of studying them longitudinally. Further, latent trajectory analyses produce richer results than group level analyses due to studying the individual differences. Children who establish patterns of high physical activity are more likely to retain these patterns through adolescence. However, developmental changes in perceived efficacy and peer behavior clearly play a strong role as well. This study can inform both family and school practices by establishing regular opportunities for physical activity in the years leading up to adolescence.
