Book excerpt: This eighty-ninth volume of the IARC Monographs is the third and last of a series on tobacco-related agents. Volume 83 reported on the carcinogenicity of tobacco smoke and involuntary smoking (second-hand smoke or environmental tobacco smoke) (IARC 2004a). Volume 85 summarized the evidence on the carcinogenic risk of chewing betel quid with and without tobacco (IARC 2004b). That volume explored the variety of products chewed in South Asia and other parts of the word that contain areca nut in combination with other ingredients, often including tobacco. In this eighty-ninth volume, the carcinogenic risks associated with the use of smokeless tobacco, including chewing tobacco and snuff, are considered in a first monograph. The second monograph reviews some tobacco-specific nitrosamines. These agents were evaluated earlier in Volume 37 of the Monographs (IARC 1985) and information gathered since that time has been summarized and evaluated.

Of particular significance is a chapter on the governmental regulation of the marketing of tobacco that indicates an economic linkage between the new and the old worlds in the use of state-granted monopolies to market tobacco in Europe. The only study to make use of existing primary sources on tobacco consumption, marketing, and regulation, this volume is both a major contribution to the historical literature and an objective and readable account of consumer attitudes toward a substance whose impact on society continues to stir frequent and heated debate. Following an introductory chapter that provides a broad historical overview of smokeless tobacco consumption, Rogonzinski presents a brief, nontechnical description of the tobacco plant and its many varieties. He then explains the manufacturing processes that have been associated with each type of tobacco use and traces the early history of these practices through an examination of the literary evidence. Book excerpt: In June of 2009, President Obama signed into law historic legislation that gives the Food and Drug Administration authority to regulate tobacco products. With this law, the government has finally meaningfully addressed the pressing need for national legal regulation of tobacco products, a need that has been apparent for decades. In this article, I assess the new law in light of the means by which the tobacco industry made a deadly and highly addictive product - the cigarette - a defining feature of American society and culture. Numerous scholars have cogently documented and analyzed the industry's deceptive manufacturing and marketing practices. This article contributes to that literature by providing a theory describing the industry's strategy, which serves to underscore its extreme malleability and to justify substantial legal intervention. I call the strategy a “disinformation plus path-dependence” strategy, as it consists of (1) the pervasive dissemination of disinformation to encourage nonrational decisionmaking by consumers about product use, and (2) the subsequent deprivation of free choice on the part of those who become addicted to the products, even if the disinformation problem is corrected. After explaining how the industry used this strategy to sell cigarettes and the government's inadequate responses, I highlight the two most recent manifestations of the strategy that the industry has implemented in response to declining U.S. cigarette sales: heavy marketing of smokeless-tobacco products in the United States and of cigarettes in other parts of the world, particularly in developing countries. Given the nature of the industry's strategy, and, in particular, of these two recent manifestations, I argue that, although the new law is a significant step, it will not alone succeed in countering the industry's most recent or future manifestations of its strategy. I thus call for adoption of further national and transnational policy measures based on the lessons we have learned over our decades-long experience with the “disinformation plus path-dependence” strategy.

Book excerpt: Tobacco use is the leading cause of preventable death in United States, causing more than 440,000 deaths annually and resulting in $193 billion in health-related economic losses each year-$96 billion in direct medical costs and $97 billion in lost productivity. Since the first U.S. Surgeon General's report on smoking in 1964, more than 29 Surgeon General's reports, drawing on data from thousands of studies, have documented the overwhelming and conclusive biologic, epidemiologic, behavioral, and pharmacologic evidence that tobacco use is deadly. This evidence base links tobacco use to the development of multiple types of cancer and other life-threatening conditions, including cardiovascular and respiratory diseases. Smoking accounts for at least 30 percent of all cancer deaths, and 80 percent of lung cancer deaths. Despite the widespread agreement on the dangers of tobacco use and considerable success in reducing tobacco use prevalence from over 40 percent at the time of the 1964 Surgeon General's report to less than 20 percent today, recent progress in reducing tobacco use has slowed. An estimated 18.9 percent of U.S. adults smoke cigarettes, nearly one in four high school seniors smoke, and 13 percent of high school males use smokeless tobacco products. In recognition that progress in combating cancer will not be fully achieved without addressing the tobacco problem, the National Cancer Policy Forum of the Institute of Medicine (IOM) convened a public workshop, Reducing Tobacco-Related Cancer Incidence and Mortality, June 11-12, 2012 in Washington, DC. In opening remarks to the workshop participants, planning committee chair Roy Herbst, professor of medicine and of pharmacology and chief of medical oncology at Yale Cancer Center and Smilow Cancer Hospital, described the goals of the workshop, which were to examine the current obstacles to tobacco control and to discuss potential policy, outreach, and treatment strategies that could overcome these obstacles and reduce tobacco-related cancer incidence and mortality. Experts explored a number of topics, including: the changing demographics of tobacco users and the changing patterns of tobacco product use; the influence of tobacco use on cancer incidence and cancer treatment outcomes; tobacco dependence and cessation programs; federal and state level laws and regulations to curtail tobacco use; tobacco control education, messaging, and advocacy; financial and legal challenges to tobacco control efforts; and research and infrastructure needs to support tobacco control strategies, reduce tobacco related cancer incidence, and improve cancer patient outcomes.
