Mais c’est la psychanalyse qui scelle d’une manière singulière la relation entre le père et sa fille : « Je vois à présent, en te regardant, combien je suis vieux, car tu as exactement l’âge de la psychanalyse. Vous m’avez toutes deux causé des soucis, mais au fond j’attends quand même plus de joies de ta part que de la sienne », lui écrit-il à la fin de 1920. Cette comparaison montre à quel point, en ses commencements, la psychanalyse s’éprouve en famille et dans le cercle des initiés. Freud observe l'activité onirique de sa fille, une enfant tourmentée, avant de devenir à deux reprises, entre 1918 et 1924, son analyste. L’expérience est décisive. Anna s'implique dans l'International Psychoanalytical Association dès sa création, fréquente ses membres, se fait même courtiser par quelques élèves de son père. Mais, disciple fervente, elle se consacre à la thérapie des enfants et devient dans ce domaine la principale représentante de l'école viennoise face à sa grande rivale de l'école anglaise : Melanie Klein.
