The distinction between national health services and social insurance systems is not real, but an abstract formulation which makes a wealth of information more manageable. Choosing these countries makes sense not because they are somehow representative of general types but because, between them, they are indicative of particular sets of problems in the politics of health and health care. The working assumption here is that the public provision of health care is embedded in a distinctively European politics.

The developments they analyze cover a time span from the initiation of modern national social policies at the end of the nineteenth century to the present. The experiences of all the presently existing Western European systems except Spain and Por-tugal are systematically encompassed, with com-parisons developed selectively with the experi-ences of the United States and Canada. The devel-opment of the social security systems, of public expenditures!and taxation, of public education and educational opportunities, and of income inequal-ity are described, compared, and analyzed for varying groupings of the Western European and North American nations. This volume addresses itself mainly to two audi-ences. The first includes all students of policy problems of the welfare states who seek to gain a comparative perspective and historical under-standing. A second group may be more interested in the theory and empirical analysis of long-term societal developments. In this context, the growth of the welfare states ranges as a major departure, along with the development of national states and capitalist economies. The welfare state is interpreted as a general phenomenon of modernization, as a product of the increasing differentiation and the growing size of societies on the one hand, and of processes of social and political mobilization on the other. It is an important element of the structural convergence of modern societies by its mere weight in all countries and at the same time a source of divergence by the variations within its institutional structure.

Citizens have seen their pensions, unemployment benefits, and general healthcare policies shrink as “belt tightening” measures are enforced. But in contrast, long-term care has seen a general growth in public financing, an expansion of beneficiaries, and, more generally, an attempt to define larger social responsibilities and related social rights. The aim of this book is to describe and interpret the changes introduced in long-term care policies in Western Europe. The volume argues that recent reforms have brought about an increasing convergence in LTC policies. Most of the new programs have developed a new general approach to long-term care, based on a better integration of social care and health care. The book explores increasing public support given to family care work (in the past, the family would take care of the elderly or infirm) and increasing growth and recognition of a extended social care market (by which care has shifted from a moral obligation based on family reciprocity to a paid, professional activity). A new social care arrangement has therefore been developing in Western countries, based on a new mix of family obligations, market provision, and public support. In order to understand such changes, this analysis will take into account the social and economical impact of these reforms.
