Book excerpt: "With health-care costs rising and an aging population, the health-care industry is progressively faced with the problem of growing demand and diminishing reimbursements. Hospital administration is often faced with a lack of quantifiable data regarding surgical suite capacity and the impact of adding new surgical procedures. With the inherent variation in surgery due to unique procedures and patients, accurately measuring maximum capacity in the surgical suite through mathematical models is difficult to do without making simplifying assumptions. Several hospitals calculate their operating room (OR) efficiencies by comparing total OR time available to total surgical time used. This metric fails to account for the required non-value added tasks between surgeries and the balance necessary for patients to arrive at the OR as soon as possible without compromising patient satisfaction. Since surgical suites are the financial engine for many hospitals and the decisions made with regard to the surgical suite can significantly impact a hospital's success, this thesis develops a methodology through simulation to more accurately define current and potential capacity levels within the surgical suite. Additionally, scheduling policies, which schedule patients based on the variability of their surgical time as well as the implementation of flexible ORs capable of servicing multiple operation genres, are examined for individual and interaction effects with regard to surgical suite capacity, patient waiting times, and resource utilization. Through verification and validation, the model is shown to be an effective tool in representing patient flow and testing policies and procedures within the surgical suite. An application to the surgical suite at Chenango Memorial Hospital (Norwich, NY) illustrates the methodology and potential impacts of this research"--Abstract.

Although simulation of health care processes is not new, few health care systems have used simulations as a basis for re-engineering the delivery of health services. There is growing appreciation that the complexity of health care processes exceeds the capacity of individual disciplines–health services research, health economics, or operations research–to guide health care reform. In this book, the authors focus on bringing the methodological rigor of evaluative research to the design and analysis of such simulation studies. The book is intended as a reference for health services researchers. Book excerpt: Emergency surgeries are serviced by three main forms of capacity: dedicated operating room time reserved for emergency surgeries, alternative (on call) capacity, and lastly, canceling of elective surgeries. The objective of this research is to model capacity implications of meeting wait time targets for multiple priority levels in the context of emergency surgeries. Initial attempts to solve the capacity evaluation problem were made using a non-linear optimisation model, however, this model was intractable. A simulation model was then used to examine the trade-off between additional dedicated operating room capacity (and consequent idle capacity) versus increased re-scheduling of elective surgeries while keeping reserved time for emergency surgeries low. Considered performance measures include utilization of operating room time, elective re-scheduling, and wait times by priority class. Finally, the instantaneous utilization of different types of downstream beds is determined to aid in capacity planning. The greatest number of patients seen within their respective wait time targets is achieved by a combination of additional on call capacity and a variation of the rule allowing low priority patients to utilize on call capacity. This also maintains lower cancelations of elective surgeries than the current situation. Although simulation does not provide an optimum solution it enables a comparison of different scenarios. This simulation model can determine appropriate capacity levels for servicing emergency patients of different priorities with different wait time targets.
