Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. In a decade and a half, Sweden has experimented with different health care modalities. As far as we know, relatively little research has been done to assess how these financial incentives, over time, have affected Sweden and the views of physicians. This work analyzes the implications of these reforms on the future structure of the well-envied health care of the Swedes.

Increased availability of large-scale longitudinal data and improved statistical methods have made it possible to address theoretically relevant questions about events such as entrance into the labour market, job mobility, divorce and death. This book consists of studies capturing the life-course from the cradle to the grave. The research questions include long-term consequences of childhood conditions; family formation and school-careers; work and parental leave; gender discrimination in job promotion; divorce and occupational career; persistence in poverty; and the intriguing question of why the highly educated tend to survive everyone else. The studies shed light on the relation between family and work, on gender inequality, social class differences, welfare state redistribution, and labour market processes. They do this in a particular context, namely Sweden in the post-war period that is, during the decades that formed one of the most advanced welfare states in modern history. One chapter provides a descriptive account of institutional and life-course change in Sweden during that period. Most authors use the Swedish level-of-living surveys, a unique data set providing ample opportunity to study social processes in a longitudinal perspective. The book will, therefore, be of relevance to those with interests in the Swedish welfare state as well as those with theoretical and reseacrh interests in the reproduction of inequality

By the 1970s it was considered a model of the successful welfare state, providing a broader and more elaborate system of social programs and security to more people than any other country, the centerpiece of which was its health care system. As Twaddle explains, however, by 1990 there was a significant shift in Sweden's health policy debates. Instead of speaking about the medical care system in terms of effectiveness, solidarity, and public planning, the discussions grew focused on competition, markets, and privatization, taking on more of the characteristics of the U.S. system. Twaddle explores the nature of the proposed changes in medical care, the context in which those changes were being proposed, and the steps that were taken to implement change. He concludes that the problem of market- oriented reforms in health care seems to be almost universal.

Although life expectancy and survival rates in the United States have improved dramatically over the past century, Americans live shorter lives and experience more injuries and illnesses than people in other high-income countries. The U.S. health disadvantage cannot be attributed solely to the adverse health status of racial or ethnic minorities or poor people: even highly advantaged Americans are in worse health than their counterparts in other, "peer" countries. In light of the new and growing evidence about the U.S. health disadvantage, the National Institutes of Health asked the National Research Council (NRC) and the Institute of Medicine (IOM) to convene a panel of experts to study the issue. The Panel on Understanding Cross-National Health Differences Among High-Income Countries examined whether the U.S. health disadvantage exists across the life span, considered potential explanations, and assessed the larger implications of the findings. U.S. Health in International Perspective presents detailed evidence on the issue, explores the possible explanations for the shorter and less healthy lives of Americans than those of people in comparable countries, and recommends actions by both government and nongovernment agencies and organizations to address the U.S. health disadvantage.

However, expenditure cuts with regard to the health care system are politically delicate.

However, separating the effects of climate from other effects presents a tremendous scientific challenge. Can we use climate and weather forecasts to predict infectious disease outbreaks? Can the field of public health advance from "surveillance and response" to "prediction and prevention?" And perhaps the most important question of all: Can we predict how global warming will affect the emergence and transmission of infectious disease agents around the world? Under the Weather evaluates our current understanding of the linkages among climate, ecosystems, and infectious disease; it then goes a step further and outlines the research needed to improve our understanding of these linkages. The book also examines the potential for using climate forecasts and ecological observations to help predict infectious disease outbreaks, identifies the necessary components for an epidemic early warning system, and reviews lessons learned from the use of climate forecasts in other realms of human activity.
