Book excerpt: ABSTRACT: Background: Individuals living in rural areas may experience challenges in accessing affordable and appropriate medical care and support, often resulting in declines in health related quality of life. This is especially true for individuals diagnosed with HIV disease. While only one third of the US population lives in the south, over two thirds of all rural AIDS cases occur in this region. Fewer than 1,500 individuals are reported to be living with HIV/AIDS in the northern region of Florida commonly referred to as the Big Bend area. This predominantly rural region includes the eight counties located between the Gulf of Mexico and the Florida-Georgia border. Purpose: The purpose of this dissertation was to identify the health-related quality of life (HRQoL) status of people living with HIV/AIDS (PLHA) in the Big Bend area who receive case management services. Questions: The following questions are addressed: 1. What is the HRQoL status for PLHA residing in the Big Bend area receiving case management services? 2. How much do previously identified correlates account for variances in HRQoL among PLHA residing in the Big Bend area? Method: A cross-sectional descriptive survey method assessed the demographic characteristics and HRQoL variables for approximately 97 PLHA who reside in the Big Bend area and receive case management. Group comparisons were made based on demographic and socioeconomic status characteristics, behavioral factors, disease status, symptom status, and functional status. Significance: The results reveal that PLHA who receive HIV case management in the Big Bend area have mild symptom burdens, are only mildly functionally impaired, yet have poor quality of life. Few group differences were observed. Three significant findings were present. First, among PLHA who take highly active antiretroviral treatment (HAART), those with AIDS reported more concerns about the impact that HIV medications have had on their life than those who do not have AIDS. Secondly, those who report using tobacco products are less satisfied with their life than tobacco non-users and, more interestingly, those who report using tobacco products are less accepting of their HIV status than those who do not use tobacco, even when controlling for other variables. And finally, those who report using illicit drugs have significantly more concerns about family, friends, and coworkers finding out about their HIV status than those who do not report using illicit drugs. Implications for HIV case management and social work policy and practice are discussed.

Book excerpt: The introduction of new medical treatments in recent years, commonly referred to as highly active antiretroviral therapy, has greatly increased the survival of patients with HIV/AIDS. As patients with HIV/AIDS continue to live longer, other important health-related outcomes, such as quality of life (QOL), should be thoroughly studied. There is also evidence that racial/ethnic minorities are disproportionately affected by HIV/AIDS, but potential health disparities among individuals already infected with HIV/AIDS have not been adequately examined in ethnically diverse populations. The purpose of this dissertation was to: (1) examine the impact of both demographic and behavioral variables on functional status and overall QOL among a population of ethnically diverse and economically disadvantaged HIV/AIDS patients; (2) examine the psychometric properties of a functional status measure--the Household and Leisure Time Activities questionnaire (HLTA); and (3) assess a proximal-distal theoretical framework for QOL using a full structural equation model in a population of patients with HIV/AIDS. Analyses were performed using data collected in the fall of 2000 from the project, Health and Work-Related Quality of Life and Health Risk Behaviors in a Multiethnic HIV-positive Population . Investigators from The University of Texas M.D. Anderson Cancer Center, The University of Texas-Houston Medical School, and The University of Texas School of Public Health conducted this project. The study site was the Thomas Street Clinic (TSC), a comprehensive HIV/AIDS care facility funded by the Harris County Hospital District (HCHD). TSC provides HIV/AIDS care to a diverse population of approximately 4000 medically indigent residents of Harris County. A systematic, consecutive sampling procedure yielded a sample size of 348 patients. Findings suggested that overall QOL, work-role functioning, household functioning, and leisure time functioning were impaired in this patient population. Results from the psychometric evaluation indicated that the HLTA was a reliable and valid measure of household and leisure time functioning status in a low-income multiethnic HIV-positive population. Finally, structural equation modeling of the proximal-distal QOL model suggested that this model was not a viable representation of the relationship between the study variables in this patient population.

Book excerpt: Exploratory research questions revealed that social support significantly moderated the relationship between HIV/AIDS-related stigma and psychological distress, such that the strength of the association between HIV/AIDS-related stigma and psychological distress was lessened for PLWHA who reported higher levels of social support. Furthermore, perceived patient-centered cultural sensitivity in health care experienced was shown to have a significant moderating effect on the relationship between HIV/AIDS-related stigma and health-related quality of life, such that the strength of the association between HIV/AIDS-related stigma and health-related quality of life was lessened for PLWHA who reported higher levels of perceived patient-centered cultural sensitivity in health care experienced. Significant differences in HIV/AIDS-related stigma scores in association with gender and race but not sexual orientation were also found in the present study. Specifically, women had higher levels of HIV/AIDS-related stigma scores than men and African Americans/Blacks and other races had higher levels of HIV/AIDS-related stigma scores than Caucasian Americans/Whites. However, HIV/AIDS-related stigma scores did not differ significantly for heterosexuals, bisexuals, and gays/lesbians.

