Book excerpt: Single-access laparoscopic surgery (SALS), performed using a single transumbilical access, has recently proved to bring advantages over other forms of laparoscopic surgery due to the reduction in the number of incisions and the consequent decrease in morbidity, less blood loss and postoperative pain. Furthermore, cosmesis may be improved because incision within the umbilicus renders surgery virtually scarless. Nevertheless, as with all new surgical concepts, questions may be raised regarding safety, usefulness and appropriateness. This book analyzes the use of SALS for different indications and abdominal surgical procedures based on a careful literature review of the experience of the most important surgical teams employing this approach and also the authors’ personal observations. The coverage is broad, encompassing, for example, the role of SALS in appendectomy, repair of abdominal wall defects, cholecystectomy, gastric surgery, nephrectomy, splenectomy, hemicolectomy and pancreatic, liver and gynecological surgery.

Book excerpt: Laparoscopic surgery is a common minimally invasive surgery that uses specialized tools to access the abdominal cavity and pelvic regions via small incisions called ports. Compared to open surgery, laparoscopy's small incision size better protects a patient's health and reduces recovery time. However, there is limited mobility in these procedures and additional training must be completed for surgeons to become proficient. To increase the freedom of motion and reduce the required skill for the surgeon, the Novel Single Incision, Free Motion (SIFM) Laparoscopic Surgical System was created. The SIFM device provides free motion of the tools with a single incision inside the body using electromagnets, and a motor-actuated cable system, and its design was explored through four experiments. Experiment 1 measured the frictional forces of different materials gliding over porcine skin tissue. Experiment 2 analyzed different permanent magnetic configurations to optimize the magnetic force between a tool on the inside of the abdominal wall and an external tool. The chosen configuration was a single-pole external magnet, coupled to an axially magnetized internal magnet. Experiment 3 analyzed the experimental and theoretical forces applied by the internal tool. The tool was able to provide sufficient cutting forces at 26.1 mm of separation between the tools. Experiment 4 measured the precision of the tool's end effector which was controlled by a stepper motor-powered cable system. The tool's end effector rotates no more than 1 degree about the y-axis and no more than 2 degrees about the x-axis. The SIFM system combines the health benefits of minimally invasive laparoscopic surgery, with the free motion and ease of open surgery. Additionally, there is limited training on the process of creating gateways for laparoscopic surgery. There are several challenges associated with placing and inserting trocars in the abdominal area to create the gateways for laparoscopic surgery. To address this issue, the Novel Trocar Training System is proposed to effectively simulate the haptic feeling of inserting a trocar into the abdomen. And model tissue for organs is tested as well by combining hydrogel alginate and silicon rubber.
