Within this system of care, healthcare professionals, including audiologists, can only practice safely and effectively if they possess an appreciation of the complexities and challenges that exist in that context. Where healthcare professionals have such awareness that aids them to recognise opportunities for errors that can cause patients harm and where they take steps to prevent these mistakes is where preventive audiology is positioned. This edited book, Complexities and Challenges in Preventive Audiology: An African Perspective, is a sequel to another book by the current editor titled Preventive Audiology: An African Perspective. While in the process of editing that book, the editor identified that a lacuna of contextually relevant collation of evidence on complexities and challenges faced by the field of audiology within the African context in implementing preventive audiology existed. The goal of this book is to delve into these complexities and challenges for various key areas in audiology. All chapters deliberate on evidence-based perspectives grounded in the African context, with deliberate and preferential reliance on contemporary locally relevant evidence that allows for accurate reflection of current complexities and challenges in ear and hearing care delivery within the African context. Contributors were encouraged to be as comprehensive as possible in their review of the literature within the African context, where available. Complexities brought about by context, such as cultural and linguistic diversity as well as traditional and alternative healthcare, on preventive audiology within the South African context, are also covered in this book. As each chapter explores prevailing complexities and challenges, potential solutions and recommendations for all challenges identified are also offered, having carefully and deliberately engaged with local evidence, local context, and local policies and regulations to ensure an Afrocentric contribution to the world of evidence. All chapters in the book have a goal of ensuring that increased efforts are directed towards the provision of clinical services that are driven through best practice by contextually relevant and responsive evidence.

Book excerpt: The CGIAR Research Program on Aquatic Agricultural Systems (AAS) is being implemented in ten communities in the Barotse floodplain of Zambia’s Western Province. With a focus on the rural poor and vulnerable, the AAS program aims to reduce poverty and improve food security by harnessing the development potential, productivity and diversity of aquatic and agricultural systems. The development challenge in the Barotse floodplain, as identified by relevant stakeholders, is to make effective use of seasonal flooding patterns and natural resources through more productive and diversified aquatic agricultural management practices that improve the lives and livelihoods of the poor and vulnerable. Food and nutrition are essential to support the overall AAS program objective and overcome the specific development challenge of the Barotse floodplain. Zambia has very high malnutrition rates, particularly for stunting in children under five. Poor nutritional status, especially of women and children, inhibits individual growth and development, and negatively impacts the overall health, productivity and economic potential of a community. The purpose of this report is to analyze the food and nutrition security situation within the Barotse floodplain. It explores multiple sectors, including nutrition, agriculture, health, and gender, at the national, provincial and community level to provide a comprehensive understanding of food and nutrition in the ten AAS communities in relation to the country as a whole. The analysis will provide informative inputs to the AAS Barotse hub design process to develop an appropriate food and nutrition research-in-development agenda.

This book looks at the economic, social and political geographies of Wales, which in the context of devolution and public service governance are hotly debated. It offers a novel ‘new localities’ theoretical framework for capturing the dynamics of locality-making, to go beyond the obsession with boundaries and coterminous geographies expressed by policy-makers and politicians. Three localities – Heads of the Valleys (north of Cardiff), central and west coast regions (Ceredigion, Pembrokeshire and the former district of Montgomeryshire in Powys) and the A55 corridor (from Wrexham to Holyhead) – are discussed in detail to illustrate this and also reveal the geographical tensions of devolution in contemporary Wales. This book is an original statement on the making of contemporary Wales from the Wales Institute of Social and Economic Research, Data and Methods (WISERD) researchers. Book excerpt: Since 2004, the U.S. government has supported the global response to HIV/AIDS through the President's Emergency Plan for AIDS Relief (PEPFAR). The Republic of Rwanda, a PEPFAR partner country since the initiative began, has made gains in its HIV response, including increased access to and coverage of antiretroviral therapy and decreased HIV prevalence. However, a persistent shortage in human resources for health (HRH) affects the health of people living with HIV and the entire Rwandan population. Recognizing HRH capabilities as a foundational challenge for the health system and the response to HIV, the Government of Rwanda worked with PEPFAR and other partners to develop a program to strengthen institutional capacity in health professional education and thereby increase the production of high-quality health workers. The Program was fully managed by the Government of Rwanda and was designed to run from 2011 through 2019. PEPFAR initiated funding in 2012. In 2015, PEPFAR adopted a new strategy focused on high-burden geographic areas and key populations, resulting in a reconfiguration of its HIV portfolio in Rwanda and a decision to cease funding the Program, which was determined no longer core to its programming strategy. The last disbursement for the Program from PEPFAR was in 2017. Evaluation of PEPFAR's Contribution (2012-2017) to Rwanda's Human Resources for Health Program describes PEPFAR-supported HRH activities in Rwanda in relation to programmatic priorities, outputs, and outcomes and examines, to the extent feasible, the impact on HRH and HIV-related outcomes. The HRH Program more than tripled the country's physician specialist workforce and produced major increases in the numbers and qualifications of nurses and midwives. Partnerships between U.S. institutions and the University of Rwanda introduced new programs, upgraded curricula, and improved the quality of teaching and training for health professionals. Growing the number, skills, and competencies of health workers contributed to direct and indirect improvements in the quality of HIV care. Based on the successes and challenges of the HRH program, the report recommends that future investments in health professional education be designed within a more comprehensive approach to human resources for health and institutional capacity building, which would strengthen the health system to meet both HIV-specific and more general health needs. The recommendations offer an aspirational framework to reimagine how partnerships are formed, how investments are made, and how the effects of those investments are documented.