Book excerpt: Abstract: This cross-sectional, correlational study was conducted to examine the associations between engagement in the patient-provider relationship and quality of life and adherence among persons living with HIV/AIDS. The project is a sub-study of an international, multi-site study examining the adequacy of a self-care symptom management model that hypothesizes relationships among person, illness representation, symptom self-care, perceived effectiveness of self-care activities, adherence, and outcomes appraisal in a large, HIV-positive sample (n=1072). Method: A convenience sample of non-hospitalized persons receiving care at The Ohio State University Infectious Diseases Outpatient Clinic and Children's Hospital Family AIDS Clinic and Educational Services was recruited from May to November, 2003. IRB approval was obtained from each of the participating sites. A 35-page survey was administered to subjects qualifying for the study who provided informed consent. Subjects received 20 dollars for completing the questionnaire. All measures were self-report. Demographic data gathered included gender, age, race/ethnicity, length of HIV diagnosis, and presence of AIDS diagnosis. The sample included 57 persons living with HIV/AIDS with a mean age of 40 years, of which 28% were women and 58% were white; the mean length of HIV diagnosis was 10 years and 39% had an AIDS diagnosis. Engagement was measured using the 13-item Engagement with Health Care Provider scale. The HIV/AIDS Targeted-Quality of Life scale, a 42-item HIV-specific instrument, was used to measure the following nine dimensions of quality of life: overall function, life satisfaction, health worries, financial worries, medication worries, HIV mastery, disclosure worries, provider trust, and sexual function. Adherence, an assessment of compliance with provider advice and medication regimen, was measured with the 5-item Advice and Instruction scale and the 15-item AIDS Clinical Trials Group (ACTG) Reasons for Nonadherence to Medications checklist, respectively. Findings: Better quality of life was found to be significantly related to engagement with health care provider (p=0.013) as was adherence to provider advice (p=0.001). While positively correlated, no statistically significant relationship was found between engagement and adherence to medication. Implications: Results of this study support existing literature suggesting a positive association between increased engagement with health care provider and better quality of life among persons living with HIV/AIDS. A limitation of the study, particularly in regard to adherence, is the self-report design. Nevertheless, the study does suggest the need to develop and promote strategies that foster a healthy patient-provider relationship in this population to improve quality of life and adherence to treatment.

Book excerpt: Once considered a terminal illness, HIVAIDS has become a chronic illness as those infected are living longer. Given the longevity achievable with the current prophylactic and therapeutic strategies for people living with HIV/AIDS (PLWHA), quality of life has emerged as a significant measure of health outcome, and quality of life enhancement is an important goal. However, little is known about quality of life in impoverished developing countries such as Zambia. Furthermore, in Zambia, there is significant variation in the impact of the HIV pandemic, with much higher HIV prevalence rates occurring in women than men and in urban versus rural areas. The purpose of the study was threefold. First, the study was to test the quality of life factor structure of the Zambian sample using the World Health Organization Quality of Life HIV (WHOQOL-HIV) instrument. Second, the study was to determine quality of life of the Zambian sample using the resultant factor structure of quality of life. Last, the study was to examine the effect of gender and residence on the quality of life of the Zambian sample using resultant factor structure of quality of life. A cross-sectional descriptive, correlational, four-group 2x2 factorial design on a convenience sample of 160 PLWHA was conducted using both the WHOQOL-HIV Model and instrument. Factor analysis yielded three new scales: "Zambian WHOQOL-HIV, Zambian WHOHIV Medication Dependence, and Zambian WHOHIV Spirituality Religion Personal Beliefs (SRPB)" and validated the "Overall Quality of Life and General Health Perceptions" scale. The quality of life of the Zambian sample was found to be above average on all the four scales. There were no gender effects on any of the four scales. There was a significant medication dependence residence effect showing that urban participants were more dependent on medication than their rural counterparts. However, this effect disappeared when controlling for individual and clinical characteristics. There was a significant gender by residence interaction effect on spirituality/religion/personal beliefs (SRPB) showing that urban females and rural men had better SRPB than rural women and urban men. This effect was stronger when controlling for individual characteristics were controlled for, but disappeared when clinical characteristics were controlled for. The study also showed that symptomatic patients had lower quality of life and SRPB. The study is important to nursing because it tested the WHOQOL-HIV instrument among a Zambian sample which has not been done before. This study validated one scale that can be used regularly to assess "overall quality of life and general health perceptions" and provided three scales that can be used for comprehensive assessment of quality of life in order to monitor disease progression and response to care. These assessments of quality of life will lead to development of holistic nursing interventions based on an individual's perception of their quality of life.

Book excerpt: A cross sectional study looked at the association between sociodemographic factors, HIV/AIDS clinical history and health-related quality of life (QOL) in HIV/AIDS persons. It used the WHOQOL-BREF-THAI questionnaire completed by 3,596 patients during 2008 in three Thai hospitals. The questionnaire explored five QOL domains: Physical, Psychological, Social, Environment and Overall and it was for the first time administered to many HIV/AIDS patients in Thailand. Descriptive statistics and inferential statics (bivariate and multivariate : one-way ANOVA, Kruskal-Wallis Test, Pearson Correlation and independent ttest)) were used to test association between independent factors and QOL. Results: The following associations were all statistically significant. Males had higher QOL than females in Physical, Psychological and Overall domains. Younger age was associated with higher QOL in Physical and Social domains, older age in Psychological and Environment domains. Married had higher psychological QOL than single and separated. Good adherence to, more than 3 year duration of ARV treatment and not suffering AIDS related events were associated with higher QOL in all except social domains. VL
